(GDR) appreciates the opportunity to assist you with this pricing to provide for the recovery after Hurricane Irma's landing in Florida. GDR has partnered with ConServe, LLC (CGC1523986) to assist you with this scope of services. Additional information is presented as follows: BACKGROUND Hurricane Irma passed along the Florida Keys and created appreciable destruction to the Adult Education Center. Appreciable wind and tidal conditions reportedly entered the property and heavily impacted the surrounding site, architectural finishes, vapor barriers, insulations, Mechanical, Electrical and Plumbing Systems, corrosion of structural fasteners and connection ties, impacts to vapor barriers and structural materials. The structure is scheduled for demolition following the State of Florida requirements for such. AHERA requirements for asbestos in schools are specifically NOT included in this pricing request per Monroe County. GDR reviewed the site on February 23, 2018 and is proceeding based on this understanding. The complete asbestos abatement is to be performed by others under contract with the County and GDR will not be responsible for managing asbestos concerns through demolition. No other plans were provided. The demolition debris are understood to be only inert and recyclable media. Based on a review of the facility, other waste exist that are specifically precluded for landfilling. It is also regulated under the Resource Conservation and Recovery Act (RCRA), the Toxic Substance Control Act. • Universal Waste - lamps, bulbs, ballasts, mercury containing thermostats, batteries, agricultural pesticides, etc. • Small Motorized Equipment (SME) — Examples of SME include push and riding lawn mowers, lawn trimming equipment, edgers, chain saws and untitled recreational equipment, that contain hazardous substances (gas/diesel, antifreeze, hydraulic, transmission, engine oil, etc). • Municipal Solid Waste (MSW) — MSW includes putrescible wastes, household garbage, etc. MSW is distinctly separate from Construction and Demolition debris and is to be disposed at a lined facility in Florida. • Automotive Scrap Tires — These items are segregated and cannot be landfilled. They are recycled for multiple applications and reuse. • White Goods — Appliance for cooling may contain Freon (R-22/R-12), which contains chlorofluorocarbons (CFC's), hydrochlorofluorocarbons (HCFC's) or ammonia. Appliances manufactured prior to 1979 may have capacitors which contain PCB's. Other potentially hazardous chemicals include mercury (switches and gauges) and oils (transmissions and compressors). • Electronic Debris (eWaste) — computers and monitors, printers, telephones, communication equipment, VCR's, TV's, cable equipment, circuit boards, large office computer equipment, CPU boxes, laptops, fax machines, CD-roms, DVD-roms, audio equipment, video games, disc drives, modems, cards, keyboards, computer mice, electronic wires or plugs, recording, surveillance or camera equipment, cameras, cell phones & accessories, LCD's, parts or pieces of computers, hair dryers, etc. Smoke Detectors: These items contain radioactive material and require proper decommissioning at a federally permitted facility. Based on information from the County, there have been no hazardous/universal/special waste surveys beyond the limited AAL PLM report provided. A comprehensive survey is required to define the scope. This scope is designed to provide comparative pricing information for abatement of the subject property. The estimate pricing source includes, but is not limited to, the indicated RS Means Residential Cost Data (2016 35' Annual Edition) price list, invoices, proposals, estimates and expert recommendations provided to GDR. This estimate may reflect repairs to areas with no visible damage in consideration with the elected method of repair and/or to address a matching Mr. Chris Rivera March 8, 2018 Page 3 continuum. GDR reserves the right to amend this estimate should any additional findings appear, including but not limited to: hidden damages, market condition change and/or unit -cost change. This estimate was produced to the best of our knowledge with the understanding that there is the possibility of errors and/or omissions. No warranties on prices, cost, errors, omissions, and/or liability can be assumed by GDR based on this information. This budgetary estimate has been prepared without knowledge of the 31 party asbestos inspector's finding and recommendations to proceed. This work scope has limitations that are presented throughout this document. If additional findings present itself that require additional work from GDR, it will be presented to the Owner for consideration. There have been no documents presented by County to present the limit of work or drawings as of this proposal. PROPOSED STATEMENT OF SERVICES The following tasks will be completed as part of this project: t i i M GDR will mobilize to prepare the structure for demolition. This includes removing the following items, transporting via a licensed FDOT hauler and sending them to a State of Florida licensed facility for decommissioning. This will include the visible light fixtures and their associated ballasts, mercury -containing switches and Freon associated with the HVAC system. GDR recommends a comprehensive survey to define the total scope of decommissioning through removal of hazardous/universal/special wastes. GDR will perform this review and provide a comprehensive list of items. PRICE: $6,700 Demolition Services A sketch is presented as an attachment to this proposal. This presents the structure and the limits of construction. This commercial building is approximately 50' x 50'. The asbestos items in the AAL report will be remediated by others. The following presents the detailed services and limitations: Ten working day notifications (Required as per the NESHAPs) will be forwarded to the State of Florida Department of Environmental Protection via certified mail to ensure proper and timely delivery. GDR will contact central locating to obtain utility clearances for the pending demolition. This will allow sufficient time for various service providers to mark their systems in the area. GDR will ensure that Owner -identified water, sewer, electrical, gas, communication and fire control systems are de -energized prior to demolition. It is the Owner's responsibility to mark utilities on private property. GDR has not proposed to perform underground surveys. GDR will cut and cap items within approximately 5-feet of the structure or as reasonably feasonable. In the event that a particular line requires substantial underground work beyond the 5 linear feet, GDR reserves the right to request additional funding to meet the needs of the project. It is GDR's understanding that substantial underground and site work (and demolition Mr. Chris Rivera March 8, 2018 Page 5 • Freon extraction. • Disposal of environmentally impacted soils. • Backfilling or stone fill. There has been no funding for landscaping, sodding, seeding or other site restoration. • Plumbing, mechanical and electrical disconnects not specifically indicated above. • Shoring or bracing. • Work hour restrictions. • Additional services not proposed or reasonably inferable herein. FEE SUMMARY GDR proposed fees for the above statement of services will be on a Lump Sum basis of $59,980 in accordance with the above detail. If the scope of work or project duration is modified, additional costs may be incurred. If this proposal is satisfactory, please present the GDR with the County contract for signature.
