Proposal No. PN1910258 Dear Ms. Salinger: Thank you for your interest in considering KTA-Tator, Inc. (KTA) for independent third-party shop inspection services. The KTA Steel and Concrete Group is pleased to submit a scope of services and rates for providing structural steel inspection. It is anticipated that KTA's services will be required during fabrication which is tentatively scheduled dates. SCOPE OF SERVICES It is our understanding the work that will be performed for WSP USA, Inc. (WSP) is third-party shop inspection, visual weld inspection, WPS review, and field visual weld inspection. KTA will provide a Certified Welding Inspector(CWI) on a part-time basis for the work at this location. The Inspector will monitor the work associated with the fabrication, welding, blasting and painting, verifying materials, and collecting documentation. The Inspector shall verify, to the extent possible, that fabrication is in compliance with approved shop drawings, call out state/otherwise specific standard specifications, and that the fabricator's QC is effectively controlling the fabrication and coating processes. Discrepancies will be brought to the attention of the fabricator and the Engineer. Coatings will be inspected to verify that the State specific or \ otherwise approved paint system is applied in accordance to the specifications. The KTA inspector will witness NDT and review the results of the testing. The scope of work for part time inspection will be discussed and agreed on prior to commencing. As a rule, care should be taken when judging the quality of a weld based on its visible appearance alone. However, the application of proper visual examination procedures prior to and during welding can increase product reliability as compared to that of products that have only been subjected to final inspection. Part-time inspection will not verify all fabrication and painting operations associated with a project. KTA can also provide professional services to review the fabricator's welding procedures (WPS), if requested. REPORTING PROCEDURES The KTA inspector will write a daily inspection report to document the activities and findings as witnessed during his presence at the shop. Reports will be submitted to the Engineer on a mutually agreeable schedule (usually weekly) after review by the KTA Project Manager. The inspector will document and track material that needs correction. Material requiring rework will not be released for erection until properly repaired. CLIENT OBLIGATIONS The client is to notify the fabricator to provide for proper, safe and unfettered access to the items to be inspected. The client is to provide the contract plans and specifications, approved shop drawings, weld procedures, assembly procedure and repair procedures. The client is to notify the fabricator to provide suitable office accommodations with a phone and wireless or high-speed internet access, and access to a copy machine. ESTIMATE OF PROBABLE COSTS The rates to provide our services are provided below. KTA will invoice WPS only for actual labor and expenses incurred. KTA will not begin work on the project until receipt of a signed KTA Authorization to Proceed using KTA's terms and conditions, or a fully executed contract from WPS with mutually agreeable terms and conditions. KTA has qualified CWI, NDT and ACl/PCI inspectors who are experienced in shop fabrication inspection and field inspection. Our inspectors are certified in accordance with the American Welding Society (AWS) QC-1 for weld inspection, in accordance with SNT-TC-I A for MT, PT, UT and RT (Film Interpretation) and in accordance with American Concrete Institute (ACI) and Precast/Prestressed Concrete Institute (PCI) for concrete inspection.
