environment for airport staff to perform routine maintenance.

This project consists of design, environmental, permitting and bidding phase services associated with the
construction of an airport maintenance facility at the Key West International Airport(AIRPORT). The project will 00
consist of the construction of an elevated parking deck. The deck will be approximately 100' x 100' to its furthest
extents although the footprint will not be square and will be configured to accommodate the site restrictions (see
Figure 1). A portion of the deck will be covered with a roof. The overall construction will match in both type and
in appearance to the existing parking deck and the roof of the terminal building.

Additional project considerations will include:

➢ The new deck area will connect to the upper deck of the existing parking garage. o
➢ Existing landscaping just south of the garage ramp will be left in place.
➢ New parking at grade will be constructed underneath the deck adjacent Nancy Cherry Lane.
➢ The existing stair on the South-West corner of the garage will likely need to be relocated.
➢ Only minor maintenance of vehicles will be performed on the deck (i.e. items normally associated with 0
major maintenance work such as lifts,monorails,trench drains etc. will not be provided).
➢ Existing operations of the parking garage cannot be affected during the construction.
Design efforts will include topographic survey of the project area and geotechnical investigation to evaluate the 0
existing soils,presence of groundwater,and the ability of the existing soils to support the anticipated design loads.
It is anticipated that the entire site will be available to the contractor for construction with minimal amount of traffic
maintenance and protection required, specifically as it relates to the control of the access and egress points along
Faraldo Circle and Nancy Cherry Lane.

1. ADMINISTRATION/PROJECT MANAGEMENT

The following items of work shall be completed under the Administration/Project Management task: a
A. The coordination with the AIRPORT staff and Utility Company personnel regarding owned and/or
operated facilities that may be impacted by the project.
B. Procurement and coordination of special services such as topographical survey and mapping,
subsurface investigations and geotechnical report, architectural code review services, and cost
estimating services necessary for completion of the work to be done under this contract. This
includes the preparation of the necessary subcontract documents, negotiation, and/or bid

solicitation and award.
C. Submission of construction documents to COUNTY for a code review, allowing the contractor
to obtain a building permit.
D. Attend up to six (6) meetings with the AIRPORT and/or Monroe County planning and building
department representatives to facilitate coordination and project reviews.
2. CONCEPTUAL DESIGN
A. Conduct a pre-design meeting with AIRPORT staff,and other appropriate agencies.
B. Engineer will develop a proposed concept plan for the site for review by AIRPORT staff and
ultimately submission to Monroe County Building Department for review by a County Planner. A
meeting will take place with the County Planner to determine the level of site plan approval
required by the County.

C. Complete an investigation to gather the necessary data for design of project. The investigation will
consist of a review of the existing data such as survey information, geotechnical data, as-built
utility drawings and parking deck construction drawings and other existing data, as it is readily 19
available. U)
D. A sub-consultant to CONSULTANT will conduct a topographic and boundary field survey of the 00
proposed project's property. The field work will include survey documentation of project features
including pavement edges, drainage features, lighting, signage and grading within the project limits
as well as boundary survey information and obtaining the Property Record Card from the County.
E. A sub-consultant to CONSULTANT will obtain the necessary soils investigation data, including cv
borings, as well as field and laboratory testing to identify existing soil conditions and subsurface
characteristics. The sub-consultant will provide a geotechnical report with site preparation
requirements and recommended foundation types. o
F. Develop conceptual site designs of facility site layout including grading and drainage concepts and
construction sequencing. The information to be included will be:
➢ Layout Plan(1 sheet).
➢ Grading Plans& Drainage Plans(1 Sheet).
G. CONSULTANT will develop conceptual building plans and elevations incorporating space/layout
and storage requirements as agreed to at the pre-design meeting, including: 0
➢ Development of approximate dimensions,and areas.
➢ Review of the existing structural and architectural systems for their adequacy for the new
construction.
➢ Conduct initial building and life safety code analysis.
H. Develop conceptual plumbing plans for the building including:
➢ Stormwater management drainage system.

J. Develop a conceptual phase opinion of probable cost. This estimate will be compared to the
AIRPORT's budget to determine if the structure, and primarily the covered area, needs to be
reduced in size or other modifications to the construction need to be made in order to bring the
project within the established budget.
K. Meet with the COUNTY to review initial building and life safety code analysis.
L. Schedule and conduct a meeting with the AIRPORT and other stakeholders to review the
Conceptual Design results.
M. After the meeting and receipt of comments from the AIRPORT, CONSULTANT will revise the
Conceptual Design. Up to two rounds of revisions are included. cv

3. PRELIMINARY DESIGN(50%) 00
A. Based upon the Conceptual Design and facility programming requirements the CONSULTANT
will further develop the conceptual building floor plan depicting the building dimensions, sections
and elevations, building structural systems, materials, and vehicle circulation/parking. The site
plans will also be further developed depicting the existing conditions, the proposed building,
parking area,and vehicle drive paths.

