Article E.—Bid Phase Services
Upon completion of the construction documents and at the request of the Airport,Jacobs will <
prepare the project documents for bidding., Jacobs will perform bidding support services,the
following terms will apply:
* The County appoints Jacobs as its Agent, and Jacobs accepts such appointment to perform
bid/procurement services on Monroe County's behalf in connection with the project
* Such services shall be performed under County's direction and in accordance to such
forms, terrns and conditions, or modifications or revisions to same as County may in its
sole discretion at any time instruct Jacobs to use. All services shall be carried out in
accordance with the procedures mutually agreed upon by County and Jacobs,

Jacobs shall not have authority to accept or bind the County in any way to changes,
modifications, revisions, alterations, amendments, or supplemental, additional, or
different terms and conditions (hereinafter referred to as "deviations") which may be
submitted or requested by a vendor or contractor. Jacobs shall immediately submit any
deviations from the County's standard terms and conditions to the County for review by
the County's Purchasing Manager or their representative and such deviations shall not be
accepted by Jacobs unless Jacobs receives express written approval thereof from the
County's Project and/or Purchasing Manager or their representative.
All documents issued by Jacobs hereunder shall be signed by Jacobs for the County if so
directed. The ownership and title of all items purchased hereunder shall pass directly
from the selling party to the County, and Jacobs shall at no time be a party to such
transaction other than as agent of the County. The County shall have the unilateral right
to have the cominitment authority of Jacobs, its employee or this limited agency LO
authorization in its entirety revoked and cancelled at any time, with or without cause. It-

For the purpose of providing a stipulated fee amount, this proposal has been based on the
following qualifications and assumptions:
• Design shall be based on site walk through and assessment report.
• Permit issuance related work such as processing plans tJarough. City, County,
and/or State agencies, other than plan revisions requested by such agencies, are
included.
• Government Agency cost for all building and engineering permit fees are not
included in this proposal.

• Construction Aditunistration Services are not included.
• Lightning protection will not be provided.
• Attendance and/or presentation to public hearings and/or other meetings with
municipal agencies are not included.
• Municipal Agency and all associated permit fees are to be paid by the client.
Jacobs is not responsible for obtaining any permits.Permits are to be obtained by
the contractor.Jacobs will monitor the contractor's progress through the permit
process for the client and provide follow up periodically with the permitting
agency to verify the status,but it is ultimately the responsibility of the contractor
to obtain all permits.
® Services do not include"design to cost".Jacobs will assist the owner in
evaluating and offering cost reduction strategies as requested by client.The
"design to cost"is basically noting that if Jacobs receives direction from the I`
client or one of its sub-consultants on how to design the project and if any LO

Article E-Bid Phase Services M
Upon completion of the construction documents and at the request of the Airport,Jacobs will <
prepare the project documents for bidding. Jacobs will perform bidding support services,the
following terms will apply:
* The County appoints Jacobs as its Agent, and Jacobs accepts such appointment to perform
bid/procurement services on Monroe County's behalf in connection with the project.
* Such services shall be performed under County's direction and in accordance to such
forms, terms and conditions, or modifications or revisions to same as County may in its
sole discretion at any time instruct Jacobs to use. All services shall be carried out in
accordance with the procedures mutually agreed upon by County and Jacobs.

® Jacobs shall not have authority to accept or bind the County in any way to changes,
modifications, revisions, alterations, amendments, or supplemental, additional, or
different to and conditions (hereinafter referred to as "'deviations") which may be

deviations from the County's standard terms and conditions to the County for review by
the County's Purchasing Manager or their representative and such deviations shall not be
accepted by Jacobs unless Jacobs receives express written approval thereof from the
County's Project and/or Purchasing Manager or their representative.,

* All documents issued by Jacobs hereunder shall be signed by Jacobs for the County if so
directed. The ownership and title of all items purchased hereunder shall pass directly
from the selling party to the County, and Jacobs shall at no time be a party to such
transaction other than as agent of the County. The County shall have the unilateral right
to have the commitment authority of Jacobs, its employee or this limited agency LO

* Government Agency cost for all building and engineering perntit fees are not
included in this proposal.
* Construction Administration Services are not included.
* Lightning protection will not be provided.

