ITEM BACKGROUND:
Contractor, Coastal Docks, LLC. was instructed by the Building Department to pay a total of $180.00 as
evidenced by Monroe County Receipt #02000020460, for Permit# 14304546. However, it was
discovered afterward the permit was not required for (232 Orchid St.; Tavernier, FL) and should not
have been issued. Coastal Docks, LLC. has requested a refund. A refund in the amount of $180.00 is
requested to be issued to the contractor, Coastal Docks, LLC. as noted above.

Re: Request for Return of Permit Application Fee
To Whom It May Concern:
On or about September 29, 2014, Anthony Bastarache from our office applied for a
building permit for a property in the name of Zubillaga, located at 232 Orchid Street,
Tavernier, Florida. The permit number is 14304546. It was incorrectly assumed both by
our office and Dawn Bender who accepted the application for the County that the subject
property is located in the county. It was later discovered that the property is located
inside the city limits of Islamorada. The fee paid was $180.00, check number 7642 dated
September 17, 2014. We respectfully request that the permit application fee of $180.00
be refunded to us.
