GENERAL INFORMATION The EYW Airport Master Plan identified a long-term development plan for Faraldo Circle to accommodate the projected 2035 passenger traffic forecasts and requirements. This plan has identified several requirements in regard to taxi stands, public bus routes, and private and commercial vehicle curbside footage required to meet the forecasted demand. This scope is consistent with the objectives outline in Section 5.3.1 of the November 2017 "Key West International Airport Master Plan Update [draft]." The project limits, as ,shown below, will include the re-alignment of Faraldo Circle and modifications to the existing curbside to accommodate the Master Plan recommendations,. Improvements include realignment of the arrivals curb from the existing landscape berm (located east of the terminal) to the commercial curb staging area located west of the terminal. For this construction package, Jacobs will provide the following services: 1. Management of the project from data collection, bid opening,award of the construction contract,construction administration and inspection services. 2. Coordination with the geotechnical subconsultant and survey subconsultant. 3. Coordination with EYW,FAA/FDOT as required. 4. Preparation of minutes of meetings and phone conversations. 5. Program verification and site investigations. 6. Preparation of the constructions documents and incorporation of Monroe County's front- end contract documents. 7. Preparation of the engineering report,construction cost estimate,and construction schedule. 8. Bidding and award assistance. 9. Construction administration services. 10. Construction inspection services. Assumptions/Design Parameters 1. This project is eligible for AIP funding. 2. Jacobs will develop plans,technical specifications,an engineering report, and a construction cost estimate at the 60% and 100%design level for review. Jacobs will develop the 7460 to be submitted to the Airport and FAA for review. 3. Jacobs will produce a final contract set of plans,specifications,engineering report, construction schedule,and construction cost estimate for bidding purposes. 4. DEP and ACOE permits are not required based on the current scope of the project. 5. This project will be designed according to Monroe County design standards and FDOT guidelines,as applicable.Construction drawings will be prepared in AutoCAD 2016 format. 6. It is assumed that this project will not require a construction management plan. 7. Landscape Design is not anticipated for this project. Minor changes to landscape berms will be shown on the demolition and or grading plans.No irrigation or planting of new vegetation other than sod is anticipated. Minor irrigation relocation may need to be made, and will be addressed during construction with field staff and design concurrence with the Engineer of Record. 8. Relocation of light poles will move a very small distance; a photometric study is not anticipated or included. 9. Construction phase services are based on an anticipated duration of 120 calendar days of construction in field. In the event the construction duration exceeds the calendar days listed above,Jacobs reserves the right to revise our proposal. Page 2 J/�COBS, 10. Jacobs Project Management Company will invoice on a percent complete, including expenses,to compensate the level of effort required for each assignment. 11. All as-built drawings to be provided to Jacobs in electronic (AutoCad) format and hard copy (printed)format. 12. For the SFWMD permit it assumed that this project will require a modification only and will not require any modification to the existing drainage system. 13. Any cost estimates provided by Jacobs will be in a basis of experience and judgement. Since consultant has no control over market conditions or bidding procedures,Jacobs does not warrant that bids or ultimate construction costs will not vary from these cost estimates. ARTICLE A—DATA COLLECTION AND PRELIMINARY DESIGN Data Collection: 1. Prepare for and attend project design kick off meeting. 2. Coordinate with subconsultants including but not limited to survey and geotechnical as well as other Project Team members. 3. Compile and review existing available information including as-built drawings,geotechnical reports, survey data,and maintenance logs. Faraldo Circle was rehabilitated approximately five years ago. This project will utilize the geotechnical information that was performed from the previous project.The as-built survey performed from the previous project will be utilized for this project. It is assumed additional survey will be needed that will require for one survey crew on site for three days. 4. Perform a detailed site inspection and evaluate conflicts to the proposed realignment. 5. Identify any additional information that may be required from field investigations or other agencies and prepare a Design Memorandum memorializing the findings. Services for Article A will be provided under a lump sum basis. ARTICLE B—DESIGN Based on the information collected under Article A,the 60%, 100%,and final contract documents for the construction package will be developed for all elements identified in this scope of work. 1. Develop technical specifications utilizing FDOT's standard specifications for construction. Submit complete technical specifications at 60%, 100%and Final. 