In accordance with the Consultant Contract for Professional Support Services, made and entered
into on March 19, 2014, and as First Amended on August 14, 2014, and as Second Amended on
February 18, 2015, and as Third Amended on March 18, 2015, and as Fourth Amended on
February 10, 2016, and as Fifth Amended on November 14, 2017 and as sixth amended on March
21, 2018, and as seventh amended on May 16, 2018,and as eighth amended on February 20,2019,
between MONROE COUNTY, hereinafter referred to as the "County," and M.T. CAUSLEY,
LLC hereinafter referred to as "Consultant," where professional services, including Inspection
Services, are allowed, hereinafter referred to as the "Contract".
All terms and conditions of the Contract as amended apply to this Task Order, unless this Task
Order amends, adds, or modifies a provision or an Article of the Contract of which will be
specifically referenced in this Task Order and the amendment, addition, or modification shall be
precisely described. These additions apply only to the project referenced in this Task Order.
This Task Order is effective on the 3rd day of January, 2020.
WHEREAS,the State of Florida building code requires owners to retain the services of a Threshold
Inspector to oversee the construction of structural elements of buildings that reach a specific
height, occupancy load, or square footage; and
WHEREAS, the new Plantation Key Courthouse and Detention Center meets those threshold
specifications necessitating the inspector requirement; and
WHEREAS, the County desires to retain the services of the Consultant to perform the threshold
inspection services for the Plantation Key Courthouse and Detention Center.
NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below, the
parties agree as follows:
1. In accordance with Article II, SCOPE OF BASIC SERVICES of the Contract, the
Consultant will perform services as described in the Threshold Inspection Plan
(Attachment A) and as outlined in the Proposal (Attachment B).
The duties of the Consultant will include but not be limited to the documentation of and
the publication of:

2. In accordance with Article III(COMPENSATION)of the Contract, the County shall pay
the Consultant a lump sum fee of Ninety Thousand and 00/100 Dollars($90,000.00)paid
monthly based upon completion of work performed and upon approval of the Project
Manager and the Director of Project Management.
All other Terms and Conditions of the Task Order shall remain in accordance with the Contract as
amended.

STRUCTURAL INSPECTION PLAN
PREFACE
1. The Threshold Inspection Services as described herein shall be paid for by the Owner, in accordance
with Florida Statutes.
2. The Special Inspector(S.I.) shall be a registered structural Engineer, licensed by the Florida Board of
Engineers to perform Threshold Inspections and included on the list of approved S.I.'s.
3. This inspection plan is intended as a minimum guideline for the S.I. and not intended to surrogate the
Building Official's or the Engineer of Record's requirements. Further, it is not intended that the
Contractor's contractual and statutory obligations are anyway relieved or foregone by the presence of
the S.I. The Contractor has the sole responsibility for any deviations from the Structural Construction
Documents. The S.I. does not replace the duties of the Building Official nor the quality control
personnel of the Contractor.
4. In addition to the requirements of the Board of Engineers,the S.I. shall have a minimum of five years
experience in inspection and actual design of similar structures. The S.I.'s authorized representatives
shall have a minimum of five(5)years experience in inspection of similar structures and shall also meet
the requirements of the Board of Engineers.
5. The authorized representative must dedicate his time on-site to the inspection requirements of this plan.
He shall not be required to perform other work on the project, including materials testing services.
6. Prior to work,the S.I. shall become familiar with the specific structural components that are to be
inspected and develop a thorough knowledge of the Contract Documents.
7. The Contractor shall cooperate with and assist the S.I. in performing his inspection duties as specified
below. The S.I. shall have free access to the project at all times. The contractor shall review this plan
and coordinate the schedule of work to accommodate the required inspections. The contractor shall
provide a minimum of twenty-four(24)hours notice to the S.I. for all inspections.
8. The Owner shall arrange for all necessary Contract Documents,including electronic copies of all
drawings and specifications,the geotechnical report and materials testing reports,to be furnished to
the Special Inspector during the progress of the work in a timely manner. Provide the Special
Inspector with electronic copies of all structural changes,revisions, addenda, etc.
PROCEDURES AND SCHEDULE
1. The S.I., or his authorized representative, shall perform site visits at a frequency determined by the
engineer-of-record to maintain inspections are being performed in accordance with this plan.
2. Minimum required inspections:
a. Inspections are to be made three days of each week during construction as a minimum.
b. The S.I. shall personally conduct an on-site inspection a minimum of once every two weeks.
c. All reinforcing bars are to be inspected prior to the concrete pour. This includes all masonry
reinforcing in addition to concrete reinforcing.
d. Inspector is to observe all concrete and grout pours from start to finish of each pour.
3. The Contractor shall advise the S.I. in advance of construction schedules and planned operations in
order to assure timely and appropriate observation inspection.
4. The S.I. shall cooperate with the Contractor and shall refrain from directing work, as this is expressly
not part of the S.I.function.

