Contractor, All Star Air Solutions LLC, was instructed to pay a total amount of $171.00 on May 7, 2015
as evidenced by Monroe County Receipt #01000007556 for Permit #15102468. However, it was
subsequently discovered that the contractor should have signed on as a subcontractor to Permit
#14102083, rather than submitting a new permit application. A refund in the amount of $171.00 is to be
issued to the contractor, All Star Air Solutions LLC, as noted above.
