C19 Amendment #001 to Alzheimer's Disease Initiative (ADI) Program Contract K Z-
1797 between the Alliance for Aging, Inc., (AAA) and Monroe County BOCC /Social
Services /In -home Services for the Contract Year 2017 -2018 in the amount of $189,458.40.
This amendment increases funding by $24,810.60. Enclosed are six (6) duplicate originals,
executed on behalf of Monroe County, for your handling. Please be sure to return two duplicate
originals once signed by the Alliance.
C33 Amendment #003 to Older Americans Act Contract AA -1729 between the
Alliance for Aging, Inc., and Monroe County Board of County Commissioners/Monroe
County Social Services to de- obligate funding by $45,000.00 for the 2017 Contract Period
(January 1, 2017 - December 31, 2017). This amendment changes the total contract funding from
$639,103.68 to $594,103.68. Enclosed are two duplicate originals for your records.
