ITEM BACKGROUND:
This program was supported by the State of Florida in FY14/15 for $250,000. During the tumultuous
year the legislature had this past year the Governor vetoed the project last minute. The program
currently is on hold with a $115,000 Health Foundation Grant awarded and contingent our securing the
$250K. Keys AHFC is requesting a partnership in putting together the $250K so that they can
continue to provide no cost primary care services to underserved children and their families though our
school based clinic.

I was able to speak with Commissioner Neugent last week Friday.
Upon conclusion of our conversation he asked that I get with you to place our item on the Agenda (through him) so to
start a dialogue with the BOCC.
Keys AHEC is requesting assistance from the BOCC for its Monroe County School Health Primary Care Program. This
program was supported by the State of Florida in FY 14/15 for $250,000. During the tumultuous year the legislature had
this past year the Governor vetoed the project last minute. The program currently is on hold with a $115,500 Health
Foundation Grant awarded and contingent our securing the $250K. We come to the BOCC requesting a partnership in
putting together the $250K so that we can continue to provide no cost primary care services to underserved children
and their families though our school based clinics.
Attached are some of the supporting documents giving an outline for the program, budgeting, Clinic flyer and the
Department of Health's Site Visit report.

Monroe County School Health Initiative
During FY 14/15 the State of Florida's Legislature approved a $250,000 allocation that was
contracted through the Department of Health to the Florida Keys AHEC for the delivery of
School Based Primary Care Medical Services within Monroe County.
Keys AHEC with assistance from the Department of Health, Community Health of South Florida
Inc. and the University of Miami School of Medicine developed and revised a model School
Health Initiative (SHI) program. This program was in operation in Miami -Dade County and was
replicated within Monroe County to address the unmet health care needs of Students and
Families of the uninsured, underinsured, Medicaid and at risk populations within the County.
There are 12,139 children in Monroe County ages 0-19 with 8,291 of them school aged (5-18).
Of those school aged approximately 3,097 (37.3%) of them are enrolled in the Medicaid or
MediKids program. Families of these children make up a large portion of those living below
(11% under 100% of the FP!_) or at (29% under 200% of the FPQ poverty levels. With only 5
practicing pediatricians in the County, the County Health Department pulling out of Primary
Care and with capped Medicaid enrollments in their private practices it creates a significant
barrier to accessing basic health care services.
School District Officials have reported 10% of their student population misses 21 or more days
of school due to illness, 47% of all students are on the Free/Reduced price Lunch program and
school health nurses have seen a rise in the need for chronic disease management (diabetes),
asthmatic, mental health, substance abuse, general primary care and oral health services.
With a joint agency and team approach Keys AHEC has worked with the Monroe County School
District, it's schools and the school health nurses to deploy a medical team consisting of two
Physician Assistant's, one Medical Director and a Registered Nurse -program assistant to offer
full time primary care medical services in 5 school based clinics. Also included on the team
have been health professions students and local medical volunteers. Services include basic
primary care and care coordination for immunizations, mental health assessments and
counseling, oral health services, prescriptions and laboratory services.
The School based clinic model now serves as the medical home for some students and families
as well as a walk in clinic for non -emergency services. Clinic days have started one half hour
prior to school and run one half hour after school on rotating days at the five clinical sites.
Medical staff provides all primary services, prescription services and refer patients for any
needed secondary or specialty services. The medical team then is available to provide follow
up care as required by a documented treatment plan.
Other ancillary services that are offered include clinical breast exams for student family
members, smoking cessation, skin cancer assessments, cardiovascular disease assessment and
nutrition services as provided by Keys AHEC. Through other outside funding sources and
partners services such as gynecological and prostate exams are available through the University
of Miami 's community health fairs.
Providing school based health services in an easily accessible location and manner has created
new access points for children and their families so that they have a medical home and can
avoid the ER. The School based clinics have also assisted local providers address health care
needs of the uninsured and Medicaid patients in an area where there are limited Medicaid
providers.
With an increased allocation the SHI can expand services to 6 medical clinics throughout
Monroe County and create access points in each diverse geographic location within the school
based setting. The expansion will provide additional medical providers to allow us to reach an
additional 2,500 students with one additional provider.
With requested funds:
1. Key West will have 5 day service (HOB and KWHS-4/1)
2. Sugarloaf and Marathon will have 5 day service (3/2)
3. Upper Keys (Key Largo and Coral Shores) will have 5 day service (3/2)
4. Will be aWe to start to offer Oral Health Assessments for all Grades in the School Medical
Clinic.
5. Add basic lab tests (non -blood draw)
Program Highlights:
• In the first year of operation the 4 Clinical Sites have provided 7,966 service encounters
for 3,156 individual patients.
o This does not include patient file reviews, parent contacts or
referral/consultation with other private providers,
• 5 Clinical sites started in September 2014 and January 2015 with a 6'h site added in Tab
be added in August 2015. Along with a third provider.
Currently taking Blue Cross Blue Shield and Cigna with Aetna and United Health (Healthy
Kids/Kidcare) pending approval.
o Medicaid will be applied for once the Clinic can run 52 weeks out of the year so
that patients can be assigned to our providers
• Additional Funding sources has been secured and applied for.
o NACCHO-$15,000 Approved
o Health Foundation of South Florida -Approved (contingent on other funding)
o Health Insurance collections -Just started receiving payments in January 2015.
• Expanding Services to Families.

