TASK ORDER FOR PROFESSIONAL ARCHITECTURAL AND ENGINEERING SERVICES FOR THE MARATHON 63RD STREET GOVERNMENT ANNEX FLOOD PANEL MITIGATION In accordance with the Continuing Contract for Professional Architectural and Engineering Services, made and entered on December 19, 2018, between MONROE COUNTY, hereinafter referred to as the "County" and M.C. HARRY AND ASSOCIATES, INC. hereinafter referred to as "Consultant", where professional services are allowed for miscellaneous projects in which construction costs do not exceed Two Million 00/100 Dollars($2,000,000.00.)or for study activity if the fee for professional services for each individual study under the contract does not exceed Two Hundred Thousand and 00/100 Dollars ($200,000.00); hereinafter referred to as the "Agreement"the parties enter into this Task Order. This Task Order is effective on the 1 411 day of QOrt,kocr 1� , 2020. WHEREAS, the Marathon 63"d Street Government Annex Building was damaged during Hurricane Irma and requires hazard mitigation to prevent future damage, and WHEREAS, the Consultant is qualified and able to perform engineering design services for the addition of flood panels to the Marathon 63"1 Street Government Annex Building. NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below, the parties agree as follows: 1. In accordance with Article II, SCOPE OF BASIC SERVICES, of the Agreement, the Consultant will perform services as described below: Consultant shall provide architectural and engineering services related to designing and installing flood panels on the Marathon 63"I Street Government Annex Building as outlined in the attached proposal, hereinafter referred to as Attachment A. 2. In accordance with Article VII,Paragraph 7.1.1,COMPENSATION, the County shall pay the Consultant a lump sum total of Seventeen Thousand Seven Hundred Ten and 00/100 Dollars($17,710.00)paid on a percent complete basis for the following phases: a. Programming and Field Verification Phase-$4,324.00 upon site visit and approval by the Director of Project Management. b. Design Development Phase-$2,282.00 upon document review and approval by the Director of Project Management. c. Construction Documents Phase - $2,730.00 upon document review and approval by the Director of Project Management. Page 1 of 2 d. Permitting Phase -$1,072.00 upon receipt of permit and approval by the Director of Project Management. e. Bidding Phase-$1,177.00 upon issuance of notice to award a contract and approval by the Director of Project Management. f. Construction Contract Administration Phase - $6,125.00 upon completion of construction and approval by the Director of Project Management. 3. SCOPE The scope for this project is highlighted in the attached Department of Homeland Security Federal Emergency Management Agency Damage Description and Dimensions Report. These highlighted items identify the issues to p be resolved. The AE will provide design solutions for construction based on the following services: BASIC SERVICES COMPENSATION M.C. Harry Associates will perform the Work outlined in this Proposal for a total Lump Sum Basic Services fee of Seventeen thousand, seven hundred and ten dollars ($17,710). A detailed breakdown on man-hours anticipated to complete this project is provided as back-up information to substantiate the proposal. Programming and Field Verification Phase $4,324 Includes one site visit to verify scope and document existing conditions. Meeting with County PM to determine/validate project expectations. o Design Development Phase $2,282 Coordinate and provide design documents including site plan, floor plan, building elevations, details, and specifications. Provide preliminary construction cost estimate. Submit drawings for review. 0 Coordinate review conference call with County's Project Manager. Construction Documents Phase $2,730 Develop design documents to include County PM's comments-site plan, floor plan, building elevations, details, and specifications. LU Develop construction cost estimate. Submit 90% CDs for final Review. Incorporate Final comments and submit for final approval. 0 Permitting Phase $ 1,072 Signed and sealed documents and courier to City's PM for their submission to Building Department. Respond to permit comments. 0 Bidding Phase $ 1,177 cti Respond to bidding RFIs. Issue Addenda.Analyze Alternates/substitutions. Construction Contract Administration Phase $6,125 Administer the construction contract. Review shop drawings/submittals. Review and approve Pay Requisitions. Conduct final site visit to determine Substantial Completion. Process close-out documents. AE Contingency Allowance $ 0 For unforeseen conditions that may arise during design.AE will need County's authorization before additional scope is done. BASIC SERVICES TOTAL-Lump Sum Fee $17,710 490 63rd Street Flood Protection Panels AE Fee Proposal Page 2 ASSUMPTIONS&CLARIFICATIONS We anticipate the total period to complete design phases for this project will be 2 months from your authorization to proceed. Our services anticipate site survey and as-built drawings will be provided by the City and are available for our use. Our fee is generated based on the level of effort we anticipate to provide the services mentioned above. Services requested beyond scope identified will attract additional services fees and such requests will be performed after such scope and fees are formally authorized by client. We do not anticipate the need for and hence have not included an allowance for Topographical Surveys, Geotechnical Engineering, Hazardous Materials Testing or Abatement, Destructive Investigations, Extended CA, Locating UG Utilities, etc. LEED certification is not a requirement for this project, therefore such services are not included. The service of a permit expeditor is not included. This proposal is valid for 90 days. MC Harry Associates will perform our services in accordance with the terms and conditions of our Professional Architectural and Engineering Services Agreement executed on 09/24/19. Sincerely, M. C.
