ITEM BACKGROUND: This Task Order is to provide data collection, design, bidding and
special services for the expansion project of the existing restroom in the departures hold area of
the concourse in the EYW terminal building. This project is to replace and expand existing
restrooms that have reached the end of their useful life. By renovating these restrooms, the
airport will have to expand the area in order to meet Florida Building Code 2014 requirements.
In addition, there is a need to accommodate service animals in order to be in compliance for
individuals with disabilities. As a result, this project will preserve capacity in the terminal to
accommodate passengers at the Key West International Airport (EYW), as described in Exhibit
A- Scope of Work.

SCOPE OF WORK
Jacobs understands that as-built clOCUrnentation of the existing facility and site is available. A
topographic survey, geotechnical investigation and Asbestos Renovation Survey will be required
to be performed,
ExISTING CONDITIONS
DESCRIPTION OF WORK
The data collection, design, bidding, and special services will provide the folloWing:
Article A - Data Collection

As requested, Tierra South Florida, Inc. (TSF) is pleased to submit this revised proposal for the
above- referenced proJect. Based on information provided by Jacobs, proposed construction will
consist of renovation and expansion of the Departure Hall Restroom, and adding a Pet Relief area
at the Key West International Airport.
PROPOSED SCOPE OF WORK
We propose to perform a one (1) Standard Penetration Test (SPT) boring to a depth of 40 feet
outside the existing building accessible to a truck mounted drill rig. The boring location will be
located in the field by Jacobs.
Upon completion of the field exploration, laboratory testing will be performed on selected samples.
A report will be issued that contains the exploration and laboratory data.
ESTIMATED FEES
It is proposed that the fee for the performance of the services outlined above is determined on a
unit price basis in accordance with the attached Fee Schedule and that the work be performed

pursuant to TSFs General Conditions enclosed herewith and incorporated into this proposal. On
the basis of the proposed quantities, it is estimated that the total fee will be approximately
$8,865.00.
Our estimate covers the work needed to present the results. Not included are reviews of drawings,
preparation of construction specifications, special conferences and any other work requested after
submittal of our report.
SCHEDULE AND AUTHORIZATION
TSF will proceed with the work after receipt of a signed copy of this proposal. With our present
schedule, we can commence work within several days of project approval (weather permitting)
and fieldwork is expected take about I day to complete. The results can be submitted in about two
weeks after completion of the field work,

We shall perform a topographic survey of existing conditions at Key West International to support
the design of the departure hall restroom renovation.
The limits of the survey are delineated in red on Exhibit A (shown below) and incorporated herein
for reference.

driveways, steps, gates, top of curb, bottom of curb, flow line, edge of pavement, centerline of
pavement, bottom of gutter, back of walk, face of walk, and above ground evidence of utility.
We will obtain rim elevations, pipe size, material and inverts of sewer and drainage structures
"here possible. Due to the nature of obtaining the data where the structures are in use, the
information obtained may be the best approximation of the true measurements. It is the

other resources prior to construction. Utility locations will include fire hydrants, water valves,
meter boxes, vaults and electrical outlets, power poles and overhead wires, We will locate wells.,
main irrigation valves, but will not locate individual sprinkler heads.
There is no provision for the excavation, probing or location of underground utilities structures or
improvements. Utilities shall be located to the extent that they are above ground and visible. This
proposal does not include the contracting of an underground utility locating service to locate the
subsurface utilities.
The survey data will be reported to the nearest 0.05 feet for horizontal locations and for the vertical
component, 0.1 feet spot elevations on ground shots and 0.03' spot elevations on paving or hard
Surfaces, with all work performed in accordance with the Standards of Practice as defined in
Chapter 5J -17, Florida Administrative Code.
Deliverables:
1. Provide four (4) signed and sealed hard copies of the topographic Survey,
2. Digital PDF files and AutoCAD 2014 or lower Ibnnat file of the above Survey.
* This proposal is based on one mobilization by our crew to accomplish the project.

We can mobilize to the site within one (1) week after receipt of an executed contract, and have the