• Proposal is based on the existing electrical systems having adequate capacity to
properly serve the improvements.No major modifications to the existing
electrical systems are included in this proposal.
• Attendance and/or presentation to public hearings and/or other meetings with

• Municipal Agency and all associated permit fees are to be paid by the client.
Jacobs is not responsible for obtaining any permits.Pe isare to be obtained by
the contractor.Jacobs will monitor the contractor's progress through the permit
process for the client and provide follow up periodically with the permitting
agency to verify the status,but it is ultimately the responsibility of the contractor
to obtain all permits.
• Services do not include"design to cost".Jacobs will assist the owner in

Hurricane.Irma caused extensive flooding and water intrusion damage to the terminal facility,
Monroe County buildings were assessed for mold and the terminal facility had sustained damage 0
due to the storm. Air quality was compromised due to the damage. -19

After the assessment documented the damaged areas the offending material was demolished so
that an acceptable air quality could be provided. The first floor of the terrninal sustained the most
damage and demolition was necessary in;public areas,restroom facilities,ticketing offices,
baggage claim,hold areas,Customs and Border Protection and mechanical support rooms, On
the second-floor demolition in the restrooms was also necessary. V.-

SCOPE OF WORK
Jacobs will provide electrical&structural engineering documents for the hurricane repairs and
replacement to the exterior lighting at the terminal building,the terminal building parking lot,the
rental car parking lot at the terminal and General Aviation FBO ramp at Marathon International
Airport.All lighting design work shall be in accordance with the IES Handbook 9th Edition and
IES RP-37-15 Outdoor Lighting for Airport Environments and shall comply with the National LO
Electrical Code,current edition.
Repairs shall include items noted in the MTH Hurricane Assessment Report Exterior Lighting c
dated February 12,2019 prepared by Jacobs.
Additional design will be completed to install new high mast lighting along the southern edge of

all applicable requirements.

damage and if poles/foundations can be repaired or need to be replaced.

Article E—Bid Phase Services
Upon completion of the construction documents and at the request of the Airport,Jacobs will
prepare the project documents for bidding. Jacobs will perform bidding support services,the
following terms will apply:

deviations from the County's standard terms and conditions to the County for review by
the County's Purchasing Manager or their representative and such deviations shall not be
accepted by Jacobs unless Jacobs receives express written approval thereof from the

• The County shall hold Jacobs and its employees harmless from any claims, suits or
liabilities arising out of any breach or other failure of performance by any contractor,
vendor or supplier under any contract or purchase order issued by Jacobs hereunder.
The following task will be performed as part of the bid phase services:
• Coordinate with the County Purchasing Department and the Airport to advertise for bids.
• Coordinate with the County to provide electronic copies of the bid documents to
prospective bidders
• Prepare for and attend a project pre-bid conference

• Prepare and coordinate the issuing one(1)addendum with the County purchasing

• Revise bid documents as necessary for the addendum

QUALIFICATIONS AND ASSUMPTIONS
For the purpose of providing a stipulated fee amount, this proposal has been based on the
following qualifications and assumptions:

required to bring the project back within budget is not a part of the lump sum fee
and shall be treated as additional services.
• Services not described in the scope of work are not included.
Items identified as not included or limited in the scope of services can be provided or
expanded upon as additional services under a separate agreement or supplemental to this
agreement.

EE&G Environmental Services, LLC (EE&G) is pleased to submit this proposal for the subject 19
work. EE&G's Statement of Services and Fee are presented below: U)
STATEMENT OF SERVICES
EE&G will provide environmental consulting and testing services to conduct limited asbestos
renovation surveys of the interior and exterior areas of these 4 conjoined T-Hangars and stand
alone Antique Hangar as specified by the Client.