1. Authority
Pursuant to Section I.E. of the standard contract between the Florida Department of Health
(hereinafter "Department") and the Florida Keys Area Health Education Center, Inc- (hereinafter
"Provider'). the Department is authorized to monitor the activities of the Provider to ensure
satisfactory performance of the terms and conditions of its contract with the Provider and to ensure
compliance with applicable laws, rules and regulations. Monitoring activities which are relevant to
the Department's contract with the Provider include, but are not limited to. 1) inspection of records,
papers, documents, facilities, goods and services of the Provider; and 2) interviews of Provider
employees.
It. Scope and Objectives of the Review
The Department's Contractual Services Agreement (Contract #COHG5) with the Provider is a unit
cost reimbursement contract with funding totaling $250,000 in state Financial assistance under
CSFA No. 64.132
On May 5, 2015, Ernest Bruton conducted a review of the program activities of the Provider at the
Provider's at one of the four locations (Horace O'Bryant School) located in Key West. Florida. The
purpose of the review was to-
a. Determine the Provider's compliance with the Contract terms and conditions and any
applicable laws and regulations
b. Detect / prevent non-compliance by identifying and resolving potential problems
c. Ensure that necessary corrective actions are delineated
The review began with an entrance conference which described the scope and purpose of the
review and concluded with an exit conference during which, review findings were discussed.
The contract monitoring tool, the current contract document and supporting invoice documentation
were used to conduct the review. Interviews with Provider staff were also conducted during the
review

8. Conditions and Contingencies: Award confingent on: (1) securing a minimum of
$250,000. in FY 2015/16 State of Florida funding: (2) securing a minimum of $15,000
in Fv 2015/16 National Association of County and City Heolfh Officials funding.
B. Additional Requirements due throughout the funding period:
1. Evaluation Consultant: You may be reg ,lred to work with on evo',uc' cn
consultant who wilt be assigned by the Foundation. if required, you will f:nd
details on the evaluator and the requirements in an attached docurnent.
2. Outreach with Elected Officials: Health Foundation recognizes that only through
collaborative effort con we successfully establish a more health promoting
public policy environment. As a result, you may be asked to work with your
Health Foundation liaison to engage and inform selected elected officials on,
pubiic policy issues relating your HFSF grant supported project and you,
organization as a whole.
3. Reporting: Progress and Final Reports must be submitted electronic&y via a link
that will be e-mailed to you at the appropriate times.
4. Data Tracking: Please trace the Income/Poverty level and the Race/Ethnicity a`
your program participants and reper, the data in the Fina! Report.
5. Organizational Status Changes: Notify your liaison within two weeks of charges in
orgonizationai status including but not limited to: key personnel such as senior or
project staff, organizational name, tax status, and organizational address.
6. Other Organizational Changes: Notify your liaison of any organizational changes,
such as new address or contact information to ensure that you maintain proper
communications with the Foundation and receive payments promptly, If a grart
payment has to be reissued, it will be subject to a $35 change fee.
7. Budget Modifications: Notify your liaison and submit a budget modification fore.
if budget changes greater than $3,000 are required during the course of the
protect. Please advise your liaison if the budget change is due to an increase o-
