Monroe County
Key West International Airport (EYW)
Departure Hall Renovations — Restroorns and Pet Relief Station Design
SCOPE OF WORK
Jacobs will provide architectural, structural, plumbing, HVAC and electrical engineering
documents for the renovation of the existing departure hall approximately 1,500 square foot +/- to
include the men's restroom renovation, new family restroom, new janitor's closet, new women's
restroom and pet relief addition. The scope will also include the renovation of the adjacent
departure hall associated with the restroom renovations and a new security delivery / exit
vestibule expansion. See attached exhibit A. 113 (revised), A. 113-1 (revised) and A. IA-3 (revised).
These improvements will be located at the Key West International Airport in Key West, Florida.
All design work shall be designed in accordance with the Florida Building Code 2014.
Jacobs understands that as -built documentation of the existing facility and site is available. A
topographic survey, geotechnical investigation and Asbestos Renovation Survey will be required
to be performed.
EXISTING CONDITIONS
The current departure hall restrooms consist of a men's and women's restroom with a small
janitor's closet. The men's restroom consists of two water closets, two urinals and three
lavatories. The women's restroom consists of five water closets and 3 lavatories. The janitor's
closet includes a mop sink. The exterior area that will be impacted by the addition includes an
exterior concrete canopy supported by steel columns, a planter and concrete sidewalk pavement.
See attached Exhibit A.
DESCRIPTION OF WORK
The data collection, design, bidding, and special services will provide the following:
Article A — Data Collection
Field Verification: Visit and inspect the site (1 visit) to determine if existing conditions match
those provided by Monroe County. Take photographs and make written documentation, sketches,
notes or reports to confirm and record the general conditions of the existing facility and site.
Within this initial phase we will review all applicable requirements.
Initial Meeting: Conduct a meeting with representatives from Monroe County and the design A/E
team to include architect, mechanical engineer, plumbing engineer, electrical engineer and
structural engineer. The purpose of the meeting will be to brief the Jacobs team on the design
intent, discuss project features, and review submittal requirements with the design team, and
answer questions developed by the Jacobs team.

• Coordinate with design and geotechnical sub - consultants, as necessary.
• Coordinate with the topographic survey sub - consultant, as necessary.
• Coordinate with the asbestos survey sub - consultant, as necessary.
• Review available record drawings
• Perform a site investigation
Services for Article A will be provided under a lump sum basis.
Article B — 30% Design
The specific tasks Jacobs will perform are as follows:
A meeting with Monroe County will be held to review the 30% Design documents and
generate comments towards the next design phase. The 30% design will include the
following drawings:
• Set up drawing set.
• Title Sheet.
• General Notes, abbreviation and code information.
• Site Plan.
• Floor plans(s) drawn at an architectural scale that will allow the entire facility to be
shown on one sheet, without break -lines and with notes and dimensions.
• Reflected ceiling plan.
• Room finish plan.
• Roof plan.
• Typical building sections to show dimensions, proposed construction materials, and
relationship of finish floor to finish grades.
• Architectural exterior elevations.
• Structural foundation and roof framing plans (schematic).
• Mechanical plan (schematic).
• Electrical power and lighting plans (schematic).
• Plumbing plans (schematic).
• Fire protection plans (schematic).
• IT /Security plans (schematic).
• 30% construction order of magnitude cost estimate.
Services for Article B will be provided under a lamp sum basis.
Article C — 90% Design
The specific tasks Jacobs will perform are as follows:
After written authorization to proceed and based on the approved 30% Design Phase documents
and any adjustments in the scope or quality of the project or in the Fixed Limit of Construction
Cost authorized by Client, Jacobs shall prepare the Final Construction Documents setting forth in
detail the requirements for the construction of the project. During this work phase, we will
prepare all required construction documents for permit and construction of the building. We will
provide detailing for the completion of the building. This work effort will include architectural,
structural, mechanical (Plumbing and HVAC) and electrical plans.