2. Coordinate and incorporate Monroe County provided contract document/front-end specifications into the final contract bid documents. (i.e. Notice to Bidders, Invitation to Bidders, Instruction to Bidders, Bid Proposal, Contract, Contract Articles, General Specifications). 3. Develop a construction schedule for use in determining phasing and duration. Page 3 J/�COBS 4. Prepare for and attend two(2) design review meetings (60%, and 100%) with the Airport to discuss project progress and review the design submittals. 5. Develop detailed construction quantities and cost estimate at the 60%, 100%, and at final contract document design levels. 6. Develop the following anticipated plans: Title Sheet(1) Index of Drawings and Notes(1) Summary of Quantities(1) General Plan and Survey Control Plan(1) Existing Conditions Plan(4) MOT Plan(2) Demolition Plan (4) Erosion Control Plan(4) Erosion Control Details(2) Geometry Plan(4) Typical Sections(1) Grading Plans(4) Cross Sections (4) Pavement Marking Plan(4) Pavement Marking Details(2) Utility Relocation Plans(2) Utility Relocation Details(2) Light Pole relocation and Details (2) Miscellaneous Plans/Details(2) Total: 48 Sheets 7. Evaluate potential bid alternates and alternatives for the construction based on available funding sources. 8. Prepare and submit SFWMD permit. 9. Develop Engineer's report. 10. Coordinate, distribute and print the following copies of the preliminary construction documents for the construction package (plans, specifications, design report, and construction cost estimate)for review by the Airport,FAA, and FDOT: Preliminary Submission (60%. 100%. Final Contract Documents for each construction package) FAA: 1 half size copies of plans, 1 hard copy of the specifications, engineer's report and estimate, and a CD containing the drawings, specifications, engineering report,and engineer's estimate for the 90%submission. FDOT: 2 half size copies of plans; and a CD containing the drawings, specifications, engineering report,and engineer's estimate for the 90%submission only. Page 4 J/�COBS Airport: 2 copies of plans(1 full size; 1 half size);2 copies of specifications;2 copies of engineering report;and 2 copies of engineer's estimate for each submission 11. Conduct an in-house QA/QC prior to each submission. The in-house QA/QC check will involve an experienced independent individual, depending on each discipline, to conduct a comprehensive check on all documents to be submitted (e.g. plans, specifications, estimates,reports). Time will be required for engineers and CADD technicians/designers to correct items listed under the in-house QAIQC checks. 12. Consultant to provide input into pavement marking installation and marking removal specifications and techniques(Sightline). 13. Address comments from 60%, and 100%design submissions. 14. Submit 7460. 15. Prepare and submit permits for Monroe County Minor Deviation and Building Permits. Services for Article B will be provided under a lump sum basis. Article C—Bid Phase Services The following task will be performed as part of the bid phase services: • Coordinate with the County Purchasing Department and the Airport to advertise for bids. • Coordinate with the County to provide electronic copies of the bid documents to prospective bidders • Prepare for and attend a project pre-bid conference • Prepare responses to address requests for information from prospective bidders • Prepare and coordinate the issuing one(1)addendum with the County purchasing department. • Revise bid documents as necessary for the addendum • Review bids for responsiveness • Prepare a bid tabulation • Make a recommendation for award based on review of the bid information submitted. • Prepare Conformed Contracts Services for Article C will be provided under a lump sum basis. ARTICLE D—CONSTRUCTION ADMINISTRATION SERVICES 1. Prepare and issue construction contract to the awarded low bidder. 2. Issue a"Notice to Proceed,"on behalf of the Airport,to the Contractor. 3. Review the Contractor's construction schedule. 4. Prepare for and attend the Pre-Construction conference. Page 5 J,/�COBS 5. Review all shop drawing submittals for conformance to the plans and specifications and respond to the CONTRACTOR and Airport with detailed summary of the applicability and acceptability of the shop drawing. 6. Respond to Contractor's RFI's. 7. Attend site visits and bi-weekly progress meetings. Observe work in progress and provide reports to Owner. 8. Prepare Field Sketches,as necessary. 9. Provide general supervision and administrative support for resident project representative (RPR). 10. Review and approve estimates submitted by the Contractor for progress/final payments. 11. Prepare and negotiate change orders,as necessary. Assume two change orders. 12. Attend final inspection meeting. 13. Review Closeout Documentation. 14. Review quality assurance testing results. 15. Prepare record drawings and provide hardcopies and electronic files to the Airport. 16. Prepare and submit FAA closeout documentation and prepare and process final change order. Services for Article D will be provided under a lump sum basis. ARTICLE E-RESIDENT PROJECT REPRESENTATIVE(RPR) 1. Attend pre-construction conference. 2. Coordinate and attend progress meetings with the contractor,Airport,and any other interested parties,as required prior to construction(e.g.Staging area coordination,setting up Contractor's trailer.). 