PROFESSIONAL ARCHITECTURAL AND ENGINEERING SERVICES
BETWEEN MONROE COUNTY AND
WILLIAM P.HORN,ARCHITECT
FOR
JACKSON SQUARE LIGHT POLE REPAIR ASSESSMENT
In accordance with the Consultant Agreement for Professional Architectural and Engineering
Services,made and entered into on December 19,2018 between MONROE COUNTY,
hereinafter referred to as the"County,"and WILLIAM P. HORN,ARCHITECT,hereinafter
referred to as"Consultant,"where professional services are allowed if construction costs do not
exceed Two Million and 00/100 Dollars($2,000,000.00),or for study activity if the fee for
professional services for each individual study under the contract does not exceed Two Hundred
Thousand and 001100 Dollars($200,000.00),hereinafter referred to as the"Agreement".
All terms and conditions of the Agreement apply to this Task Order, unless this Task Order
amends,adds,or modifies a provision or an Article of the Agreement of which will be specifically
referenced in this Task Order and the amendment, addition, or modification shall be precisely
described. These additions apply only to the project referenced in this Task Order.
This Task Order is effective on the 19th of November,2019.
As a result of Hurricane Irma damages to several light poles and fixtures on the Historic Jackson
Square site,the County desires an assessment for replacement, in-kind of three light fixtures and
two poles.
The Consultant is qualified to conduct an assessment of the missing lighting fixtures and report
the findings,provide recommended fixes and estimated cost and duration of those fixes.
NOW,THEREFORE,in consideration of the mutual promises and covenants set forth below,the
parties agree as follows:
1. In accordance with Article 11, SCOPE OF BASIC SERVICES of the Agreement the
Consultant will perform services as described below:
a. Consultant shall conduct an on-site evaluation to assess the available electrical
infrastructure and investigate the lighting reparations, including inspection of the
poles and fixtures.
b. Consultant shall provide written documentation of the findings and recommended
fixes to replace damaged lighting,paying strict attention to electrical lighting codes,
(Including the Florida Dark Sky movement,to reduce light pollution to the area and

neighboring homes) and specify an estimated cost and duration of the work to
complete.
c. Consultant will provide the Following deliverables:
i. Design and specifications to install new lighting poles and fixtures, to
include an electrical plan set;
Bidding assistance and Construction support not include in this Task Order.
2. In accordance with Article VII,paragraph 7.1.1 of the Agreement,the County shall pay The
Consultant a lump sum fee ofTwo Thousand tour Hundred and 00/100 Dollars($2,400.00)
paid upon document review and approval by the Director of Project Management.
All other Terms and Conditions of the Task Order shall remain in accordance with the Agreement.
IN WITNESS WHEREOF,each party caused this Task Order to be executed by its duly authorized
representative.

STOCK ISLAND. FLORIDA
THE ARCHITCTURAL & ENGINEERING PEE FOR THIS PROJECT HAS BEEN ESTIMATED
AS FOLLOWS:
SCOPE OF WORK INCLUDES REPLACING THREE LIGHT FIXTURES AND 2 (POSSIBLY 3)
LIGHT POLES TO MATCH THE EXISTING LIGHTING ON THE SITE. THE NEW LIGHT POLES
AND FIXTURES WILL. MATCH THE EXISTING POLES AND FIXTURES ON THE SITE BUT
WILL NESD TO MEET NEW CODE REQUIREMENTS. WORK INCLUDES GETTING HARC
APPROVAL AND PRODUCING PLANS AND SPECIFICATIONS FOR PERMIT AND
CONSTRUCTION.
THE PEE WILL BE AS FOLLOWS:
ARCHITECTS WORK INCLUDES COORDINATION WITH THE COUNTY STAFF. HARC
STAFF AND THE ELECTRICAL ENGINEER AND PRODUCING THE EXISTING SITE PLAN.
FEE=: $700.00
ELECTRICAL ENGINEERS CONSTRUCTION DOCUMENTS AND SPECIFICATIONS:
FEE= $1,700.00
THE TOTAL FEE IS S2.400.00.
THE BID AND CONSTRUCTION PHASE ARE NOT INCLUDED IN THE SCOPE OF WORK.
IF WORK IS NEEDED IN THESE PHASES IT CAN BE COMPLETED AT OUR STANDARD
HOURLY RATES AS PER OUR ANNUAL CONTRACT.
THE COUNTY WILL PROVIDE THE ARCHITECT A SITE PLAN BASE.
