AND CARDNO,INC.FOR JOE LONDON FIRE TRAINING ACADEMY
In accordance with the Agreement for Professional Architectural and Engineering Services,made
and entered into on December 19, 2018 between MONROE COUNTY, ("County') and
CARDNO, Inc,("Consultant"),where professional services are allowed if construction costs do
not exceed $2,000,000.00, or for study activity if the fee for professional services for each
individual study under the contract does not exceed$200,000.00("Agreement"),the parties enter
into this Task Order.
All terins and conditions of the Agreement apply to this Task Order, unless this Task Order
amends, adds, or modifies a provision or an Article of the Agreement in which case the Article
will be specifically referenced in this Task Order and the amendment, addition, or modification
shall be precisely described. These additions apply only to the project referenced in. this Task
Order.
This Task,Order is efTective on the 221 of January 2020.
WHEREAS,the Florida Department of Environmental Protection Site Investigation Section
collected samples of ground water, surface water,sediment and soil to test for perfluorooctanoic
acid(PFOA)and perfluorooctane sulfonate (PFOS)at the Monroe County—Joe London Fire
Training Academy(Facility)on September 26,2019; and
WFIEREAS, the Florida Department of Environmental Protection ('FDEP") notified Monroe
County Fire Rescue of high concentrations of said contaminants in a letter dated November 8,
2019;and
WFIEREAS, the County is required under Rule 62-780.600, Florida Administrative Code, to
conduct a site assessment at the Facility;and
WHEREA S,the Consultant is qualified to provide professional consulting services for the needed
assessment.
NOW,THEREFORE, in consideration.of the mutual promises and covenants set forth below,the
parties agree as follows:
1. In accordance with Article 11, SCOPE OF BASIC SERVICES of the Agreement,the
Consultant will prepare a draft assessment plan for FDEP review,conduct soil and
groundwater assessments and.prepare a Site Assessment Report(SAR)in accordance
with Tasks 1-4 shown on Cardno's Technical and Cost Proposal,Attachment"A"to this
Task Order, incorporated herein by ref-lerence.
2. In accordance with Article VII paragraph 7.1.1 of the Agreement,the County shall pay
the Consultant a Not-to-Exceed(NTE)total of Seventy-Nine Thousand Nine Hundred
00/100 dollars($79,900.00)paid on a percent complete basis for the following phases:

Background
Cardno understands that FDEP representatives conducted a preliminary investigation of
the referenced facility in Monroe County, Florida, and that perfluorooctanoic acid (PFOA)
and perfluorooctane sulfonate (PFOS) were detected in soil and groundwater above the
current Provisional Cleanup Target Levels (CTLs) and U.S. EPA Health Advisory Levels
(HALs). This includes residential direct exposure (RDE) soil cleanup target levels
(SCTLs) for PFOS at one sample location, and leachability SCTL exceedances for PFOS
and/or PFOA at numerous locations. The groundwater CTL of 70 nanograms per Liter
(ng/L) for PFOS and PFOA (either individually or combined) was also exceeded in each
of the two groundwater samples collected. Preliminary surface water standards were
also exceeded during this investigation. Based on these results, the FDEP submitted a
letter to Chief James Callahan (Monroe County Fire Rescue) dated November 8, 2019,
requiring that the responsible party initiate site assessment activities within 60 days of
discovery. This letter also indicated that, as the potentially responsible party for this site,
Monroe County/Monroe County Fire Rescue is subject to the requirements for
assessment and remediation of contamination under Chapter 62-780, F.A.C.
As requested by Monroe County, Cardno has developed this proposed scope of work to
expand the previous study, with the intent of delineating documented impacts to both soil
and groundwater, and to collect sufficient information from which to evaluate potential
remedial measures, if required. The proposed scope of work has been developed based
on Cardno's recent experience with similar sites, and the distribution and concentrations of
PFAS documented within the FDEP study. However, the proposed scope of work can be
modified pending review by Monroe County representatives. Additionally, the County may