A meeting with Monroe County will be held to review the 90% Design documents and
generate comments towards the final Bid Documents.
Documents shall include the following:
• Title sheet including index of drawings, abbreviations, schedule of material indications,
and schedule of notations and symbols.
• An area location map and legal description of property.
• Life - safety plans to show exit strategy, rated doors, emergency wall openings and ramps.
Fire extinguishers, emergency lighting, exit signs, smoke and fire dampers, and other life
safety- equipment relevant to the facility shall be indicated by symbol.
• Floor plans including: All dimensions and any target notes explaining the extent of work,
wall types, or other component, assembly or direction regarding the construction.
• Reflected ceiling plans.
• Room finish plans.
• Preparation of design presentation boards and presentations to Key West International
Airport architectural review committee.
• Exterior elevations showing location of the rain water leaders, openings and light
fixtures.
• Interior elevations showing all restroom elevations.
• Building and wall sections to establish vertical controls and construction types for the
Project.
• Details of the following: Doorjamb, head and sill conditions. Wall types. Window head,
sill and jamb conditions. Interior signage to include room identification, emergency
exiting and equipment signs, and any other items pertinent to the identification of the
project.
• Room finish and door schedules coordinated with the floor plans.
• Structural Engineering foundation plans, structural schedules and details.
• Structural analysis and calculations.
• Fire Protection Engineering: Detail of Fire Protection (suppression and fire alarm) design.
• Mechanical Engineering: Detail of HVAC system to communicate the design intent and
coordination with other disciplines. Detailed design of water and sanitary lines for the
new restroom and pet relief area design.
• Electrical Engineering: Drawings for lighting, including circuiting and luminaire
identification and switching. Convenience outlets and circuiting. Power systems and
equipment. Panel schedules, applicable installation details, general legend and
abbreviations.
• Security Design: Provide new hardware, access control and video surveillance for the
new exterior access / delivery vestibule. System shall be designed to be compatible with
the existing security and access control systems at the airport.
• 90% construction order of magnitude cost estimate.
Services for Article C will be provided under a lump sum basis.
Article D —100% Design
The specific tasks Jacobs will perform are as follows:

prepare the bid and construction documents for permit and construction of the building. We will
finalize the construction documents and incorporate final owner comments from the 90% review.
The construction documents will include signed and sealed architectural, structural, mechanical
(Plumbing and HVAQ and electrical plans.
Documents shall include the final drawings for following:
• Title sheet including index of drawings, abbreviations, schedule of material indications,
and schedule of notations and symbols.
• An area location map and legal description of property.
• Life - safety plans to show exit strategy, rated doors, emergency wall openings and ramps.
Fire extinguishers, emergency lighting, exit signs, smoke and fire dampers, and other life
safety- equipment relevant to the facility shall be indicated by symbol.

Project.

Services for Article D will be provided under a lump sum basis.
Article E — Bid Phase Services

Upon completion of the construction documents and at the request of the Airport, Jacobs will
prepare the project documents for bidding. Jacobs will perform bidding support services, the
following terms will apply:
• The County appoints Jacobs as its Agent, and Jacobs accepts such appointment to perform
bid /procurement services on JAA's behalf in connection with the project.
• Such services shall be performed under County's direction and in accordance to such
forms, terms and conditions, or modifications or revisions to same as County may in its
sole discretion at any time instruct Jacobs to use. All services shall be carried out in
accordance with the procedures mutually agreed upon by County and Jacobs.
• Jacobs shall not have authority to accept or bind the County in any way to changes,
modifications, revisions, alterations, amendments, or supplemental, additional, or
different terms and conditions (hereinafter referred to as "deviations ") which may be
submitted or requested by a vendor or contractor. Jacobs shall immediately submit any
deviations from the County's standard terms and conditions to the County for review by
the County's Purchasing Manager or their representative and such deviations shall not be
accepted by Jacobs unless Jacobs receives express written approval thereof from the
County's Project and /or Purchasing Manager or their representative.
• All documents issued by Jacobs hereunder shall be signed by Jacobs for the County if so
directed. The ownership and title of all items purchased hereunder shall pass directly
from the selling party to the County, and Jacobs shall at no time be a party to such
transaction other than as agent of the County. The County shall have the unilateral right
to have the commitment authority of Jacobs, its employee or this limited agency
authorization in its entirety revoked and cancelled at any time, with or without cause.
The County shall be obligated directly to the selling party for all payments for materials,
equipment, supplies and services procured hereunder.
• The County shall hold Jacobs and its employees harmless from any claims, suits or
liabilities arising out of any breach or other failure of performance by any contractor,
vendor or supplier under any contract or purchase order issued by Jacobs hereunder.
The following task will be performed as part of the bid phase services:
• Coordinate with the County Purchasing Department and the Airport to advertise for bids.
• Coordinate with the County to provide electronic copies of the bid documents to
prospective bidders
• Prepare for and attend a project pre -bid conference
• Prepare responses to address requests for information from prospective bidders
• Prepare and coordinate the issuing one (1) addendum with the County purchasing
department.
• Revise bid documents as necessary for the addendum
• Review bids for responsiveness
• Prepare a bid tabulation
• Make a recommendation for award based on review of the bid information submitted.