OBLIGATIONS
The S.I. is obligated to both Owner and the Building Official for observing that the work is executed in
substantive accordance with the Structural Contract documents, which include the permitted drawings
and specifications,addenda, and change orders, issued sketches and revision drawings, etc.
It is the obligation of the S.I. to notify the Contractor,Building Official,Architect/Engineer of Record,and
Owner of the following:
1. Observe the construction of the primary structural components as outlined in this plan and report that
these components are being built in general conformance to the Contract Documents.
2. The use of materials or workmanship which do not conform to the Structural Construction Documents
or which will cause improper construction which is not acceptable.
3. Work which is not being done in accordance with the approved Structural Construction Documents.
Items to be inspected include,but are not limited to,those listed herein.
4. The recommended removal of faulty construction or of construction performed without inspection and
not capable of being inspected in place.
5. The request for interpretations from the Architect/Engineer of record.
6. The contractor shall, upon being informed by the S.I., immediately cause to eliminate such
discrepancies and deviations.
CONSTRUCTION OBSERVATION
The S.I. shall visit the site at intervals appropriate to the stage of construction in order to observe the progress
and quality of the Work completed by the Contractor. Such visits and observation are not intended to be an
exhaustive check or a detailed inspection of the Contractor's work but rather to allow the S.I.,as an experienced
professional,to become generally familiar with the Work in progress and to determine, in general, if the Work
is proceeding in accordance with the Structural Construction Documents.
The S.I.shall not be responsible for inspection of any building component that is not shown or specified on the
structural contract documents.
The S.I. shall not supervise, direct or have control over the Contractor's work nor have any responsibility for
the construction means, methods, techniques, sequences or procedures selected by the Contractor nor for the
Contractor's safety precautions or jobsite safety programs in connection with the Work. These rights and
responsibilities are solely those of the Contractor.
The S.I.shall not be responsible for any acts or omissions of the Contractor,subcontractor any entity performing
any portions of the Work, or any agents or employees of any of them. The S.I. does not guarantee the
performance of the Contractor and shall not be responsible for the Contractor's failure to perform its Work in
accordance with the Contract Documents or any applicable laws,codes,rules or regulations.
FIELD REPORTS
The S.I.shall provide written Field Report to the Owner, Contractor,Architect, and Building Official after
each site inspection. Reports shall be signed/sealed by the S.I. and shall contain the following:

NON-CONFORMANCE LOG
S.I. shall keep a log of all deficiencies and review it on a daily basis with Contractor, updating as corrections
are completed. The S.I. shall issue a Non-compliance Notice to the Contractor for any items that are not
corrected in a timely manner. The S.I. shall not issue "Certification Letter" until all non-conformance items
have been resolved and closed.

MTCI Private Provider Services proposes to provide Threshold Building Inspection
Services required per Florida Building Code for construction of the Plantation Key
Judicial Courthouse and Detention Facility project located in the Village of Islamorada,
Florida. The project consists of a one-story detention center and a two-story
courthouse. This proposal is based on the EOR's Structural Inspection Plan, attached
hereto this proposal, provided to MTCI on 12.9.19.
Scope of work to be provided is limited to the following:
Threshold Building Inspection Services:
Scope of work includes field inspections during construction of the referenced project.
Field inspections will be limited to the inspection of the main structural components of
the building shell.
The Special Inspector shall observe that the work is executed in substantial compliance
with the permitted Official Contract Documents. The Official Contract Documents are
defined as the permitted plans, recorded addendums, and the specifications with all
amendments thereto, including the Structural Inspection Plan provided by the Engineer
of Record.
1 - DUTIES
The Special Inspector and his/her authorized representative shall maintain a
record of the progress, working conditions, observations given to the Contractor
and suspected deviations from the Official Contract Documents. The reports shall
be in writing and shall be made out promptly at the end of each day. The reports

will be present at all times on the job site in the form of a jobsite log and an
inspection field report for each day an inspection is made.
The report may consist of any or all of the following:
A. Report of each inspection
B. Jobsite log of area inspected
C. Special records (pile driving logs, mill tests, concrete tests)
D. Record of placing and curing concrete
E. Changes made in the field during a particular observation
2 - REPORTING INFORMATION AND OBSERVATIONS
It is the duty of the Special Inspector to perform following duties and notify the
Contractor, Architect/Engineer of Record, and the Owner of the following:
A. Inspect all primary structural components as outlined in Structural
Inspection Plan prior to their concealment, for compliance with the
permitted drawings on file with Monroe County and all standards of the
Florida Building Code; Report non-compliance with permitted documents
and anomalies. It is the responsibility of the contractor to submit to
Monroe County any revisions to the permitted documents.
B. The use of materials, equipment, or workmanship which does not conform
to the Official Contract Documents or which may cause improper
construction and is not acceptable.
C. Work which is not being done in accordance with the approved Official
Contract Documents.
D. The recommended removal or repair of faulty construction or of
construction performed without inspection and not capable of being
inspected or tested in place.
E. The request for interpretations from the Architect/Engineer of Record.
3 - REQUIREMENTS OF THE OWNER
A. The Owner shall arrange for all necessary construction records to be
furnished to the Special Inspector during progress of the work in a timely
manner. Such records shall consist of but may not be limited to: concrete
cylinder test reports, soil density test records, mill records, shop drawings,
shoring drawings, pile logs, etc. that may be requested by the Special
Inspector and available to the Owner or his/her Consultants.

