EDCAP performed an assessment of the existing stormwater management system at Key West International Airport (EYW) and outlined a phased program for stormwater improvements for EYW. EDCAP was initiated to address needs as identified in the Federal Aviation Administration (FAA). On June 21-22, 2012 the FAA conducted an inspection to determine compliance with 14 CFR part 139. They wrote the following regarding drainage in their Letter of Correction to the Airport: 1. 139.305 (a)(6) - Paved areas Taxiway and ramp pavement is not properly drained to prevent ponding. Taxiway C centerline, holding position markings, and surface painted signs were obscured by water. Excessive ponding was present on the general aviation ramp,along the edges of Taxiway A at the West side of the airfield,at the Taxiway C holding position,Taxiway A I-A4,Taxiway A7 and A8, and along the West side of the Taxiway E holding position marking. 2. 139.309 (b)(2) - Safety areas The Runway 9-27 safety area on the South side of the runway is not properly drained. Excessive water accumulation was present almost the entire length of the runway between the runway pavement and Taxiway A. Jacob's recommendation was to pursue a phased implementation of Alternatives. The first phase Would be for improvements that Could be implemented with minor permitting consequences in both time and effort. The following list the recommended Phased Improvements: Phase 1 • Phase I Permitting o SFWMD Letter modification of the existing ERP for Phase I Improvements. o Facility wide permit for the operation of the 19 wells on the Airport property with FDEP. The area wide permit will require the evaluation of each drainage well's physical characteristics and current condition. The existing drainage wells require routine maintenance and inspections to verify that the sedimentation basins, prior to the wells, and that each well is cleaned and operating properly. • Perform routine maintenance and inspection of existing drainage wells to verify each drainage well's physical characteristics and current condition. Clean each well and sedimentation basin as required. • Grade/ maintain infield areas in accordance with FAA advisory circular 150/5300-13A. • Construct Exfiltration trenches to expedite the removal stormwater runoff for the airfield Page 2 of 6 and reduce the volume of stormwater that overflows to the adjacent salt ponds. Phase 2 Develop plans and Letter Modification permit documents for Interconnection of existing drainage basins. This will connect the runway infield areas with taxiway infield areas which will allow for a connection to the phase I exfiltration trenches and the phase 3 pumped drainage wells. Phase 3 Develop plans and Full Permit Modification documents for pumped drainage wells and low berms at fence perimeter. DETAILED SCOPE OF DESIGN SERVICES FOR PHASE 1 Jacobs will provide planning and design services for the following: Article A — SFWMD Permit Jacobs will prepare an SFWMD Letter modification of the existing ERP for Phase A Improvements. Services for Article A will be provided under a lump sum basis. Article B — Design Plans and Specifications Jacobs will prepare plans and specifications for: I Obtaining drainage well physical characteristics and current condition, for nineteen drainage wells. The plans will also address routine maintenance and cleaning of the wells as required. 2 Backflow devices for twelve gravity drainage wells. 3 Two exfiltration trenches, approximately 800 feet in total length. Drawings and Specifications will be prepared for 90%and final contract document. Design will be developed for all elements identified in this scope of work. • Develop technical specifications using the FAA's standard specifications for construction. • Coordinate and incorporate Monroe County provided contract document/frontend specifications into the final contract bid documents. (i.e. Notice to Bidders, Invitation to Bidders, Contract, etc.) • Prepare for and attend two (2) meetings at the Airport to discuss project progress and review the design submittals. • Prepare for and attend one (1) meeting at the Airport to discuss construction phasing with Airport Staff and Tenants. • Provide a detailed construction cost estimate for the project at the 90%® and at final contract document design levels Page 3 of 6 • Prepare FAA construction management plan • Develop the following anticipated plans. o Title Sheet(1) o General Plan (2) o Notes and Summary of Quantities (1) o Existing Conditions Plan(l) o Proposed Works plan (3) o Drainage & Miscellaneous Details Plan (1) o Drainage/Erosion Control Plan (1) o Drainage/Erosion Control Details (1) o General Phasing and Safety Plan (2) o Detailed Phasing and Safety Plan (2) Total 15 Sheets 0 Coordinate, distribute and print the following copies of the preliminary construction documents (plans, specifications, and construction cost estimate