3. Provide full-time RPR services for the work associated with the project. The RPR's for the project shall have field experience in the type of work to be performed,be fully qualified to make interpretations,decisions,field computations,and have knowledge of testing requirements and procedures. The RPR's shall be responsible for the following: • Checking of construction activities to ensure compliance with the plans and specifications. Inform the contractor and County of any work,which is in non- compliance. • Coordinate quality assurance tests and verify they have been performed at the frequency stated in the specifications. • Review certifications for conformance with the specifications. Page 6 J/�COBS • Document quantities of materials used on the project by actual measurements and computations in a field notebook or computer printout retained in a folder. • Maintain a set of working drawings on the job site,which can be used to prepare"As- Built"drawings. • Review the test reports and certifications for conformance with the specifications and maintain a file for all reports and certifications. • Inform the County of deficiencies so convections can be made and retesting performed. • Jacobs does not have control over or charge of and shall not be responsible for construction means,methods,techniques,sequences or procedures of construction or for safety precautions and programs in connection with the work,since these are solely the CONTRACTOR'S responsibility under the contract for construction. • Jacobs shall make every reasonable effort to ensure that the CONTRACTOR completes the work in accordance with the current approved schedule and carries out the work in accordance with the contract documents. • Review close out documentation. • Review payment requests and certified payrolls from the contractor. • Maintain a diary, which will contain entries made and signed by the resident engineer. Each entry should include the following,plus any additional pertinent data: a. Date and weather conditions. b. Names of important visitors. C. Construction work in progress and location. d. Size of contractor's work force and equipment in use. e. Number of hours worked per day for contractor and subcontractors. 4. Quality Assurance Testing of Construction Materials will be provided by our subconsultant, Tierra South Florida.Quality Assurance for pavement markings will be provided by Sightline. 5. Attend final inspection and create punch list. 6. Attend follow up site visit to confirm completion of punch list items. Services for Article E will be provided under a lump sum basis. (AID) will provide Professional Services to JACOBS for the design, bidding, and construction administration phases of the Realignment and Curbside Improvements of Faraldo Circle at Key West International Airport (EYW): General Information The EYW Airport Master Plan identified a long-term development plan for Faraldo Circle to accommodate the projected 2035 passenger traffic forecasts and requirements. This plan has identified several requirements in regard to taxi stands, public bus routes, and private and commercial vehicle curbside footage required to meet the forecasted demand. This scope is consistent with the objectives outline in Section 5.3.1 of the November 2017 "Key West International Airport Master Plan Update [draft]." The project limits, as shown below, will include the re-alignment of Faraldo Circle and modifications to the existing curbside to accommodate the Master Plan recommendations, Improvements include realignment of the arrivals curb from the existing landscape berm (located east of the terminal) to the commercial curb staging area located west of the terminal. The realigned terminal loop will provide two full lanes for through traffic and one additional lane of commercial, taxi, public bus, and private vehicle (ride-share) of various sizes and locations along the curbside. Construction drawings will be prepared in AutoCAD 2016 format. General Scope The Project Team will provide the following general services for this project: x Review and compile existing Survey and Geotechnical data. k Coordination with JACOBS and EYW, as-required. x Preparation of minutes of meetings. x Program verification and site investigation. x Design and preparation of the bid form and technical construction specifications. h Preparation of the Engineer's Report and construction cost estimate. h Bidding and award assistance. x Construction Administration. Specific Scope The Project Team will prepare construction documents for the realignment of the existing bituminous concrete pavement and curbside along Faraldo Circle. The project will be advertised as a unit price base bid contract. Specific tasks related to the above items are identified in each phase of the project as described below. Phase 1 - Data Collection AID will perform the following tasks under this phase: x Compile and review existing available information including as-built drawings, geotechnical reports, survey data, and maintenance logs. k Update existing survey and geometric base files with as-built survey and elevational information. x Prepare for and attend a Pre-Design meeting with the Airport and JACOBS. x Perform a detailed site inspection and evaluate conflicts to the proposed realignment. x Coordinate with JACOBS and EYW on project issues such as design alternatives, project phasing, construction staging, budget, and schedule. A draft copy of technical specifications and front-end documents will also be prepared. Phase 3- Contract Documents (100% Design) Upon receipt of final comments from JACOBS and EYW, AID will proceed with the preparation of the 100% contract documents. This effort includes incorporating comments by updating the construction drawings, project manual (front-end documents, general provisions, and technical specifications), Engineer's Report, construction cost estimate, and construction schedule. AID will submit electronic copies of the 100% contract documents to JACOBS for distribution during the bidding phase. Phase 4—Bidding and Award Services This phase will include the effort necessary for the Owner to advertise for and receive bids from contractors for the Project. AID will review the bids and make a recommendation of award to the Owner and the FAA. Coordinate with the Owner, Tenants, and project team members throughout the duration of procurement, construction and closeout of the project. 