BETWEEN MONROE COUNTY AND PEREZ ENGINEERING AND DEVELOPMENT,INC.
FOR ALUMINUM CANOPY AWNINGS AT NEW TRANSFER.STATION OFFICES__ _
In accordance with the Consultant Agreement for Professional Engineering and Surveying
Services,made and entered into on the 21' day of October,2015,between MONROE COUNTY,
hereinafter referred to as the"County,"and PEREZ ENGINEERING AND DEVELOPMENT,
INC., hereinafter referred to as "Consultant," where professional services are allowed for
miscellaneous projects in which construction costs do not exceed One Hundred Thousand and
00/100 Dollars ($100,000.00) or for study activity if the fee for professional services for each
individual study under the contract does not exceed Ten Thousand and 00/1 00 Dollars
($10,000.00); hereinafter referred to as the"Agreement"the parties enter into this Task Order.
All terms and conditions of the Agreement apply to this Task Order, unless this Task Order
amends,adds,or modifies a provision or an Article of the Agreement of which will be specifically
referenced in this Task Order and the amendment, addition, or modification shall be precisely
described. These additions apply only to the project referenced in this Task Order.
This Task Order is effective on the SU l y of j 2019.
WHEREAS, the County demolished and removed the old transfer station offices and replaced
them with new modular office units at the transfer station locations on Card Sound Road in Key
Largo, Overseas Highway in Long Key, and Blimp Road in Cudjoe Key; and
WHEREAS, as the project is nearing completion, County desires to include aluminum canopy
awnings at all three weigh stations for added protection from the elements; and
WHEREAS,Consultant proposes to visit each site and determine conditions to create a complete
set of construction documents for the canopy awnings; and
NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below,
the parties agree as follows:
1. In accordance with Article II, SCOPE OF BASIC SERVICES, of the Agreement, the
Consultant will perform services as described below:
Consultant shall visit each transfer station site to determine the existing conditions and
create a complete set of construction documents for the construction of the aluminum
canopy awnings. The construction documents will include foundation plans, framing
plans, roofing plans, and construction details. The aluminum canopy awnings shall
each have the following characteristics:

a) Aluminum canopy structurally independent from the existing building it serves.
b) Approximately 20' x 13' to cover portions of the stairs and access ramp for the
building it serves.
- c) -Constructed-of aluminum framing and aluminum panel roofing. --
d) Founded on concrete auger piles.
e) Existing footings for stairs and access ramps may need to be modified to allow for
, proper anchorage of canopy columns.
f) No mechanical, electrical,or plumping plans will be included in the plans.
2. In accordance with Article VII,paragraph 7.1.1 of the Agreement,the County shall pay the
Consultant a lump sum fee of Seven Thousand Four Hundred Fifty Dollars and 00/100
($7,450.00)paid on a percent complete basis for the following phases:
• Preliminary Plans for County review $4,000.00
• Completion of Construction Documents $3.450.00
Total $7,450.00
All other Terms and Conditions of the Agreement remain unchanged.
IN WITNESS WHEREOF,each party caused this Task Order to be executed by its duly authorized

Dear Cary Knight:
Perez Engineering & Development, Inc. is pleased to submit this proposal. We understand the
intended work to be the addition of aluminum canopies located at the front of each of the weigh
station buildings listed above (3 total). Our professional engineering services shall consist of the
following:
1. Scope of Services
Perez Engineering & Development Inc. will visit each site to determine existing conditions and
create a complete set of construction documents for the construction listed below. The construction
documents will include foundation plan, framing plan,roofing plan and construction details.
2. Construction Documents—Aluminum Canopy
The following is a brief description of the proposed canopy, that will be specified in each of the
three construction documents:
- Aluminum canopy structurally independent from the existing building it serves
- Approximately 20' x 13' to cover portions of the stairs and access ramp for the building it
serves
- Constructed of aluminum framing and aluminum panel roofing
- Founded on concrete auger piles
• Existing footings for stairs and access ramps may need to be modified to allow for proper
anchorage of canopy columns
• No mechanical, electrical or plumping plans will be included in these plans
Note: Prior to proceeding with design;partial excavation or demolition may need to be done,
allowing Perez Engineering &Development Inc. to observe existing structural members. This
demolition is not included in this service agreement and shall be coordinated by owner.

3. COMPENSATION
Compensation for our services shall be billed as follows:
a. Construction Documents
• Preliminary Plans for Monroe County Review $4,000
• Upon Completion of Construction Documents $3,450
Total Fee $7,450
b. Construction Phase Services
Construction Phase Services are not included in this scope of work. However, Perez Engineering
and Development Inc. can provide these services, under a separate agreement, at the request of
Monroe County Public Works and Engineering. These services include items such as
construction coordination, construction monitoring, submittal review and plan revisions.