for review by Monroe County, FAA, and FDOT Preliminary Submission (90%, Final Contract Docurnents)— Monroe County 2 copies of plans (I full size, I half size); 2 copies of specifications; 2 copies of estimate for each submission. FAA 2 half size copies of plans and a CD containing the drawings, specifications, and engineer's estimate for each submission FDOT 2 half size copies of plans and a CD containing the drawings',, specifications, and engineer's estimate for each submission. • Conduct an in-house QA/QC review prior to each submission. The in-house QAd check will involve an experienced independent individual, depending on each discipline,to conduct a comprehensive check on all documents to be submitted (i.e. plans, specifications, estimates, etc.). Time will be required for engineers and CAD D technicians to correct items listed under the in-house QA/QC review. • Address final comments from 90%design for final contract documents. Services for Article B will be provided under a lump sum basis Article C - FDEP Drainage Wells Permit Utilizing the information obtained from Article B, item 1,Jacobs will prepare an application for an FDEP area wide permit,for the operation of the nineteen drainage wells on the Airport property. Page 4 of 6 Services for Article C will be provided under a lump sum basis. Article D—Advertising and Bidding Services I. Supply electronic copy of plans and specifications for use in the County's electronic bidding system. 2. Prepare an "Invitation to Bid", all forms for advertisement, bid proposals, contract bonds, labor and DBE requirements and other contract documents needed to solicit public bids for the construction of the project. The Owner shall be responsible for the cost and placement of all public advertisements required including all newspapers in which the advertisement is placed. 3. Prepare for and attend the Pre-Bid Conference, one (1) day. 4. Prepare and respond to Bidders questions. 5. Issue necessary Addenda and Directives to Bidders. One (1) addendum is assumed. 6. Review the bids received, prepare the final bid tabulation, and make recommendations/rejections of award of the Contract to the Owner. 7. Prepare construction contract drawings and specifications. Services for Article D will be provided under a lump sum basis. Article E—Post Design Construction and RPR Services Jacobs will provide a part time RPR for daily inspection and coordination with Airport Staff. The RPR is estimated to be required an average 4 hours/day for the 4 week construction period, with coordination time before start of construction and for closing out the project after construction completion. Jacobs will perform Construction Contract Administration with RPR services being provided through sub consultant Perez Engineering & Development Inc. The construction phase will commence with the award of the contract for construction services. The construction-related services include the following services: • Provide office administrative support in order to respond to contractor RFI's, Change Orders, Pay Requests, and Request for Field Modifications. • Assistance with processing of pay request and change orders. • Attendance at a pre-construction meeting. • Review of shop drawings. • Assist County regarding resolution of construction issues. • Review record drawings prepared by the Contractor. • Attend final walk through with Contractor. • Prepare final punch list and Statement of Completion of construction documents. • Coordinate as required for Government Agency sign-off and Permit Certifications Page 5 of 6 Jacobs will not be responsible for construction means, methods, techniques, sequences, or procedures,for safety precautions and programs in connection with the work,or for the acts or omissions of the contractor. See attached proposal / Scope of work from Perez Engineering & Development Inc. for RPR services Services for Article E will be provided under a lump sum basis. ADDITIONAL SERVICES Services not included and not anticipated in the scope of work, include, but are not limited to the following: • Design and permitting of Phase 2 and Phase 3 of the EMERGENCY DRAINAGE CORRECTIVE ACTION PLAN TECHNICAL MEMORANDUM, dated May 30, 2013. • Building Department Permitting. • No offsite Roadway improvements are anticipated. • No offsite utility improvements are anticipated. • It is not anticipated that wetlands, wildlife habitat or soil/ groundwater contamination will affect the design or construction on the site or that any government agency permitting or mitigation will be required for these environmental concerns by Jacobs. Government Agency cost for all building and engineering permit fees are not included in this proposal. Jacob's assume the owner will supply signatures for permits or designate a responsible entity for such. Items identified as not included or limited in the scope of services can be provided or