AGENDA ITEM WORDING: Approval of an agreement between Monroe County Homeless
Services Continuum • Care and Monroe County Board of County Commissioners (Social Services) to
track accurate and up-to-date information about the homeless and at-risk population from 09/30/2013
through 10/01/2014.
ITEM BACKGROUND: A Homeless Management Information System (HMIS) is a software
application designed to record and store client-level information on the characteristics and service
needs of homeless persons, coordinate care, manage their operations, and better serve their clients.
PREVIOUS RELEVANT BOCC ACTION: Approved on 06/19/2013
CONTRACT/AGREEMENT CHANGES: ClientTrack HMIS program will replace Pathways
COMPASS BMIS program for the new contract period of 09/30/2013 through 10/01/2014.

WHEREAS, The Monroe County Homeless Services Continuum • Care (MC CoC) funds human
service organizations in accessing the ClientTrack Homeless Management Information System
(HMIS)
• track accurate and up-to-date information about the homeless and at-risk population being
served • local organizations; and
WHEREAS, Monroe County Social Services wishes to participate in ClientTrack Homeless
Management Information system to share information about clients and their needs, track client
assistance, and work jointly with other agencies to help clients move toward achieving their
objectives;

MC Coc will:
1. Provide access fees for the ClientTrack's HMIS system from 9130113 through 1010112014;
2. Provide all forms necessary for the organization to gain access to the ClientTrack system;
3. Provide User and Confidentiality Training;
4. Provide technical assistance and support in the use of the HMIS.
This Agreement will remain in effect from September 30, 2013 through October 1, 2014.