QUALIFICATIONS AND ASSUMPTIONS
For the purpose of providing a stipulated fee amount, this proposal has been based on the
following qualifications and assumptions:
• As -built documents will be provided by owner, including underground utilities.
• Design shall be based on previously approved planning / schematic design
documents attached. See attached exhibit A.1 B (revised), A.1 B -1 (revised) and
A. l A -3 (revised).
• Permit issuance related work such as processing plans through City, County,
and /or State agencies, other than plan revisions requested by such agencies, are
not included.
• Government Agency cost for all building and engineering permit fees are not
included in this proposal.
• Review of change orders is not included, as that will be part of the future
construction task.
• Construction administration services required by lending institutions and /or
prospective tenants in this project are not a part of our services in this phase.
• Preparation of close -out /warranty documents coordination is not included.
• Construction Administration Services are not included.
• Construction Administration does not include the preparation of As -Built
drawings.
• Jacobs will provide electrical coordination of Voice / Data & Communications
systems. (Voice / Data & communications system designs by others.)
• Life Cycle Cost Analysis is not included.
• The testing scope of services along with the construction testing is by others.
• Lightning protection will not be provided.
• Water fire flow test is not included.
• Preparation of 3D renderings and /or marketing brochures are not included.
• Architect /Engineer construction inspections are not included. Inspection services
for building / permitting inspections is not in our scope (no inspectors on staff),
none are anticipated by the architect / engineer.
• Proposal is based on the existing electrical systems having adequate capacity to
properly serve the renovation and addition. No major modifications to the
existing electrical systems are included in this proposal.
• Attendance and /or presentation to public hearings and /or other meetings with
municipal agencies are not included.
• Municipal Agency and all associated permit fees are to be paid by the client.
Jacobs is not responsible for obtaining any permits. Permits are to be obtained by
the contractor. Jacobs will monitor the contractor's progress through the permit
process for the client and provide follow up periodically with the permitting
agency to verify the status, but it is ultimately the responsibility of the contractor
to obtain all permits.
• Printing of plans shall be treated as reimbursable direct expenses and are
included as part of this proposal.
• Services do not include "design to cost ". Jacobs will assist the owner in
evaluating and offering cost reduction strategies as requested by client. The
"'design to cost" is basically noting that if Jacobs receives direction from the
client or one of its sub - consultants on how to design the project and if any

changes are required due to the project being over budget, then the re- design
required to bring the project back within budget is not a part of the lump sum fee
and shall be treated as additional services.
• It is assumed that the existing utilities (with required capacity) are available
adjacent to the renovation and addition.
• Other services not described in the scope of work.
Items identified as not included or limited in the scope of services can be provided or
expanded upon as additional services under a separate agreement or supplemental to this
agreement.
REFERENCES
The client will need to provide the following documentation to Jacobs before commencing with
our scope of work.
• As -built documentation of the existing facility, site and underground utilities.
• Construction Budget.
• Program requirements.