want to present the proposed scope of work for FDEP review and consideration prior to
implementation.
This proposal is provided as a cost plus multiplier (CPM) not-to-exceed (NTE) proposal that can
be modified, if required, based on the evaluation of additional information provided by FDEP.
However, the proposal can be modified to a lump sum (LS) cost basis, if preferred by the
County. Activities performed by Cardno for this project will be consistent with our current
Agreement with Monroe County to Furnish Professional Consulting Services (executed
December 19, 2018).
Proposed Scope of Work
Task 1 —Prepare Draft Assessment Plan for FDEP Review —Obtain Access Agreements -
Geophysics
Cardno will meet with Fire Training Center representatives to discuss historical training activities
with respect to PFAS use at the site, and will review information provided by FDEP regarding
PFAS impacts to soil, groundwater, sediment, and surface water; groundwater and soil
characteristics; well construction details, etc.; in order to refine the proposed assessment plan for
the site. Cardno assumes that a meeting with FDEP will be conducted to review the proposed
scope of work, and to discuss FDEP's long range expectations regarding offsite delineation and
remediation requirements. This task will also include initial project administration and
subcontracting services, as well as preliminary clearing of proposed drill locations via a
geophysical survey. Geophysical clearing of drill locations may be omitted if detailed records of
underground utility locations are available.
Task 2 —Soil Assessment
FDEP Consultants collected shallow soil samples from eleven (11) soil borings, with leachability
SCTLs exceeded in at least one sample interval at all eleven locations. However, the residential
direct exposure (RDE) SCTL was only exceeded at one location. Based on these results, Cardno
proposes to advance shallow soil borings at the four (4) locations shown on Figures 1 and 2, and
to collect samples from each location for PFAS analysis. The primary purpose of these samples
is to refine the delineation of leachability SCTL exceedances to the southwest (one boring), and
(more importantly) to delineate the RDE SCTL exceedance near the northwest corner of the site.
Soil samples will be collected from two depth intervals (0 to 0.5 ft below land surface (bls) and
from between 0.5 ft bls and the water table), for analysis of PFAS compounds. A total of eight (8)
soil samples will be analyzed for PFAS, plus one equipment blank, one field blank, and
(potentially) one matrix spike/matrix spike duplicate sample (11 soil samples total).
Task 3 —Groundwater Assessment
FDEP installed two temporary monitor wells to a depth of 12 ft bls during the initial site
investigation. The Provisional GCTL for both PFOS and PFOA (70 ng/L) were exceeded at both
well locations, with combined PFOS and PFOA concentrations of 1,540 ng/L and 17,200 ng/L,
for wells TMW-1 and TMW-2, respectively. These elevated concentrations, coupled with the
high solubility (miscibility) of PFAS compounds in groundwater indicates that a significant PFAS
plume may be present at this site. As a result, Cardno is proposing installation of nine (9)
permanent monitor wells to a depth of approximately 12 ft bls, as well as two deeper monitor
wells (to approximately 30 ft bls), at locations shown on Figure 3. One of the nine shallow wells
is proposed on adjacent, undeveloped property to the west, and to additional shallow wells are

proposed outside of the facility fence to the south and southeast. Site access agreements or
right-of-way permits may be required for these three well locations.
Two deeper monitor wells are designed to provide evaluation of the vertical extent of PFAS
impacts to groundwater, near the locations of the previous temporary shallow wells. In order to
prevent potential drag-down of contamination during drilling activitieis, a temporary surface
casing will be used to isolate the shallow borehole from the deeper borehole during
construction. Based on the number of wells proposed, the proposed depth of deeper wells at
the site, and the extensive limestone Iithology present, Cardno is proposing the use of Sonic
drilling techniques to minimize potential issues with borehole advancement, and to speed up the
drilling and well installation processes. Additionally, due to the exceedingly low level GCTLs
associated with PFAS, high level decontamination procedures will be implemented to minimize
the potential for cross contamination between boring or sample locations.
Once installed and developed, each of the eleven (11) new monitor wells, will be sampled for
PFAS constituents via method 537(modified) (includes 21 analytes with PFOS and PFOA).
Cardno will also conduct chloride analysis on select wells to determine if alternative GCTLs
might be applicable to the site. In addition to the 11 well samples, two (2) equipment blanks,
two (2) field blanks, two (2) sample duplicates, one (1) trip blank, one (1) matrix spike/matrix
spike duplicate sample (potentially), and two (2) investigation-derived waste samples will be
analyzed for PFAS constituents (total of twenty-one samples). Samples will be collected in
compliance with the FDEP Standard Operating Procedures Manual (DEP-SOP-001/01), and
Draft SOPs and Draft PFAS Sampling SOPs, dated October 2019.
After wells are installed and developed, the top of casing elevation of all monitor wells will be
surveyed relative to an arbitrary datum to allow determination of groundwater flow at the site.
Task 4— Data Evaluation and Report Preparation (Site Assessment Report (SARI)
The information obtained during implementation of Tasks 1 through 3 will be evaluated and
discussed within a SAR document. Sample results will be summarized in tables and on figures,
and associated Iithologic logs, groundwater sampling logs, and laboratory analytical reports will
be included within the document. Conclusions and recommendations will also be provided in
the report, including a preliminary recommendation regarding the need for additional sampling
or potential remedial measures.
Cost Proposal
This cost proposal assumes two days total for mobilization and demobilization to and from the site
(Drilling Contractors and Consultants), five days of borings and well installations and development,
and two additional days to complete subsequent groundwater sampling.

