Food Services, Inc., and Monroe County Board of County Commissioners /Monroe County Social
Services/Nutrition Services (for the provision of Congregate and Home Delivered Meals throughout
Monroe County) on a month to month basis while the County conducts the competitive solicitation
process.
ITEM BACKGROUND: GA Food Service, Inc., is the current food vendor which provides the
meals for the Monroe County Nutrition Program operated by Monroe County Social Services. The
County wishes to continue to provide meals as supplied by GA Food Service, Inc., on a month to
month basis beginning January 1, 2019, while at the same time, conducting the competitive
solicitation process.

Please accept this letter as 30-day notice of rate adjustment effective January 1 2018 through
December 31 201 during the 5th extension option as stated in our agreement.
We request an adjustment of a 2.2% increase, reflecting the Unadjusted 12 Month Averaged
Consumer Price Index for All Urban Consumers (CPI-U) as released after month of October.
The new adjusted 201 rate will be $3.88 per meal. Also included, please see updated Supply
Sheet for 2018.

8 1 h AMENDMENT AGREEMENT
Monroe County Nutrition Program
This 8 1h Amendment to Agreement ( "Extension ") is entered into this 20 day of November, 2018, by and
between the Board of County Commissioners of Monroe County, Florida, a political subdivision of the State of
Florida hereinafter called the County, and GA Food Service, Inc., a Florida corporation whose mailing address
is 12200 32n Court North, St. Petersburg, Florida, hereinafter called the Contractor.
WITNESSETH
WHEREAS, the parties entered into an agreement dated January 19, 2012, for provision of meals for the
Monroe County Nutrition Program Congregate and Home Delivered Meals program operated by the County;
and
WHEREAS, on the December 10, 2014 the parties entered into an Amendment to Extend Agreement extending
the termination date of the original agreement in order to align the County's competitive solicitation cycle with
the six (6) year Alliance for Aging (AAA) grant cycle; and
WHEREAS, the AAA grant award contract will be let no later than 1/1/2019; and
WHEREAS, if the county receives a AAA grant award the County desires to continue providing the services
under the original agreement dated 1/19/2012, on a month to month basis while the county's competitive
solicitation process is completed;
NOW, THEREFORE, in consideration of the mutual covenants and obligations contained in this Extension and
in the original agreement, the parties agree as follows:
1. Beginning January 1, 2019, the Contractor shall continue to provide meals pursuant to the agreement dated
January 19, 2012 and any amendments thereto on a month to month basis.
2. County may terminate the G.A Food Services Agreement dated January 19, 2012 and all amendments
thereto upon 30 days written notice provided by the Director of Social Services.
3 Contractor shall comply with all federal, state, and local food safety regulations. Pursuant to paragraph 15 of
the original Agreement the county may cancel this Agreement if any of the following occur:
a. A temporary or permanent closure related to food service violations,
b. Administrative Complaints regarding food safety, or
c. 10 or more high priority /significant findings on sanitation inspections within the past 12 months,
beginning July 1, 2018. (Per Department of Business and Professional Regulation (DBPR), "high
priority violations could contribute directly to a foodborne illness or injury and could pose a
direct or significant threat to public health, safety, or welfare." Per Department of Health
(DOH), "an unsatisfactory inspection means that the violations were a significant threat to public
health and sanitation and require correction before the next routine inspection." Information

4. Contractor shall maintain documentation of a food safety management program within the facility that
meets or exceeds the minimum requirements of federal, state, municipal, or other agencies authorized to
inspect or accredit the food service operation and shall, upon request of the County, provide documentation
of said food safety management program and the results of any inspections related thereto.
Upon any request by the County, Contractor shall provide documentation of the three most recent food
preparation inspections conducted by the state regulatory authority. Documentation shall be provided in
writing, via email, to the Nutrition Department or requesting staff.
6. Upon any request by the County, Contractor shall provide County with a written plan of correction for any
high priority or significant findings on sanitation inspections. Documentation shall be provided in writing,
via email, to the Nutrition Department or requesting staff.
7. Contractor shall immediately notify the County of any closures or Administrative Complaints regarding
food safety or of any sanitation inspections as referenced in DEPARTMENT OF ELDER AFFAIRS
PROGRAMS AND SERIVCES HANDBOOK Chapter 4: Older Americans Act Service Requirements:
Section 5 Nutrition Program Policies Food Service Contract Provisions July 2018 4 -163.
8. Upon any request by the County, Contractor shall provide County with a description of Contractor's
delivery standards and sanitation that includes holding temperatures for transporting and serving food.
Documentation shall be provided in writing, via email, to the Nutrition Department or requesting staff.
9. County, AAA, and /or Department of Elder Affairs staff shall have the right, upon 24 hours written notice, to
inspect the food preparation and storage areas.
10. All other terms and conditions of the contract dated January 19, 2012, and any amendments thereto, shall
remain in full force and effect.