We anticipate completion of the deliverables in accordance with the following schedule:

Dear Ryan:
As requested, Tierra South Florida, Inc. (TSF) is pleased to submit this revised proposal for the
above - referenced project. Based on information provided by Jacobs, proposed construction will
consist of renovation and expansion of the Departure Hall Restroom, and adding a Pet Relief area
at the Key West International Airport.
PROPOSED SCOPE OF WORK
We propose to perform a one (1) Standard Penetration Test (SPT) boring to a depth of 40 feet
outside the existing building accessible to a truck mounted drill rig. The boring location will be
located in the field by Jacobs.
Prior to drilling at the project site, TSF will notify the local utility companies and request that
underground utilities be marked. Our experience, however, is that the utility companies will not
mark privately owned utilities. Private utilities will be the owner's /Airport's responsibility. Our
proposal assumes that private utility lines will be located in the field by others prior to
mobilization of the drill rig. TSF will recommend a utility line locating service upon request.
We believe that significant coordination will likely be required to perform the field operations. We
expect the field work will be performed at or close to the street. Therefore, it is likely that field
work will be performed at night time, and will require Maintenance pf Traffic (MOT). We have
included MOT in our fee estimate.
Upon completion of the field exploration, laboratory testing will be performed on selected samples.
A report will be issued that contains the exploration and laboratory data.
ESTIMATED FEES
It is proposed that the fee for the performance of the services outlined above is determined on a
unit price basis in accordance with the attached Fee Schedule and that the work be performed

pursuant to TSF's General Conditions enclosed herewith and incorporated into this proposal. On
the basis of the proposed quantities, it is estimated that the total fee will be approximately
$8,865.00.
Our estimate covers the work needed to present the results. Not included are reviews of drawings,
preparation of construction specifications, special conferences and any other work requested after
submittal of our report.
SCHEDULE AND AUTHORIZATION
TSF will proceed with the work after receipt of a signed copy of this proposal. With our present
schedule, we can commence work within several days of project approval (weather permitting)
and fieldwork is expected take about 1 day to complete. The results can be submitted in about two
weeks after completion of the field work.