Cost Proposal Summary
A total cost proposal of$77,800 is proposed for the tasks outlined above. Utility clearing via
geophysics is included in this proposal because hand clearing locations will not be possible due to
the shallow subsurface Iithology. Additionally, the proposed scope of services is designed to
minimize potential for subsequent mobilization charges to refine lateral or vertical delineation of
documented impacts, and to provide sufficient information from which to initiate preliminary
remedial measures evaluations.
Cardno will initiate the scope of work immediately after receipt of written authorization to proceed from
Monroe County.

Dear Fire Chief Callahan:
As you are probably aware,the Florida Department of Environmental Protection(Department)has been
engaged in investigations related to the historic use of aqueous film forming foam (AFFF) at Certified
Fire Training Facilities across the State. The Department has been conducting these investigations
because of two contaminants of emerging concern,perfluorooctanoic acid(PFOA) and perfluorooctane
sulfonate (PFOS),which historically have been used in a wide variety of products,including AFFF. As
part of this effort, the Department requested that the Monroe County Fire Rescue—Joe London Fire
Training Academy(Facility) complete a questionnaire regarding their use of AFFF. Because the Facility
indicated previous usage of AFFF,the Department's Site Investigation Section(SIS) collected samples
from ground water, surface water, sediment, and soil at the Facility on September 26, 2019.
Currently, there is no federal regulatory limit, or maximum contaminant level(MCL) for PFOS or
PFOA,however, the U.S. Environmental Protection Agency has established a Health Advisory Level
(HAL) to provide guidance to state and local officials in evaluating drinking water quality. The HAL is
the level below which adverse health effects are not anticipated to occur over a lifetime of exposure.
This HAL is 70 nanograms/Liter(ng/L). The Department,pursuant to Chapters 373 and 403, Florida
Statutes (F.S.), and the rules promulgated thereunder in Chapter 62-780, Florida Administrative Code
(F.A.C.),has derived provisional groundwater cleanup target levels (GCTLs) and soil cleanup target
levels (SCTLs) for PFOA and PFOS.
The Department received the certified sample results on October 21, 2019. The following link to

In Summary:
* The groundwater sample collected from well TMW-1 had a concentration of
1,100 ng/L of PFOA and 440 ng/L of PFOS, exceeding the provisional cleanup
target level of 70 ng/L. The groundwater sample collected from well TMW-2
had a concentration of 5,200 ng/L of PFOA and 12,000 ng/L of PFOS, also
exceeding the provisional cleanup target level.
* Every soil and sediment sample collected exceeded the provisional leachability
soil cleanup target level. Additionally, soil sample SS-8 exceeded the direct
exposure cleanup target level for PFOS.
* Surface water sample SW-1 exceeded the surface water screening levels for
PFOA and PFOS. SW-2 exceeded the surface water screening level for PFOS.
Based on these findings, there is an indication that PFOA and PFOS contaminants have been
released or discharged into the environment at the Facility. As the current property owner and
operator of the Facility, you may have legal responsibility for site assessment and rehabilitation
pursuant to Chapters 376 or 403, F.S., and the rules promulgated thereunder in Chapter 62-780,
F.A.C.
Please be advised that Chapter 62-780, F.A.C., establishes a process and time schedule for
assessing and remediating contaminated sites. All persons who have legal responsibility for site
rehabilitation, pursuant to Chapters 376 or 403, F.S., are required to comply with the provisions of
this rule and are subject to enforcement to compel such compliance.
A responsible party is required by 62-780, F.A.C., to initiate a site assessment within 60 days of
discovery of the contamination and to submit a site assessment report to the Department within
270 days of discovery of the contamination. As a potentially responsible party at the above-
identified site, Monroe County/Monroe County Fire Rescue is subject to the requirements for
assessment and remediation of such contamination under Chapter 62-780, F.A.C.
