Dear Mr. Mafera:
We are pleased to be of assistance with regard to the above -referenced site, which we understand is
developed with one 5,200-square-foot, slab -on -grade, single -story building. The building is
currently vacant and slated for demolition. In response to your request, we have prepared this
proposal to conduct a pre -demolition asbestos and hazardous components survey as well as limited
lead -based paint screening. This proposal outlines the site -specific proposed scope of services, cost
estimate, and schedule for completion.
SCOPE OF PROPOSED SERVICES
Asbestos Survey
HSA Golden will perform a pre -demolition asbestos inspection of the site building, in general
accordance with the protocols established in Asbestos Hazard Emergency Response Act (AHERA)
and the National Emission Standard for Hazardous Air Pollutants (NESHAP) to identify, quantify
asbestos containing materials (ACMs), pursuant to US EPA definitions. The inspection will be
performed by an AHERA-certified Asbestos Inspector under the direction of a State of Florida -
Licensed Asbestos Consultant (LAC). The scope of the asbestos survey will include all interior and
exterior building materials utilized in the construction of the building, including the building roof.
Sample collection will be limited to accessible areas within and around the Site building.
All bulk samples will be delivered to an American Industrial Hygiene Association (AIHA) :: ational
Voluntary Laboratory Accreditation Program (NVLAP) accredited laboratory for laboratory analysis
by Polarized Light Microscopy (PLM) using dispersion staining techniques, in accordance with EPA
Method EPA/600/R-93/ 116 (July 1993). In accordance with this sampling method, each individual
layer of multi -layered materials will be separated prior to analysis. The samples will be submitted
to the laboratory under a seven-day turnaround time. Results will be reported for each building
material layer. The entire assemblage of layers of an analyzed building material will be considered

to be ACM, or asbestos -contaminated material if one or more layers of a given sample are found to
contain asbestos. Based on the building dimensions, HSA Golden estimates up to 50 bulk building
material samples will be collected and submitted for laboratory analysis by PLM under the scope of
this proposal.
Hazardous Components Survey
HSA Golden will visually inspect the building's existing lighting systems for the presence of
polychlorinated biphenyls (PCBs). Random fluorescent light fixture ballasts will be inspected for
a "non -PCB" label (10% frequency). Ballasts that exist without a "non -PCB" label will be assumed
to contain PCBs.
HSA Golden will inspect existing lamps to identify fluorescent and High Intensity Discharge (HID)
lighting lamps that are currently used or will be removed prior to building demolition. Currently,
all fluorescent and HID lighting lamps contain mercury that must be disposed of as hazardous waste;
however, since 1998, new low-level mercury lamps have been available for installation in new
construction. If accessible, fixtures that contain the older mercury lamps (which must be disposed
of as hazardous waste) and those with newer low-level mercury lamps (which are classified as non-
hazardous waste) will be differentiated during the building inspection.
HSA Golden will inspect accessible Heating Ventilation and Air Conditioning (HVAC) switches,
switchgear or switches associated with refrigeration systems for the presence of mercury. HSA
Golden will inspect accessible areas throughout the Site building for containers which may contain
hazardous components including, but not limited to, electric transformers with thermoelectric oil,
paints and solvents, cleaning supplies, fuels and stored batteries.
If available, Material Safety Data Sheets will be reviewed from available manufacturer information
to determine the presence of hazardous materials in the components identified. If equipment is
identified that is suspected to contain PCBs, mercury, or other hazardous materials, an inventory of
the equipment will be conducted including digital photographs and written descriptions of field
observations and labeling.
Lead -Based Paint UP) Screening
HSA Golden will conduct a limited LBP screening to test representative metal components within
the Site building (such as door frames, support beams, handrails, etc.) that are suspected to contain
lead. The collected paint chip samples will be submitted, using chain of custody procedures to a
laboratory accredited through the Environmental Lead Laboratory Accreditation Program (ELLAP)
for analysis for lead only utilizing Atomic Absorption Spectroscopy (AAS) in accordance with
ASTM Standard E-1613 and EPA Method SW-846/6010. The samples will be submitted to the
laboratory under a seven-day turnaround time. The results of laboratory analyses will be compared
to the HUD and EPA definition of Lead Based Paint, as well as guidance documents from OSHA.
Positive results of the analysis will be reported in percent lead by weight.

Report Preparation
HSA Golden will provide, under the direction of an LAC, a complete and detailed report assessing
existing conditions. The report will include, but will not be limited to, the following:
• A summary of ACM homogeneous areas and sample analysis results;
• A description of ACM material friability, quantity and observed physical condition;
• A description of ACM Location in the Site building;
• A discussion of federal, state, and local applicable regulatory requirements;
• A summary of hazardous components identified; and
• A summary of LBP sample areas and analytical results.
The report will include certifications of the building inspector and analytical laboratories, and the
State of Florida license of the Asbestos Consultant. General recommendations for abatement of
identified ACMs will be provided. The final report will be reviewed and signed by a Florida LAC
and electronic copies of the report will be forwarded to you.
CLIENT -FURNISHED INFORMATION
If available, project -related technical data including available site plans or drawings and previous
asbestos testing reports are requested. It is understood that Client will provide arrangements for
HSA Golden personnel to gain access to the building interior and its roof at the agreed time of the
field work, and will arrange for the building to be vacant at the time of the building inspection and
bulk building material sampling.
LIMITATIONS
The bulk building material sample collection for this Pre -Demolition Asbestos Survey will be
destructive, and will involve gross damage to the building, where necessary. Roofing material
samples will be collected to the deck substrate. This roof sampling may void a standing roof
warranty, and it should be expected that the building roof will leak as a result of the asbestos survey
sampling at some point in time. If the specified site building is to be occupied during or following
the asbestos survey sampling, or if considerable time may elapse between the performance of this
building asbestos survey and the onset of demolition, you may wish to forego roof sampling until
such a time when asbestos survey sampling and subsequent roof leaks would not impair site
operations or create moisture related issues.
SCHEDULE AND FEE ESTIMATE
When site access is provided, HSA Golden will expediently perform the field work for this Scope
of Services. With your confirmed site access, the field work for this project will be conducted within
10 days of receipt of this signed proposal. HSA Golden will provide the written report within three
weeks of initiation of field work. Unforeseen site conditions or project delays beyond the control

of HSA Golden may result in an adjustment to the indicated schedule. Should such conditions arise,
HSA Golden will notify the you as soon as possible.

The cost estimate was prepared assuming up to 50 bulk asbestos samples to be analyzed by PLM and
10 paint chip samples to be analyzed by AAS. If additional samples are necessary to thoroughly
characterize on site building conditions, we will notify you prior to submittal of samples to the
laboratory. If additional asbestos bulk samples or paint chip samples are required, the additional
samples will be collected, analyzed and invoiced at a per -sample fee of $20.00 and $30.00
respectively.
