Per Florida Statute 112.3143. 1 hereby disclose by written memorandum that 1 will abstain from
the vote on certain issues that are brought before the Monroe County Board of Commissioners with entities
that I am involved with.
I will abstain from the vote on issues concerning the following entities: Guidance Care Center,
Inc., a private. not -for -profit entity, which receives some of its operational funding from the County, as I
currently sit on the Board of Directors of the Care Center. I am also a member of the Board of the Historic
Florida Keys Foundation. Inc.
At the September 16, 2015 BOCC meeting, 1 will abstain from the vote on the following
item(s):
NCI: Ratiflcation of a contract between .Monroe County BOCC/Monroe County
Social Services Ofonroe County Transit) and the Cornnwnity Transportation
Coordinator, Guidance Care Center, Inc for a contract period of 71112015 to
6/30/2016. This is a non -monetary partnership/collaboration contract required bra the
State of Florida Commission for the Transportation Disadvantaged
#C'13 Approval of contract with the State of Florida, Department of State, Division of
Historical Resources to fund the project "Nonroe county Cultural Resources
Assessment L'pdale" as set forth In the application submitted by the Historical Florida
Ktys Foundation (HFKF). (Commissioner Rice abstained from the vote an this item at
Ike meeting.

AGENDA ITEM WORDING: Ratification of grant adjustment submission to FDLE Byrne JAG

ITEM BACKGROUND: One of the original proposed projects did not come to fruition resulting in a
revised distribution of the approved projects and their funding. The grant adjustment reflects an
increase of $3,000.00 in the award amount as a result of the redistribution of funding. FDLE also
requested a modification to the state and federal purpose areas as well as a revision of the budget
narrative to reflect a unit cost methodology. The grant adjustment addresses these items.
PREVIOUS RELEVANT BOCC ACTION: At the January 21", 2015 BOCC the Board approved
the Certificate of Acceptance of Sub -grant Award for Guidance/Care Center, In., d/b/a The fleron
Assisted Living: Assisted Living Prevents Recidivism in Dually -Diagnosed Adults with Mental Illness
& Substance Abuse project. On May 20, 2015 the BOCC approved a revised distribution of approved
projects for submission to FDLE under the Edward Byrne Memorial Justice Assistance Grant program
increasing the original award by $3,000.
CONTRACT/AGREEMENT CHANGES: Increase in the contract amount from $17,345.00 to
$20,345.00 and a change in the federal and state purpose areas and revisions to the budget documents
to reflect a change in the unit cost methodology.

entitlements, and service planning. This individual interacts with community
organizations on behalf of the residents and advocates for new / ongoing services.
Site Managers also participate in our special 90-day recovery program for residents
who have tested positive for drugs or alcohol.
The Mental Health Support Workers work with referral agencies, prospective clients,
families, staff and other resources. They also perform all other support functions
including resident orientation, case management, and record maintenance. He or
she also maintains client files, progress notes, correspondence logs, They provide
supervision of medication use and observe clients for unusual behavior or possible
intoxication, makes recommendations to management staff regarding client plans
and activities, assist with daily living skills training, provide transportation, provide
access to and participation in social activities, and arrange/escort to medical
appointments.