C. Prepare a Stormwater Prevention Plan and submit to AIRPORT and COUNTY for
review/approval.
D. CONSULTANT will prepare preliminary set of specifications (project manual). Construction
Specification Institute (CSI) specifications will be used when possible for the building systems,
while FDOT specifications will be utilized for site work improvements. When special
specifications are required, they will be prepared in the same format as the other technical
specifications and will be assigned an individual identifier that distinguishes them from the
standard specifications.
E. Prepare and submit a FAA Form 7460-1, Notice of Proposed Construction or Alteration,
through the online FAA OEAAA system. One form will be submitted for temporary
construction equipment(i.e. crane)and one will be submitted for the permanent structure.

F. Prepare a Preliminary Engineer's Opinion of Probable Costs for each major element in the
project. This estimate will be compared to the AIRPOORT's budget to determine if portions of
the bid plans should be revised/modified in order to bring the project within the current budget. -19

G. Submit preliminary plans and specifications to the AIRPORT for review. 00
H. Preliminary plans and specifications will be submitted to the Building Department/Planning Board .�

4. FINAL DESIGN(100%)
A. Submit a 95%set of plans and specifications to the AIRPORT for review.
B. After receipt of 95% set review comments from the AIRPORT, complete the drawings and
specifications for bidding and construction purposes.
C. Prepare a bid package, incorporating comments received during the preliminary design phase, to
reflect the specific requirements of the COUNTY. This will include the Notice to Bidders, the
standard contract form, the bidding forms, contractor assurances and questionnaire, the standard
provisions and conditions, and the specifications.
D. The bid set drawings will be stamped and signed by an engineer/architect registered to practice in
the State of Florida.
E. Prepare final quantity take-offs from the various design documents, by type of material or other
specification identifier. An updated opinion of probable construction costs will be generated and
compared with the available budget for the project.

F. Obtain permits, as required, to allow construction of the project. At this time, the anticipated
permits required for this project are as follows:
• Site Plan approval from the COUNTY for the proposed improvements on the site.
• Stormwater Prevention Plan and Operational Phase Pollution Prevention Plan approvals
from the COUNTY, FDEP and SFWMD as appropriate.
G. Attend a final project meeting with AIRPORT staff to review the final construction documents
and determine a project bidding schedule.
H. Provide electronic print ready copy of the bid set to the COUNTY for advertisement via
DemandStar.
5. BIDDING AND AWARD ASSISTANCE
A. Attend and facilitate a project pre-bid meeting to discuss specific project requirements with N
prospective bidders and answer any questions. Document the meeting proceedings in a formal
set of meeting minutes.

B. Provide responses to questions received from potential bidders through the COUNTY,and issue M
formal contract addendums as required. 00
C. Prepare and compile bid results, contact the selected contractor, and draft award letters _
preliminary to the start of construction. The CONSULTANT will also assist the AIRPORT in
reviewing and forwarding contracts for execution by the selected construction contractor.

- The estimated project budget (design and construction) based upon discussions with the
SPONSOR is$2,350,000.
- Bidding for the project is anticipated to occur in the Fall of 2019.
- Application, review and permit fees will be the responsibility of the AIRPORT.
- Monroe County Building Codes require that structural foundations extend deeper than the
Florida Building Code standards.
- This contract provides services only through the bidding and award process. A separate
contract will be executed for the construction phase.
- Preparation of Short Environmental Assessment not included in scope.
- NPDES Industrial Discharge Permit not included in scope.
PROPOSED SCHEDULE OF COMPLETION
The CONSULTANT agrees to complete the work under this phase of the Agreement in a manner satisfactory to the
SPONSOR within 6 months after receiving an executed copy of this contract from the SPONSOR accompanied by
a resolution from its governing body authorizing said execution or within such extended periods as agreed to by the

The topographic survey will be performed to support the design of a proposed maintenance facility

Limits:

The survey limit is based on the exhibit you provided in your e-mail dated February 18, 2019 and
shall extend to the full right-of-way of Feraldo Circle all within the area defined in green on Exhibit
A(below), incorporated herein for reference. _
Scope:
Topographic Survey:
Obtain sufficient elevations to define the existing topography in detail based on a 25-foot by 25-
foot grid on impervious surfaces and 50-foot by 50-foot grid on natural ground surfaces, including
significant grade changes in elevations. The elevations shall be referenced to the North American
Vertical Datum of 1988 (NAVD 88) and horizontally referenced to the North American Datum of
1983 (NAD 83-2011) Epoch 2010.00 and tied to the National Geodetic Survey(NGS) benchmark
network. Additionally, we shall establish a minimum of two (2)benchmarks at the site. We shall
calculate and place the vertical conversion factor from NAVD 88 to the National Geodetic Vertical
Datum of 1929 (NGVD 29) on the face of the map.
We shall locate all above ground improvements including, but not limited to pavement, ramps,
driveways, steps, gates,top of curb, bottom of curb, edge of pavement, pavement markings,

centerline of pavement, bottom of gutter, back of walk, face of walk, top of bank, toe of slope,
and above ground evidence of utility. We will obtain rim elevations,pipe size, material and inverts
of sewer and drainage structures where possible. Due to the nature of obtaining the data where the
structures are in use, the information obtained may be the best approximation of the true
measurements. It is the responsibility of the end user to verify the diameter and material utilizing
as-built drawings or other resources prior to construction. Utility locations will include fire
hydrants, water valves, meter boxes, vaults and electrical outlets, power poles and overhead wires.
We will locate wells, main irrigation valves, but will not locate individual sprinkler heads.
There is no provision for the excavation, probing or location of underground utilities structures or
improvements. Utilities shall be located to the extent that they are above ground and visible. This
proposal includes the contracting of an underground utility locating service to locate the
subsurface utilities (see attached proposal by our consultant under separate cover).
The survey data will be reported to the nearest 0.03 feet for horizontal locations and for the vertical 19

component, 0.1 feet spot elevations on ground shots and 0.01' spot elevations on paving or hard iM

surfaces, with all work performed in accordance with the Standards of Practice as defined mi
Chapter 5J-17, Florida Administrative Code.
We shall obtain all recorded plats and right-of-way maps to graphically delineate the platted
boundary lines within the digital files. Surveyor shall recover sufficient monumentation in the
field to spatially place the boundary within the digital file. There is no provision to perform
0
boundary surveys or surveys to resolve conflicts, ownerships or occupation issues relative to the
recorded instruments. This information will be spatially placed within the digital file of the
topographic survey, based on recovered monumentation to depict the lines relative to the
topographic locations. This is not a boundary survey and the information should not be relied
upon as such. It is strongly recommended that if the design is contingent on accurate boundary 0
placement and especially prior to construction,a boundary survey should be performed to establish
the lines. We will not be responsible for the misuse of the topographic survey for purposes it was
not intended.
The topographic survey limits shall also include the interior and top of the garage structure.
Tree Survey:
We will locate all indigenous and protected specimen trees having a 3" and larger caliper diameter
at breast height relative to the boundary, with the tree species listed to the best of knowledge and
ability of the surveyor (without the benefit of a botanist or landscape architect). It is the
responsibility of the end user to verify the identity of the species.
We will tag each tree with a number and obtain the diameter and horizontal position of the trees.
It is the responsibility of the client's consultant (botanist or landscape architect) to correctly
identify the species relative to the tag number, which we will provide as an Excel tabulated listing.

The Excel tabulated table listing which will comprise of the tag number, diameter and species of
trees(corrected by the client's consultant and returned to us) will be incorporated into our survey.
The location of shrubbery and landscaping is not included in this proposal. Large,bunched groups
of trees shall be located as clusters with a count and diameter listed. We will not locate exotic or
non-native species.
Mangroves consist of prop roots and by nature do not possess a defined tree trunk; therefore, the
edge of the mangroves will be located.
Clarifications:
1. Client shall arrange for all access to the property. Delays caused by the inability to gain access
to the property at no fault of Avirom& Associates, Inc. may incur additional charges.
2. Requirements to the survey requested by governmental agencies that are more stringent than -19

Standards of Practice as set forth in Chapter 5J-17 FAC pursuant to Section 472 Florida i�
Statutes will be an additional expense to client based on our contracted hourly rates. 00
3. This proposal is based on mobilization by our crew from Boca Raton to service the project.
4. This proposal is based on the site being free and clear of storm debris. We will not move
obstacles to obtain survey data, or if field conditions are considered hazardous to the safety of
our crews.

5. The Tree Survey is based on performing the field work in conjunction with the topographic
survey.
Deliverables:
Two signed and sealed hard copies of the survey (including a PDF file of the hard copy) shall be
furnished to the client, together with an AutoCAD (2017 or lower) digital drawing file created in
Carlson 2017.
Time
frame:
We can mobilize to the site within two (2)weeks after a notice to proceed, contingent on available
accommodations and have the project completed within four (4) weeks after de-mobilization.

