TASK ORDER FOR PROFESSIONAL ENGINEERING SERVICES BETWEEN MONROE COUNTY AND PEREZ ENGINEERING AND DEVELOPMENT,INC. FOR GENERAL STRUCTURAL ASSESSMENT OF 2855 OVERSEAS HIGHWAY,MARATHON, FL 33050 In accordance with the Agreement for Continuing Professional Engineering and Surveying Services,made and entered into on October 21,2015,as first amended on April 19,2018,as second amended on October 17, 2018 and as third amended on August 21, 2019, between MONROE COUNTY, hereinafter referred to as the "County," and PEREZ ENGINEERING AND DEVELOPMENT,INC.hereinafter referred to as "Consultant,"where professional services are allowed if construction costs do not exceed One Hundred Thousand and 00/100 Dollars ($100,000.00), or for study activity if the fee for professional services for each individual study under the contract does not exceed Ten Thousand and 00/100 Dollars ($10,000.00), hereinafter referred to as the "Agreement". All terms and conditions of the Agreement apply to this Task Order, unless the Task Order modifies a provision or an Article of the Agreement of which will be specifically referenced in the Task Order and the modification shall be precisely described. This Task Order is effective October 18, 2019. WHEREAS,the County is considering acquiring a property and desires the Consultant to provide a structural assessment of the building located at 2855 Overseas Highway, Marathon, FL 33050 prior to signing a purchase agreement. NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below, the parties hereby agree as follows: 1. In accordance with Article II, SCOPE OF BASIC SERVICES of the Agreement, the Consultant will complete a general structural assessment of the building(per attached copy of the 4 October 2019 proposal). The scope of work includes: a. Field Assessment including: 1) Review of the building plans provided by Monroe County, if available; 2) Visual inspection of structure and structural elements; 3) Note: Destructive inspecting/testing is excluded from this scope of work. b. Structural report including: 1) Damage observed; 2) Probable causes of damage observed; 3) Recommendations to owner (e.g. repair, rehabilitation, or further investigation if necessary Page 1 of 2 • 4) NOTE:These services are intended to encompass the structural members and their finishes (i.e. wall and roof coverings) only. These services exclude mechanical, electrical,and plumbing aspects of the buildings. 2. In accordance with Article VII,paragraph 7.1.1 of the Agreement,the County shall pay the Consultant a lump sum fee of Four Thousand Seven Hundred and 00/100 Dollars ($4,700.00)paid on a percent complete basis for the following phases: a. Completion of a Field Assessment - $2,000.00 upon completion and approval by the Director of Project Management. b. Delivery of Structural Report-$2,700.00 upon document review and approval by the Director of Project Management. All other terms and conditions of the Agreement remain unchanged and in full force and effect. IN WITNESS WHEREOF, each party caused this Task Order to be executed by its duly authorized representative. Execution by the Consultant must be by a person with authority to bind the entity. We understand that Monroe County is considering acquiring the property. This assessment is intended to determine the general condition of the structure, as well as determining if detailed inspection/assessment and repair cost estimates are needed. At the conclusion of this general structural assessment, Perez Engineering and Development, Inc can continue with a detailed assessment; at the request of Monroe County. This detailed assessment will be executed under a separate agreement and can include the following, as needed: - Reinspection after partial demolition o Demolition shall be the responsibility of the owner - Coordination with contractors to complete a cost estimate of required repairs - GPR scanning of existing reinforced concrete, if recommended The following is a summary of the services to be provided during the General Structural Assessment: 1.
