Horn for Blue Heron Park Damage Description Report- 1 copy enclosed TASK ORDER PROFESSIONAL ARCHITECTURAL AND ENGINEERING SERVICES BETWEEN MONROE COUNTY AND WILLIAM P. HORN ARCHITECT, P.A. FOR BIG PINE COMMUNITY PARK DAMAGE DESCRIPTION REPORT In accordance with the Consultant Agreement for Professional Architectural and Engineering Service, made and entered on January 21, 2015, and as amended on February 15, 2017 and April 2018, between MONROE COUNTY, hereinafter referred to as the "County", and WILLIAM P. HORN, ARCHITECTS, P.A., hereinafter referred to as "Consultant", wherein professional services are allowed if construction costs do not exceed $2,000,000.00, or for study activity if the fee for professional services for each individual study under the contract does not exceed $200,000.00 ("Agreement"). All terms and conditions of the Agreement apply to this Task Order, unless the Task Order modifies an Article of the Agreement of which will be specifically referenced in this Task Order and the modification shall be precisely described. These additions apply only to the project referenced in this Task Order. , I This Task Order is effective on the a '- day of /1/(DI/I!'141/6/ , 2018. The Big Pine Community Park sustained heavy damage due to the high winds and flooding caused by Hurricane Irma in September, 2017. The damages to the electrical, plumbing, sewer systems and irrigation system could not be identified due to the loss of utility services. The damages to these items must be documented prior to beginning any design or restoration efforts to restore the park to pre-Hurricane Irma conditions. 1. In accordance with the Agreement Article II, Consultant shall evaluate and document the damage resulting from Hurricane Irma. The systems to be evaluated may require temporary utility connections be arranged by the Architect prior to performing the evaluation. The evaluation shall include the electrical, plumbing, sewer systems and the • irrigation system. The detailed damage description for the electrical system shall describe what element it is serving and include the length of conduit, length and gauge of wire, number and size of junction boxes and breakers, number of duplex outlets, number and type of lighting fixtures, scoreboards and any other electrical item that is identified as damaged by Hurricane Irma. The damage description for the plumbing shall include the length and size of pipe and include a description of what element it is serving. The damage description for the sewer shall include the length-and size of pipe, lift stations, grinder pumps and any other damaged sewer component. In accordance with the Agreement Article III Paragraph 3.2 if additional services are required, the County shall request such services in writing and the Consultant shall respond with a fee proposal to perform the requested services. i 3. In accordance with the Agreement Article VII, paragraph 7.2,1 the County shall pay the Consultant, upon delivery of the detailed damage description report, a lump sum of Forty- Five Thousand Two Hundred Sixty and 00/100 Dollars ($45,260.00). 4. In accordance with the Agreement as Amended on April 19, 2018, Article IX, paragraph 9.32 of the Agreement, the Contractor is bound by the terms and conditions of the Federally-Funded Sub award and Grant Agreement between County and the Florida Division of Emergency Management (Division), found at the following link on the Monroe County web page: http:/lfl-rnonroecounty.civicplus:com/Bids:aspx?CatID=18. 5. In accordance with the Agreement as Amended on April 19, 2018, Article IX, paragraph 9.34.1, Consultant shall provide a baseline project schedule indicating the preconstruction activities to be monitored and the anticipated completion dates for milestones.