Task I —Asbestos Inspection

An EE&G asbestos inspector will conduct a walk-through of the building to visually assess a.
suspect friable and nonfriable asbestos-containing materials (ACM)for condition, homogenous U)
areas,and functional spaces. Upon receipt of notice to proceed, EE&G will collect up to 50 bulk 0
samples total from the renovation areas and transport the samples to our in-house asbestos
laboratory for analysis. The estimate of 50 samples maximum assumes sampling of each
homogenous area (HA) in the subject interiors and exterior areas of the five structures as per
provided plans. E
Task 2-Laboratory Analysis
Bulk samples will be analyzed for asbestos content by polarized light microscopy (PLM) at
EE&G's in-house asbestos laboratory. The laboratory analysis report will be included in the
final asbestos survey report. EE&G anticipates that 50 samples will be adequate to complete
the survey of the building.
As per United States (US) Environmental Protection Agency (EPA) regulations, any liable
materials found to contain less than 10%asbestos by visual estimation, may require analysis by
the objective Point Count Method for confirmation of asbestos content. EE&G recommends
only samples that are less than 1% (1 sample of 3) for this analysis. This analysis may be
performed for an additional $35 per sample at the request of the Client Any unused sample

fees from unanalyzed samples may be applied to any Point Counts required.
Task 3—Asbestos Closeout Package
For each project, EE&G will prepare a final written report containing our findings, and
recommendations, based upon the results of the inspection. Field diagrams and/or photographs
will be provided in the report. The report will be reviewed and signed by a Licensed Asbestos
Consultant. For an additional fee, EE&G can provide additional consulting services for the
removal of identified friable ACM, nonfriable ACM, including bid assistance, contractor selection,
and final air clearance monitoring.
LIMITATIONS AND ASSUMPTIONS'
The following limitations and assumptions apply to this proposal: 17

• EE&G may be required to collect roof and/or exterior samples to satisfy survey
requirements. Although EE&G can temporarily patch sample holes, EE&G assumes no 0
responsibility for voiding of any roof warranties or for the integrity of the roof or
temporary patch. EE&G recommends patching be conducted by a roofing contractor to
maintain any roof warranty in force.

• Some building materials may not be accessible or exposed due to

EE&G's total estimated fee for completion of the proposed work Is not to exceed a maximum
of $4,400.00. This fee estimate is expected to be sufficient to accomplish the Statement of
Services described; however, no guarantee is made or implied.

This minimum fee assumes that EE&G will have complete access to survey the five structures
in one single daily visit. This fee also assumes that each suspect material will be sampled and 0
analyzed, and that a maximum 50 PLM samples will be sufficient to adequately survey the five IL
structures. If more than 50 samples are required to adequately complete the survey, EE&G will U)
collect the additional samples, the client will be notified, and the additional samples will be 0
analyzed by PLM for $10.00 each upon authorization by the client. Point Count analysis as
needed at$35.

General Conditions:
a. Payments for invoices prepared by EE&G are due and payable upon delivery of the final
Asbestos Survey Report. EE&G reserves the right to apply a 1.5% monthly finance charge on
all balances over 30 days outstanding.
b. This Agreement may be terminated by either party hereto upon 15 days notice in writing to the
other party. Upon termination, EE&G shall prepare and submit a final invoice for services
rendered to the date of termination together with any termination expenses incurred.
C. The parties hereto shall maintain in full force and effect comprehensive public liability
insurance with coverage limits which are reasonable in light of the work to be undertaken,and
workmen's compensation insurance as required by law.
d. Any drawings and specifications developed pursuant to this Agreement are instruments of
service, and as such the original documents, tracings, and field notes are and remain the
property of EE&G regardless of whether the work for which they were prepared is executed.

e. In the event that legal action is instituted to enforce any of the terms of this Agreement, the
party which does not prevail shall pay the legal expenses of the prevailing party, including
attorneys fees.
f. The parties hereto each binds itself, its successors, executors, administrators and assigns to
the other party to this Agreement and to the successors, executors, administrators and
assigns of such other party in respect of all covenants of this Agreement.
g. EE&G's liability for services to be rendered under this Agreement shall be limited to$250,000,
unless Client pays for the assumption of additional liability by EE&G as a separate line item in
Article 3,Basis of Compensation.
h. If applicable, Client agrees that EE&G shall not be responsible for liability caused by the
presence or release of hazardous substances at the site. The Client will either make others
responsible for liabilities due to such conditions, or will indemnify and save harmless EE&G LO
from such liability. The provisions of this Article (6,h) shall survive any termination of this
Agreement.
