abstain from the vote on certain issues brought before the Monroe County Board of
Commissioners with entities with which I am involved.
I will abstain from the vote on issues concerning the following entities:
Guidance Care Center, Inc., a private, not - for - profit entity, which receives some of its
operational funding from the County, as I currently sit on the Board of Directors of the
Guidance Care Center. I am also a member of the Board of the Historic Florida Keys
Foundation, Inc.

This form is for use by any person serving at the county, city, or other local level of govemment on an appointed or elected board, council;
commission, authority, or committee. It applies to members of advisory and non - advisory bodies who are presented with a voting conflict of
interest under Section 112.3143, Florida Statutes.
Your responsibilities under the law when faced with voting on a measure in which you have a conflict of interest will vary greatly depending
on whether you hold an elective or appointive position. For this reason, please pay close attention to the instructions on this form before
completing and filing the form.
INSTRUCTIONS FOR COMPLIANCE WITH SECTION 112.3143, FLORIDA STATUTES
A person holding elective or appointive county, municipal, or other local public office MUST ABSTAIN from voting on a measure which
would inure to his or her special private gain or loss. Each elected or appointed local officer also MUST ABSTAIN from knowingly voting on
a measure which would inure to the special gain or loss of a principal (other than a govemment agency) by whom he or she is retained
(including the parent, subsidiary, or sibling organization of a principal by which he or she is retained); to the special private gain or loss of a
relative; or to the special private gain or loss of a business associate. Commissioners of community redevelopment agencies (CRAs) under
Sec. 163.356 or 163.357, F.S., and officers of independent special tax districts elected on a one -acre, one -vote basis are not prohibited
from voting in that capacity.
For purposes of this law, a "relative" includes onfy the officer's father, mother, son, daughter, husband, wife, brother, sister, father -in -law,
mother -in -law, son -in -law, and daughter -in -law. A "business associate" means any person or entity engaged in or carrying on a business
enterprise with the officer as a partner, joint venturer, coowner of property, or corporate shareholder (where the shares of the corporation
are not listed on any national or regional stock exchange).

Dear Sheriff Ramsay,
I am writing this letter to request assistance of use of the Sheriff's Asset Forfeiture Funds to
assist in the required installation of a generator at The Heron Assisted Living Facility.
Both locally and nationally, jails /prisons contain a significant number of non - violent mentally ill
men and women. The Heron Assisted Living Facility provides stable, affordable, supportive
housing to adults with mental illness and who are dually diagnosed (with mental illness and a
history of substance abuse). The supportive services we provide improve the residents' ability to
manage his /her mental illness and maintain sobriety- therefore greatl reducing the risk of
recidivism and improving the quality of life of this very vulnerable population. Information
providd_Fy a onroe County Sheriffs Office indicates that it costs approximately $80.40 /day to
maintain a prisoner in the County Jail. The daily cost of a stay at North Florida Evaluation and
Treatment Center (State Funded Forensic Mental Health Facility) is in excess of $233.00. The cost
for a bed day at The Heron is $78.84 and provides a therapeutic environment with access to an
array of psychosocial rehabilitation activities.
The problem of incarceration of adults with mental illness and substance abuse, especially those
who are not major offenders, remains a significant concern in our community. Almost all jail
inmates with co- occurring mental illness and substance use disorders will leave correctional
settings and return to the community. Many of these individuals return to homelessness.
Monroe County has only on Assisted Living F acility with a Mental H e a lth License serving a total
of 16 low- income residents (The Heron). The Monroe County Homeless Services Coalition from
the 2018 Point -in -Time study performed indicated that there were 973 homeless people in Monroe
County at that time with 153 of those individuals in jail, up from previous years due to Hurricane
Irma. In fad, the problems of inappropriate incarceration and its link to homelessness have
become so epidemic that the federal Substance Abuse and Mental Health Services
Administration (SAMHSA) has identified major initiatives "t divert individuals with mental
illness from the criminal justice system to mental health treatment and appropriate supported
services ". The H Living Facility of this kind in Monroe County serving

this population: providing housing with support services to adults who are persistently and
chronically mentally ill. By directing these type clients to The Heron and similar programs,
whether through diversion upon entry to the criminal justice system or at reentry, we can
provide a cost effective, stable housing and the supportive services necessary for successful
community integration, and therefore decreasing the likelihood of recidivism. The Heron offers a
more effective recovery environment than alternatives such as homelessness, correctional
institutions, and state psychiatric hospitals.
The state of Florida is requiring that ail AHCA licensed Assisted Living Facilities have a generator
completely installed by December 2018. This is a new requirement after Hurricane Irma and
without compliance the Heron cannot stay open which would be detrimental to our county. We
are requesting $15,000 in SAFfJmndsJo be used for The Heron and any funds that can be
provided will be matched by the Patrick Lee Foundation leveraging our ability to meet this
requirement by the deadline. We have already raised significant funds for this project and this
remaining amount will ensure we can complete the installation by the deadline.