expanded upon as additional services under a separate agreement or supplemental to this agreement. ( ) is pleased to submit this revised proposal for the above-referenced project. The proposal is based on information provided by Jacobs. It is our understanding that Bore Hole Permeability (BHP)tests by FDOT method will be required SCOPEPROPOSED As requested, we will perform a total of twelve (12) BHP tests in four test holes per FDOT method as described in FDOT "Drainage Handbook Exfiltration Systems". BHP tests will be performed at depths of 10 feet, 15 feet, and 20 feet in each test hole (i.e. three BHP tests in each test hole). The BHP tests will be performed at the approximate locations provided or approved by Jacobs. BHP test locations will be located in the field by Jacobs. Prior to drilling at the project site, TSF will notify the local utility companies and request that underground utilities be marked. Our experience, however, is that the utility companies will not mark privately owned utilities. We have included Ground Penetrating Radar ( ) in our estimate to clear utilities at the boring locations. It is important to note that while the use of GPR will minimize the potential of striking utilities, it is not a 1001/14 accurate. Therefore, private utilities will be the owner's/Airport's responsibility. We believe that significant coordination will likely be required to perform the field operations. As we understand, the all work will be conducted in the day time. It is our understanding that Jacobs or the Department of Aviation will provide an escort to the test locations. BHP test results will be provided upon completion of the field work. SCHEDULE AND AUTHORIZATION TSF will proceed with the work after receipt of a signed copy of this proposal. With our present schedule, we can commence work within several days of project approval (weather permitting) and fieldwork is expected take about 2 to 3 days to complete. We shall provide a resident inspector who is qualified to make interpretations, decisions, field computations, etc. and has knowledge of testing requirements and procedures. I —SCOPE OF SERVICES 1. Prepare for and attend a pre-construction meeting. 2. Check activities to ensure compliance with the plans and specifications. Inform the County and Jacobs of any work that is non-compliant. 3. Ensure that all testing required by the specifications are a performed. Ensure that all materials used are accompanied by the numerical test results or a certification from the manufacturer that the material meets the applicable standards. 4. Ensure material tests are performed at the frequency stated in the specifications and witness the tests as they are performed; as required. 5. Review the test reports and certifications for conformance with the specifications and maintain a file for all reports and certifications. 6. Inform the County and Jacobs of deficiencies so corrections can be made and retesting performed. 7. Document quantities of materials used on the project by actual field measurements and computations in a field book. 8. Review pay request applications from the contractor. 9. We shall not have control over or charge of and shall not be responsible for construction means, methods, techniques, sequences or procedures of construction or for safety precautions and programs in connection with the work, since these are solely the CONTRACTOR'S responsibility under the contract for construction. We shall make every reasonable effort to ensure that the CONTRACTOR completes the work in accordance with the current approved schedule and carries out the work in accordance with the contract documents. 11. ITEM 2-INDEMNIFICATION The CLIENT agrees, to the fullest extent permitted by law, to indemnify and hold the Consultant harmless from any damage, liability, or cost (including reasonable attorneys' fees) to the extent caused by the CLIENT's negligent acts, errors or omissions and those of anyone whom the CLIENT is legally liable, and arising from the project that is the Subject of this Agreement. The Consultant agrees, to the fullest extent permitted by law, to indemnify and hold the CLIENT harmless from any damage, liability, or cost (including reasonable attorneys' fees) to the extent caused by the Consultants negligent acts, errors or omissions in the performance of professional services under this Agreement. ITEM 3-TERMINATION The CLIENT, through written notice to Perez Engineering & Development, Inc. may terminate this agreement in whole or in part (without cause) for the owner's convenience. This agreement is not a guaranteed total fee. If such a termination takes place, Perez Engineering & Development, Inc. will receive payment for all work performed prior to receiving notice to cease work. Perez Engineering & Development, Inc. will also be paid for any costs incurred in association with their stopping work on the project.