Tierra South Florida's General Conditions
1. SCOPE OF WORK: Work means the specific geotechnical, analytical, testing or other service to be performed by Tierra South Florida, Inc. (TSF) as set forth in TSF's proposal, Client's acceptance of the scope of
work and these General Conditions. Additional work ordered by Client shall also be subject to these General Conditions. "Client" refers to the person or business entity ordering the work to be done by TSF Client
shall communicate these General Conditions to each and every third parry to whom Client transmits any part of TSF's work. TSF shall have no duty or obligation to any third party greater than that set forth in TSF's
proposal, Client's acceptance of TSF's proposal and these General Conditions. The ordering of work from TSF, or the reliance on any of TSF's work, shall represent acceptance of the terms of TSF's proposal and
these General Conditions, regardless of the terms of any subsequently issued document.
2. RIGHT -OF -ENTRY -The client will provide right -of- -entry for TSF and all necessary equipment in order to complete the work. While TSF will take all reasonable precautions to minimize any damage to the property,
it is understood by Client that in the normal course of work some damage may occur; the correction of which is not part of this agreement
3. DAMAGE TO EXISTING MAN -MADE OBJECTS -The Client, will provide the location of all underground utilities or obstructions to TSF who, in the prosecution of their work, will take all reasonable precautions
to avoid damage or injury to any such subterranean structure or utility. The Owner agrees to hold TSF harmless for any damages to subterranean structures which are not called to TSF attention and correctly shown
on the plans furnished and will reimburse TSF for any expenses in connection with any claims or suits including reasonable attorney fees at the trial and appellate levels.
4. IN -PLACE MATERIALS TESTING -TSF will not be responsible for repair or damage to portions of structures designated for in -place materials testing. Repairs can be made for aesthetic reasons if requested in
advance of the work to be performed. The cost for labor and materials would be charged.
5. SAMPLE RETENTION -TSF will retain all soil and rock samples obtained for geotechnical explorations for 30 days, Samples subjected to Construction Materials and Laboratory testing are disposed of subsequent
to testing. Further storage or transfer of samples can be made at Client's expense upon written authorization.
6. DEFINITION OF RESPONSIBILITY (OBSERVATION SERVICES) - The presence of our field representative will be for the purpose of providing observation and field testing Our work does not include
supervision or direction of the actual work of the contractor, his employees or agents. The contractor for this project should be so advised.
6 1 The Contractor should also be informed that neither the presence of our field representative or the observation and testing by our firm shall excuse him in any way for defects discovered in his work It is
understood that TSF will not be responsible for the Contractor's job or site safety on his project. That will he the sole responsibility of the contractor.
7. STANDARD OF CARE - Service performed by TSF under this Agreement will be conducted in a manner consistent with that level of care and skill ordinarily exercised by members of the profession currently
practicing under similar conditions. No other warranty, expressed or implied, is made.
TI Client recognizes that subsurface conditions may vary from those encountered at the location where borings, surveys or explorations are made by TSF and that the data, interpretations and recommendations
of TSF are based solely on the information available to it TSF shall not be responsible for the interpretation by others of information developed.
S. ORAL AGREEMENTS -No oral agreement, guarantee, promise, representation or warranty shall be binding
9. OWNERSHIP OF DOCUMENTS -All reports, boring logs, field data and notes, laboratory test data, calculations, estimates and other documents prepared by TSF, as instruments of service, shall remain the property
of TSF until final payment is received and a letter of copyright transfer been executed
10. BASIS OF PAYMENT - Payment is due within 30 days of date of invoice. Payments not made when due shall bear interest at eighteen (1 S) percent annum or at the maximum rate allowed by law from the date of the
invoice until same is paid.
to I Ifthe Client fails to make any payment due to TSF for service and/or expenses within 60 days of date of invoice, TSF may, after giving seven days' written notice to Client, suspend services until all outstanding
amounts have been paid to TSF in full. Further, TSF may, in addition to withholding services, or singularly, withhold reports, plans and other documents not paid in full by the Client. In the event that final
payment for completed work is not made, TSF shall request that all copyrighted documents which were submitted to client be returned and all information used in project plans be removed from project
documents.
10 1 In the event it is necessary to take legal action to effect collection, whether or not litigation is commenced, the Client agrees to reimburse TSF for expenses in connection with any claims or suits, including
reasonable anomey's fees, including but not limited to the trial and appellate levels.
10 3. This contract shall be governed by the laws of the State of Florida.
11. CONSTRUCTION REVIEW - TSF cannot accept responsibility for any design work unless the work includes services for construction review to determine whether or not the work performed is in substantial
compliance with TSFs conclusions and recommendations
12. INDEMNIFICATION -TSF agrees to hold harmless and indemnify Client from and against liability arising out of TSFs negligent performance of the work. Client agrees to indemnify and hold TSF harmles3 from
all liability including all costs, attorney's fees and expenses of defense for any claims by any other person or corporation which may arise out of the performance or breach of this contract for which TSF was not solely
negligent
13. LIMITATION OF LIABILITY -The Client /Owner agrees to limit TSF' liability for negligent professional acts, errors or omissions, such that the total aggregate liability of TSF shall not exceed 850,000 or the total
fee for the services rendered on this project, whichever is greater The Owner further agrees to require the contractor and his subcontractors a similar limitation of liability suffered by the contractor or the subcontractors
arising from TSF negligent professional acts, errors or omissions
13.1 If Client prefers to have higher limits on professional liability, TSF agrees to increase the limits up to a maximum of 81,000,000 upon Client's written request at the time of accepting our proposal provided
that Client agrees to pay an additional consideration of 5 percent of our total fee. The additional charge for the higher liability limits is because of the greater risk assumed and is not strictly a charge for
additional professional liability insurance.
14. INSURANCE -TSF represents and warrants that it and its agents, staff and consultants employed by it are protected by Worker's Compensation insurance and Employer's Liability Insurance in conformance with
applicable state laws. TSF has such coverage under public liability and property damage insurance policies that TSF deems to be adequate. A Certificate of Insurance can be supplied evidencing such coverage upon
request
14 1 Within the limits and conditions of such insurance, TSF agrees to indemnify and save client harmless from and against any loss, damage or liability arising from any negligent acts by TSF, its agents, staff and
consultants employed by it. TSF shall not he responsible for any loss, damage or liability beyond the amounts, limits and considerations of such insurance. TSF shall not be responsible for any loss, damage
or liability arising from any acts by clients, its agents, staff and other consultants employed by it
14.2 Cost of the above coverage is included in our quoted fees. If additional coverage or increased limits of liability are required, TSF will endeavor to obtain the requested insurance and charge separately for
costs associated with additional coverage or increased limits
15. TERMINATION -This agreement may be terminated by either party upon seven days written notice in the event of substantial failure by the other party to perform in accordance with the terms thereof. Such
termination shall not be effective if the substantial failure has been remedied before expiration of the period specified in the written notice In the event of termination, TSF shall be paid for services performed to the
termination notice date plus reasonable termination expenses.
15 1 In the event of termination or suspension for more than three months, prior to completion of all reports contemplated by this Agreement, TSF may complete a report on the services performed to the date of
notice of termination or suspension. The expenses of termination or suspension shall include all direct costs for TSF in completing such analyses, records and reports.
16. CLIENT'S OBLIGATION TO NOTIFY TSF - Client represents and warrants that it has advised TSF of any known or suspected hazardous materials or conditions, utility lines and pollutants at any site at which
TSF is to do work hereunder, and unless TSF has assumed in writing the responsibility of locating subsurface objects, structures, lines or conduits, Client agrees to defend, indemnify and save TSF harmless from all
claims, suits, losses, costs and expenses, including reasonable attorney's fees as a result of personal injury, death or property damage occurring with respect to TSF's performance of its work and resulting to or caused
by contact with subsurface or latent objects, structures, lines or conduits where the actual or potential presence and location thereof were not revealed to TSF by Client
17. HAZARDOUS MATERIALS -This agreement shall not be interpreted as requiring TSF to assume the status of an owner, operator, generator, Storer, transporter, treater or disposal facility as those terms appear within
RCRA or within any Federal or State statute or regulation governing the generation, transportation, treatment, storage and disposal of pollutants