EQUIPMENT00
• 400/350 MHz GPR Antenna. The antenna is mounted in a stroller frame which rolls over the surface. The surface needs to be
reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs,landscaping,and vegetation will
limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. GPR works by sending pulses of
energy into a material and recording the strength and the time required for the return of the reflected signal. Reflections are
produced when the energy pulses enter into a material with different electrical properties from the material it left. The strength of
the reflection is determined by the contrast in signal speed between the two materials. The total depth achieved can be as much as
8'or more with this antenna but can vary widely depending on the conductivity of the materials. Depths provided should always be cy

• Electromagnetic Pipe Locator.The EM locator can passively detect the electromagnetic fields from live AC power or radio signals O
travelling along some conductive utilities.It can also be used in conjunction with a transmitter to connect directly to accessible,
metallic pipes,risers,or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting EM
field can then be detected by the receiver. A utility's ability to be located depends on a variety of factors including access to the
utility,conductivity,grounding,interference from other fields,and many others.Depths provided should always be treated as

• Traceable Rodder.The rodder has a copper wire encased in 5/16"of fiberglass. The line is pushed through accessible pipes before
placing a current on the wire which is then traced from the surface. The maximum traceable depth is 10'depending on the soil
conditions and the maximum distance is 300'. The line can be pushed through a pipe with direct access such as a sewer line at a
cleanout or a storm drain catch basin. It may not be able to be pushed through deeper pipes within manholes and conduits will not
be accessed by GPRS. The signal cannot be located through metallic pipes. For more information,please visit:Link
OPTIONAL EQUIPMENT
• GPS. This handheld GPS unit offers accuracy down to 4 inches,however,the accuracy will depend on the satellite environment and
obstructions and should not be considered to be survey-grade. Features can be collected as points,lines,or areas and then
exported into Google Earth or overlaid on a CAD drawing. For more information,please visit:Link
• Sewer Camera. The sewer camera has a 2"diameter,self-leveling camera head that provides high quality images in pipes up to 9"in
diameter. There is also a 512 Hz sonde behind the camera that can be located from above ground and provide the approximate
depth to the pipe for pipes up to 10'-15'deep. The locatable signal from the sonde will not pass through metal pipes or solid metal
barriers except for cast iron. The camera and sonde are pushed through the pipe and access will need to be obtained through a
cleanout or an open pipe within arm's reach and can be pushed through for a maximum of 200'. Access to confined spaces such as
manholes would need to be provided and facilitated by the client. Video and photos of the interior of the pipe can be provided but
GPRS does not provide inspection services or make judgements regarding the integrity of the pipes.For more information,please

As requested, Tierra South Florida,Inc. (TSF) is pleased to submit this proposal for the above-
referenced project. Based on information provided by McFarland-Johnson,proposed construction 0
will consist of an elevated (matching top of current parking deck) airport maintenance parking U)
facility with a partial roof(roof to match terminal roof). The existing structure is founded on i�
auger-cast piles. 00
PROPOSED SCOPE OF WORK
Our scope of work will consist of
• Four(4) Standard Penetration Test(SPT)boring to a depth of 30 feet;
• Two(2)Constant Head permeability tests to a depth of 15 feet per SFWMD method; 0
• A report will be issued that contains the exploration data, permeability test results, a
discussion of the site and subsurface conditions, geotechnical recommendations for
foundation design, and a discussion of some construction considerations.
Prior to drilling at the project site, TSF will notify the local utility companies and request that
underground utilities be marked. Our experience, however, is that the utility companies will not
mark privately owned utilities. Private utilities will be the owner's/Airport's responsibility. Our
proposal assumes that private utility lines will be located in the field by others prior to
mobilization of the drill rig.TSF will recommend a utility line locating service upon request.
We believe that significant coordination will likely be required to perform the field operations.
This proposal is based on the assumption that all boring locations are open and accessible to
a truck mounted drill rig.
ESTIMATED FEES
It is proposed that the fee for the performance of the services outlined above is determined on a
unit price basis in accordance with the attached Fee Schedule and that the work be performed
pursuant to TSF's General Conditions enclosed herewith and incorporated into this proposal. On

the basis of the proposed quantities, it is estimated that the total fee will be approximately
$16,885.00.
Our estimate covers the work needed to present the results.Not included are reviews of drawings,
preparation of construction specifications, special conferences and any other work requested after
submittal of our report.
SCHEDULE AND AUTHORIZATION
TSF will proceed with the work after receipt of a signed copy of this proposal. With our present
schedule, we can commence work within several days of project approval (weather permitting)
and fieldwork is expected take about 2 to 3 days to complete. The report can be submitted in about
three weeks after completion of the field work.
