Approval of Second Amendment to Administrative Services
Agreement between Blue Cross and Blue Shield of Florida and Monroe County Board of County
Commissioners to include Teladoc Services effective January 1, 2017 at an additional cost of $1 per
employee per month through December 31, 2020, which is estimated to cost approximately $19,200
per year for four years that equates to $76,800.
ITEM BACKGROUND:
As an alternative to seeking medical care at a local primary care
physician that requires time off work; or at an urgent care whose fees are more than primary care; or
at an Emergency Room (where the cost is much higher) for non-emergency conditions, we will be
providing our plan participants the services of Teladoc. This is a 24 hour/7 days a week/365 days
per year service and will be free for those plan participants who utilize the services beginning
January 1, 2017. This service is expected to yield savings to the Health Fund, by decreasing the
need of participants to seek after hour services at higher cost locations, such as urgent care or
hospitals.
During the September 19, 2016 BOCC Special Meeting the Board approved the addition of Teladoc
Services at the cost of $1.00 per employee per month (PEPM), changing the cost of the contract as
follows:
From $45.75 PEPM ($878,400) to $46.75 PEPM ($897,600) from 1/1/17 12/31/17
From $47.25 PEPM ($907,200) to $48.25 PEPM ($926,400) from 1/1/18 12/31/18
From $47.25 PEPM ($907,200) to $48.25 PEPM ($926,400) from 1/1/19 12/31/19
From $48.50 PEPM ($931,200) to $49.50 PEPM ($950,400) from 1/1/20 12/31/20
There is an expected enrollment of 1600 employees which equates to approximately $19,200 per
year for these services for the next 4 years ($76,800).

Cross and Blue Shield effective November 1, 2015 providing for services until 2020 with the Blue
CONTRACT/AGREEMENT CHANGES:
Amendment of Exhibit B to the Administrative Services Agreement between Blue Cross and Blue
Shield of Florida and the Monroe County Board of County Commissioners to include Teledoc
Services effective January 1, 2017. The fee will be $1.00 per employee per month.