Topographic Survey with Location of Existing Improvements.
We shall perform a topographic survey of existing conditions at Key West International to support
the design of the departure hall restroom renovation..
Limits:
The limits of the survey are delineated in red on Exhibit A (shown below) and incorporated herein
for reference.
Topographic Survey:
Obtain sufficient elevations to define the existing topography in detail, including significant grade
changes in elevations. The elevations shall be referenced to the North American Vertical Datum
of 1988 (NAVD 88) and horizontally referenced to the North American Datum of 1983. (NAD 83-
2011) Epoch 2010.00 and tied to the National Geodetic Survey benchmark network. Additionally,
we shall establish a minimum of two (2) benchmarks at the site to utilize for construction. We
shall calculate and place the vertical conversion factor from NAVD 88 to the National Geodetic
Vertical Datum of 1929 (NGVD 29) on the face of the map.
We shall locate all above ground improvements including, but not limited to pavement, ramps,
driveways, .steps, gates, top of curb, bottom of curb, flow line, edge of pavement, centerline of
pavement, bottom of gutter, back of walk, face of walk, and above ground evidence of utility.
We will obtain rim elevations, pipe size, material and inverts of sewer and drainage structures
where possible. Due to the nature of obtaining the data where the structures are in use, the
information obtained may be the best approximation of the true measurements. It is the
responsibility of the end user to verify the diameter and material utilizing as -built drawings or

