TASK ORDER FOR PROFESSIONAL ENGINEERING AND SURVEYING SERVICES FOR THE BIG PINE KEY SENIOR CENTER STRUCTURAL ASSESSMENT In accordance with the Continuing Contract for Professional Engineering and Surveying Services, made and entered on October 21,2015,as first amended on April 19,2018,as second amended on October 17, 2018 and as third amended on August 21, 2019, between MONROE COUNTY, hereinafter referred to as the "County" and PEREZ ENGINEERING & DEVELOPMENT, INC., hereinafter referred 'to as "Consultant", where professional services are allowed 'for miscellaneous projects in which construction costs do not exceed One Hundred Thousand 00/100 Dollars($100,000.00.)or for study activity if the fee for professional services for each individual study under the contract does not exceed Ten Thousand and 00/100 Dollars ($10,000.00); hereinafter referred to as the"Agreement"the parties enter into this Task Order. All terms and conditions of the Agreement apply to this Task Order, unless this Task Order amends,adds,or modifies a provision or an Article of the Agreement of which will be specifically referenced in this Task Order and the amendment, addition, or modification shall be precisely described.These additions apply only to the project referenced in this Task Order. This Task Order is effective on 10 January 2020. WHEREAS,the Big Pine Key Senior Center on Big Pine Key is owned by Monroe County and in need of structural maintenance,and WHEREAS,the Consultant is qualified and able to perform a structural assessment of the building to allow the County to determine future plans for the building. NOW,THEREFORE,in consideration of the mutual promises and covenants set forth below,the parties agree as follows: 1. In accordance with Article II,SCOPE OF BASIC SERVICES of the Agreement,Perez Engineering&Development Inc.will perform an onsite visual assessment,of the structural members and deliver an assessment report of the findings.This scope of services does not intend to include destructive or in-situ testing of the existing members. The following is a brief description of the information;to.be,specified in the`stiuctural condition assessment. • General conditions of the structure o Including concrete access stairs and ADA ramp-. • • Summary defining members in need of repair • Photographs depicting members in need of repair • Page 1 of 2 , .. , �.✓MhLL%4"°,1'ii-V'ar:..iw:.._e•.-v:.-a.--...-ns... ' • Recommendations to owner(e.g.repair,replacement,or further investigation if necessary) • Estimated repair costs This assessment includes the senior center building only and is intended to encompass the structural members said building and their finishes(i.e. wall and roof coverings)only. These services exclude mechanical,electrical,and plumbing aspects of the building. Repair plans are not included in this scope and shall be under separate agreement,as needed. 2. In accordance with Article VII paragraph 7.1.1 of the Agreement,the County shall pay the Consultant a lump sum total of Four Thousand Two Hundred Fifty and 00/100 dollars ($4,250.00)paid on a percent complete basis for the following phases: a. Field Inspection-$1,500.00 upon completion of field inspection and approval by the Director of Project Management b. Assessment Report-$2,750 Upon Completion of assessment report and approval by the Director of Project Management All other Terms and Conditions of the Agreement remain unchanged and in full forces and effect.