2. Participate in a preconstruction conference. 3. Review and approve or take other appropriate action upon review of the shop drawings, samples, and other submissions furnished by the Contractor and submitted to AID. AID shall determine if the shop drawings, samples and other submissions reasonably conform to the design of the project and the requirements of the contract documents. Such action(s) shall be taken with reasonable promptness so that the progress of the construction of the project will not be delayed. AID will maintain a log of all Contractor submittals, which shall include the submittal date, the action taken, and the date returned to the Contractor. 4. Prepare for and Participate in bi-weekly construction meetings (as necessary) via teleconference. JACOBS will prepare the meeting Agenda and record meeting minutes for each meeting. 5. Conduct site visits at times appropriated during the construction and inform the Owner of any deviation from the contract documents or the contractor's construction schedule observed by or brought to the attention of AID. Based on its on-site observations, AID will keep the Owner informed as to the progress and quality of the work and will endeavor to protect the Owner against defects and deficiencies in the work completed by the contractor. However, AID shall not have control or be in charge of and shall not be responsible for construction means, methods, techniques, sequences or procedures or for the safety precautions and programs in connection with the project construction, for the acts or omissions of the Contractor, Subcontractors, any of their agents or the Subcontractor's employees, or any other person performing any of the work, or for the failure of such persons to carry out the work in accordance with the contract documents. Further, AID will notify the Owner of any such act, omission, or failure on the part of the Contractor observed by AID during on-site visits. 6. Assist with preparing routine change orders and assess the justification for such change orders, as required. 7. Review test results provided by the Contractor and the Owner. Page 5 of 7 November 2,2017 EXHIBIT A SCOPE OF WORK 8. Provide design clarifications and recommendations to assist the Owner in resolving field problems relating to construction. AID will receive, review and respond to all Contractor Requests for Information (RFI's) and issue clarification as necessary through Engineer's Supplemental Instructions (ESI's) 9. Perform, together with the Owner, one (1) inspection of the construction site to determine if the project is substantially complete, and a final inspection to determine if the project has been completed in accordance with the contract documents, and if each contractor has fulfilled all of its obligations thereunder, so that AID may approve in writing, if applicable, final payment to the Contractor. To avoid misunderstandings or questions, AID understands and agrees that the Owner shall have responsibility for the general administration of the construction contract. Accordingly, AID shall not have the authority or responsibility to issue direct instructions to the Contractor on work stoppage authorizations that are contractually obligated to the Owner, or to require special inspections and/or tests. However, AID shall provide continuing counsel to the Owner throughout the construction of the project. Special Services In addition to the Basic Services described above, AID will provide the following special services required under this contract: 1. Monroe County Permitting —AID will submit a pre-permit application to the City of Key West and Monroe County to aid the contractor in obtaining a construction permit at the time of award. 2. An Obstruction Evaluation/Airport Airspace Analysis (OE/AAA) for the relocation of existing light poles, bus stop canopies, and public art will be filed for the location of the permanent structure elevations of each object. AID will file a temporary airspace case for the construction equipment anticipated to be used. 3. Record Drawings — AID will provide Signed and Sealed record drawings for the project upon receipt of contractor's red-lined drawings and As-Built survey. Exclusions 1. Landscape Design is not anticipated for this project. Minor changes to landscape berms will be shown on the demolition and or grading plans. No irrigation or planting of new vegetation other than sod is anticipated. Minor irrigation relocation may need to be made, and will be addressed during construction with field staff and design concurrence with the Engineer of Record. 2. Physical Relocation of light poles will be designed by AID showing an (x,y) coordinate for each location.