B. The Owner shall request that the Contractor provides a shoring and
reshoring plan to the Building Department, which is signed and sealed by
an Engineer registered in the State of Florida. Such registered engineer
may represent the form supplier. Confirm that shoring and reshoring has
been inspected and approved by the Shoring Engineer.
C. The owner shall arrange for the services of the geotechnical engineer that
will certify all piling, if used, and filling compaction prior to pouring pile
caps or footings and slabs on grade.
D. The owner will arrange for the services of a testing agency to perform and
report all testing required by the contract documents.
4 - LIMITATIONS
Unless otherwise provided in written requests, this contract limits the Special
Inspector to provide services only with regard to the Structural Frame of the
Building including primary framing systems, and all items included in the
Structural Inspection Plan. Piling, if used, and soil compaction inspections will be
provided by the geotechnical consultant hired by the owner. This Contract does
not include inspection of any safety provisions as required by OSHA or other
safety standards, which apply during the construction period nor does it apply to
elements such as windows, doors, glazing, balcony railings, fire protection,
roofing, Mechanical/Electrical systems, Architectural components, site work or
other elements not contributing to the capacity of the structural building frame.
We are available to perform inspections for installation of the windows, doors,
glazing and guardrail systems, if so authorized.
All certifications issued will refer to the completed work being in substantial
accordance with the Official Contract Documents. Inspection reports of our
findings will be furnished on a weekly basis.
Inspectors assigned to this project will be individuals with substantial experience in
Special Inspections for Threshold Buildings. We are available to cooperate with the
Developer/Contractor's schedule, even if inspections are required seven days a week
and/or 24-hours per day.
We propose to have one part-time, experienced field representative inspector at the site
to start the project. However, during peak construction additional inspectors will be
assigned to this project on as needed basis, if requested by the Owner and the
Contractor. The number of inspectors and hours required will be established in
coordination with the Owner and the Contractor.

We propose to provide Threshold Building Inspection Services based upon
approximately 20 hours per week, Monday through Friday, and not upon an 8-hour day;
i.e. some days could be 10 hours and other 6 hours depending on the Contractors
schedule. Any time over 40 hours per week will invoiced additionally and at 1 1/2 times
the standard rate. Inspections requested on Saturdays will be invoiced at 1.5 times the
standard rate and 2 times the standard rate for inspections requested on Sunday and
US Federal recognized holidays. If two inspectors are required, they will have basic 40-
hour weeks. It is not envisioned that the single inspector will be on site for 20 hours per
week but some weeks for more and others a lesser hourly period. Special Inspector for
the Threshold building will be a registered professional engineer and will check the work
being performed by his/her field representative inspector. His/her time is based on a
standard rate of$150.00 per hour.
In order to estimate the Threshold and Building Inspection budget for this project, we
have considered thirty (30) weeks construction for the shell portion of the entire project.
Threshold and Special inspections will be needed during construction of only the
structural framing members of the project.
Fee for Threshold Inspections for the Main Frame of the building
One inspector at 20 hrs/wk for 30 wks (20 x 30 x$125.00): $ 75,000.00
Registered Engineer Supervisor for 30 weeks: $ 15,000.00
Main Frame Inspection Allowance for 30 weeks construction: $ 90,000.00
Invoices will be issued monthly upon the first inspection at the rate of$7,500 per month
with the assumption of this $90,000 project to be completed within twelve (12) months.
The fee is a based upon the parameters described above and does not include
inspections and certifications of auger pile installation, if used. Construction of the pile
caps and construction above the pile caps is included in the fee. We shall provide
Special Inspection for Threshold Building Services at $7,500 per month. Inspections
requested on Saturdays will be invoiced additionally at 1.5 times the standard rate and 2
times the standard rate for inspections requested on Sunday and US Federal
recognized holidays.
The Inspector shall perform inspection of the structural components and any special
inspection during construction and attend construction coordination meetings with the
Contractor and other entities involved on an as needed basis during his/her involvement
on full time basis. Checking of the shop drawings, RFI's and review of submittal and
response is not included in the scope of work of the field inspections, which will be
performed by the Architect and Engineer of record. Testing laboratory retained by the
Owner shall make concrete test cylinders, perform testing, and submit reports for
review.

Terms and Conditions
