ITEM BACKGROUND: The purpose of Amendment #002 of the Older Americans Act (OAA)
contract is to modify paragraph_3.5.2. , Financial Consequences of Attachment I of the original
contract.
PREVIOUS RELEVANT BOCC ACTION: Prior approval granted to Amendment #001 Older
Americans Act (OAA) Contract #AA-1329 on 2/20/13.

THIS AMENDMENT is entered into between the Alliance for Aging, Inc. hereinafter referred
to as the "Alliance", and Monroe County Social Services.
The purpose of this amendment is to modify paragraph 3.5.2 of attachment I of the original
contract to read:
3.5.2 Financial Consequences
The Contractor shall ensure the provision of services to the projected number of clients in
accordance with the Service Provider Application and within the contract amount. The
Contractor shall ensure expenditure of 100% of the contract amount budgeted for services
to clients at the unit rates established in this contract. On a service -by -service basis, in the
event the Contractor has a surplus of 1% or more at the end of the contract term, the
Alliance will reallocate this surplus to other provider agencies in accordance with its
policies. If, or to the extent, there is any conflict between this paragraph and paragraphs 38
and 38.1 of the standard contract, this paragraph shall have precedence.
All provisions in the contract and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the contract are hereby amended to conform with this amendment.
This amendment and all its attachments are hereby made a part of the contract.

THIS AMENDMENT is entered into between the Alliance for Aging, Inc. hereinafter referred to
as the "Alliance", and Monroe County Social Services.
The purpose of this amendment is to increase the allocation under title IIIB by $31,000 as a result of the
2012 ITN "Empowering Older Adults through Internet Access". This amendment also modifies the
provider's contract to add the following section to Attachment I of the original contract
2.7.8 Empowering Older Adults Through Internet Access
Contractors funded under the Empowering Older Adults Through Internet Access ITN must submit a
plan to the Alliance prior to spending funds. The plan will detail how the funds will be spent and how
the effectiveness of these expenditures will be measured. The Alliance will reimburse only for
expenditures that are made according to the plan.