other resources prior to construction. Utility locations will include fire hydrants, water valves,
meter boxes, vaults and electrical outlets, power poles and overhead wires. We will locate wells,
main irrigation valves, but will not locate individual sprinkler heads.
There is no provision for the excavation, probing or location of underground utilities structures or
improvements. Utilities shall be located to the extent that they are above ground and visible. This
proposal does not include the contracting of an underground utility locating service to locate the
subsurface utilities.
The survey data will be reported to the nearest 0.05 feet for horizontal locations and for the vertical
component, 0.1 feet spot elevations on ground shots and 0.03' spot elevations on paving or hard
surfaces, with all work performed in accordance with the Standards of Practice as defined in
Chapter 5J -17, Florida Administrative Code.
Deliverables:
1. Provide four (4) signed and sealed hard copies of the topographic survey.
2. Digital PDF files and AutoCAD 2014 or lower format file of the above survey.
Clarifications:
• This proposal is based on one mobilization by our crew to accomplish the project.
Timefiame:
We can mobilize to the site within one (1) week after receipt of an executed contract, and have the
project completed within four (4) weeks after mobilization. Mobilization is wholly dependent on
the availability of accommodations for our field crew.
Fee:
The fee to provide the above surveying services shall be a lump sum amount of $ 4,380.00.

EE &G Environmental Services, LLC (EE &G) is pleased to submit this proposal for the subject
work. EE &G's Statement of Services and Fee are presented below:
STATEMENT OF SERVICES
EE &G will provide environmental consulting and testing services to conduct a limited asbestos
renovation survey of the interior and exterior areas as specified by the Client.
Task 1 —Asbestos; Inspection
An EE &G asbestos inspector will conduct a walk- through of the building to visually assess
suspect friable and nonfriable asbestos - containing materials (ACM) for condition, homogenous
areas, and functional spaces. Upon receipt of notice to proceed, EE &G will collect up to 15 bulk
samples total from the interior renovation areas and transport the samples to our in -house
asbestos laboratory for analysis. The estimate of 15 samples maximum assumes sampling of
each homogenous area (HA) in the subject interior and exterior areas of the structure as per
provided plans.
Task 2 - Laboratory Analysis
Bulk samples will be analyzed for asbestos content by polarized light microscopy (PLM) at
EE &G's in -house asbestos laboratory. The laboratory analysis report will be included in the
final asbestos survey report. EE &G anticipates that 15 samples will be adequate to complete
the survey of the building.
As per United States (US) Environmental Protection Agency (EPA) regulations, any friable
materials found to contain less than 10% asbestos by visual estimation, may require analysis by
the objective Point Count Method for confirmation of asbestos content. EE &G recommends
only samples that are less than 1% (1 sample of 3) for this analysis. This analysis may be
performed for an additional $35 per sample at the request of the Client. Any unused sample
fees from unanalyzed samples may be applied to any Point Counts required.

Task 3 — Asbestos Closeout Package
EE &G will prepare a final written report containing our findings, and recommendations, based
upon the results of the inspection. Field diagrams and /or photographs will be provided in the
report. The report will be reviewed and signed by a Licensed Asbestos Consultant. For an
additional fee, EE &G can provide additional consulting services for the removal of identified
friable ACM, nonfriable ACM , including bid assistance, contractor selection, and final air
clearance monitoring.
LIMITATIONS AND ASSUMPTIONS'
The following limitations and assumptions apply to this proposal:
• EE &G may be required to collect roof and /or exterior samples to satisfy survey
requirements. Although EE &G can temporarily patch sample holes, EE &G assumes no
responsibility for voiding of any roof warranties or for the integrity of the roof or
temporary patch. EE &G recommends patching be conducted by a roofing contractor to
maintain any roof warranty in force.
• Our fee is based on the collection and analysis of up to a maximum 15 samples.
• The Owner of the property, or the Owner's authorized representative, will provide on-
call, complete access to all areas of the building.
• Some building materials may not be accessible or exposed due to
renovation /modification activities that have occurred since original building construction.
EE &G will not be held responsible for sampling, analysis, and subsequent identification
of ACBMs that are not "reasonably accessible ". For the purpose of this proposal,
"reasonably accessible" includes, but is not limited to, "accessible without destruction of
floors, walls, or hard ceilings, capable of sampling with hand tools, and reachable with a
16' extension ladder."
• Any large scale destruction of walls or finishes in search of ACBMs will only be
conducted by EE &G at the direction of Jacobs Engineering. and only after written
permission is granted.
• If available, the Owner will provide architectural plans available for use during the
survey.

