FOR ROTH BUILDING AT 50 HIGH POINT RD.,TAVERNIER,FL In accordance with the Consultant Agreement for Professional Architectural and Engineering Services, made and entered into on September 9,2014;and amended on January 18,2017,and second amended on April 2, 2018, and third amended.on June 6, 2018, between MONROE COUNTY, hereinafter referred to as the"County,"and WOOD ENVIRONMENT&INFRASTRUCTURE SOLUTIONS, INC.,hereinafter referred to as "Consultant,"where professional services are allowed if construction costs do not exceed Two Million and 00/100 Dollars ($2,000,000.00), or for study activity if the fee for professional services for each individual study under the contract does not exceed Two Hundred Thousand and 00/100 Dollars($200,000.00),hereinafter referred to as the"Agreement". The Roth Building has concrete spalling damages along the perimeter slab and exterior walls. The County is pursuing an engineered solution that will provide remediation and repair of these structure conditions. Therefore, the County seeks professional architectural and engineering services in accordance with the current Agreement with Wood Environment&Infrastructure Solutions,Inc. All terms and conditions of the referenced Agreement apply to this Task Order,unless this Task Order amends, adds, or modifies a provision or an Article of the Agreement of which will be specifically referenced in this Task Order and the amendment, addition, or modification shall be precisely described. These additions apply only to the project referenced in this Task Order. This Task Order is effective on the 4:6 of gopeAr6018. 1. In accordance with Article II SCOPE OF BASIC SERVICES of the Agreement,the Consultant will perform services as described below: a. Consultant shall conduct an on-site evaluation and assessment. Consultant shall • prepare a Phase I structural report documenting the extent and cause of the spalling damages as well as a recommended aesthetic repair of the Roth Building. The findings shall be utilized for the basis of a professional recommendation that no further investigation is required or (If required) a proposal for a Phase II report that will include: Permit ready drawings identifying the recommended repairs. The findings shall be utilized as the basis of design for repair of the observed damage. b. It is expected that the deliverables will be submitted to County within 8 weeks of receiving the approved task order. 2. In accordance with Article VII paragraph 7.1.1 of the Agreement, the County shall pay the Consultant a lump sum fee of Five Thousand Eight Hundred Eighteen and 00/100 Dollars ($5,818.00)paid per the following schedule of payment.
