Marathon Tax Collector 12/17/2019 P TASK ORDER FOR PROFESSIONAL ENGINEERING AND SURVEYING SERVICES FOR THE MARATHON TAX COLLECTOR BUILDING REPAIR In accordance with the Continuing Contract for Professional Engineering and Surveying Services, made and entered on October 21,2015,as first amended an April 19,2018,as second amended on October 17, 2018 and as third amended on August 21, 2019, between MONROE COUNTY, hereinafter referred to as the "County" and PEREZ ENGINEERING & DEVELOPMENT, INC., hereinafter referred to as "Consultant", or "Contractor" where professional services are allowed for miscellaneous projects in which construction costs do not exceed One Hundred Thousand 00/100 Dollars ($100,000.00.) or for study activity if the fee for professional services for each individual study under the contract does not exceed Ten Thousand and 00/100 Dollars ($10,000.00); hereinafter referred to as the"Agreement"the parties enter into this Task Order. This Task Order is effective on the I fir' day of Decapllacr ,2019. WHEREAS,the Marathon Tax Collector Building was damaged during Hurricane Irma and is in need of repairs,and WHEREAS, the Consultant is qualified and able to perform engineering design services for the repairs to the Marathon Tax Collector Building. NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below, the parties agree as follows: 1. In accordance with Article II, SCOPE OF BASIC SERVICES, of the Agreement, the Consultant will perform services as described below: Consultant shall visit the Marathon Tax Collector Building site to determine the existing conditions and shall create a complete set of construction documents for the repairs. The construction documents will include partial floor plan,repair plan,and repair details. Proposed repairs to include: • Settling slab in"vault room." • Settling slab adjacent to vault room. • Damaged concrete tie beams and columns. No mechanical, electrical,or plumbing plans will be included. Note: Prior to proceeding with design, partial excavation or demolition may need to be done to allow for observation of existing structural members. This demolition is not included in this Task Order. Page 1 of 2 l 2. In accordance with Article VII,Paragraph 7.1.1, COMPENSATION,the County shall pay the Consultant monthly in current funds for the performance of this Task Order as an agreed upon lump stun fee as follows: • Preliminary Plans for Monroe County Review S3,000.00 • Upon Completion of Construction Documents 14,050.00 Total Fee $7,050.00 3. All other terms and conditions of the Agreement remain in full force and effect. IN WITNESS WHEREOF, each party caused this Task Order to be executed by its duly authorized representative.