FEE ESTIMATE
Presented below is EE &G's fee estimate for completion of the proposed work:

EE &G's total estimated fee for completion of the proposed work is not to exceed a maximum
of $1,850.00. This fee estimate is expected to be sufficient to accomplish the Statement of
Services described; however, no guarantee is made or implied.
This minimum fee assumes that EE &G will have complete access to survey the area in one
single visit. This fee also assumes that each suspect material will be sampled and analyzed,
and that a maximum 15 PLM samples will be sufficient to adequately survey the areas. If more
than 15 samples are required to adequately complete the survey, EE &G will collect the
additional samples, the client will be notified, and the additional samples will be analyzed by
PLM for $10.00 each upon authorization by the client. Point Count analysis as needed at $35.

PROFESSIONAL SERVICES AGREEMENT BETWEEN
JACOBS ENGINEERING
AND
EE &G ENVIRONMENTAL SERVICES, LLC
This Agreement is made on the day of September 2017 by and between Jacobs Engineering
(the "Client ") and EE &G Environmental Services, LLC ( "EE &G ").
WITNESSETH
That for the considerations set forth below, the parties hereto do agree as follows:
1. Description of Services:
EE &G's proposal No. 2016 -2642 dated September 26, 2017.
2. Period of Performance: Testing will be performed in June 2016. Final report(s) will be submitted
in within 7 working days of last testing date.
3. Basis of Compensation:
X Not to exceed $1,850.00 without prior authorization by the client
X Assumes maximum 15 PLMs analyzed. If more than 15 PLMs are needed to properly
categorize the structure, the Client will be billed $10 per additional PLM analyzed.
4. Method of Invoicing:
X One invoice to be submitted at the completion of the project.
Monthly Progress Invoicing.
Professional Retainer: None required.
General Conditions:
a. Payments for invoices prepared by EE &G are due and payable upon delivery of the final
Asbestos Survey Report. EE &G reserves the right to apply a 1.5% monthly finance charge on
all balances over 30 days outstanding.
b. This Agreement may be terminated by either party hereto upon 15 days notice in writing to the
other party. Upon termination, EE &G shall prepare and submit a final invoice for services
rendered to the date of termination together with any termination expenses incurred.
C. The parties hereto shall maintain in full force and effect comprehensive public liability
insurance with coverage limits which are reasonable in light of the work to be undertaken, and
workmen's compensation insurance as required by law.
d. Any drawings and specifications developed pursuant to this Agreement are instruments of
service, and as such the original documents, tracings, and field notes are and remain the
property of EE &G regardless of whether the work for which they were prepared is executed.
e. In the event that legal action is instituted to enforce any of the terms of this Agreement, the
party which does not prevail shall pay the legal expenses of the prevailing party, including
attorney's fees.
The parties hereto each binds itself, its successors, executors, administrators and assigns to
the other party to this Agreement and to the successors, executors, administrators and
assigns of such other party in respect of all covenants of this Agreement.
EE &G's liability for services to be rendered under this Agreement shall be limited to $250,000,
unless Client pays for the assumption of additional liability by EE &G as a separate line item in
Article 3, Basis of Compensation.
If applicable, Client agrees that EE &G shall not be responsible for liability caused by the
presence or release of hazardous substances at the site. The Client will either make others
responsible for liabilities due to such conditions, or will indemnify and save harmless EE &G
from such liability. The provisions of this Article (6,h) shall survive any termination of this
Agreement.
IN WITNESS WHEREOF, the parties hereto have caused the Agreement to be executed by their duly
authorized officers on the date first written above.
