the Task Order modifies an Article of the Professional Services Agreement of which will be
specifically referenced in this Task Order and the modification shall be precisely described, These
additions apply only to the SUGARLOAF VOLUNTEER FIRE DEPARTMENT SITE
ANALYSIS referenced in this Task Order.
This Task Order is effective n the dati of &0—em44, 2017.
SCOPE OF WORK
Consultant will furbish the personnel, services, equipment, materials, and other requirements
necessaiN to conduct one site visit, review all drawings, surveys and pertinent information
provided by COUNTY to develop two potential site plans to accommodate a new fire station
same as the proposed Cudjoe Fire Station on the Sugarloaf Volunteer Fire Department Lot.

functions will have to be placed above the bays. An alternative is to include an open meeting
room of approximately 1,600 SK This space can double as fire station space but would,
preferably, be capable of isolation from the dedicated fire station facilities,
It is permissible (and possibly required to assume the use of a 47' x 160') portion of the FDOT
ROW immediately adjacent to the sewer pump station (RE4 00118480-000000) and running 160
LF iii a northwesterly direction parallel to US-1. The County has requested that FOOT surplus
Sugarloaf Volunteer Fire Department Site Analysis Task Order
that section of the US-1 ROW to construct a new fire station and expects that request to be
granted. Exhibit B shows the current land use restrictions and expected site expansion,

DELIVERABLES
Provide two (2) signed scaled original documents including all full sized plans, and one
electronically signed digital copy in ADOBE PDF format before Tuesday December 12 2017
SITE VISIT
Consultant will conduct one (1) site visit, Consultant will coordinate the site visit with the
County's project manager.
ADDITIONAL SERVICES
In accordance with Article Ill of the Professional Services Agreement, any fee for Additional
Services shall be negotiated at the time when services are required, The County shall describe in
writing the requested additional service(s) to the Consultant,

COUNTY RESPONSIBILITY
The County's responsibility is outlined in Article IV of the Professional Services Agreement,
COMPENSATION
In accordance with Article VII of the Professional Services Agreement, the County shall pay the
Consultant in current funds n lot sum foe of Twelve Thousand nine hundred fourteen Dollars
and 00/100 ($12,914.00) billed monthly on a percentage complete basis,
REIM13URSABLE EXPENSES
Article 73.1 of the Professional Services Agreement shall allow reimbursable expenses in
accordance with the March 18, 2015 Professional Services Agreement and as amended on
January 18, 2017,
