emergency repairs. Approval to authorize County Administrator to execute construction contract for
emergency repairs to No Name Key Bridge on behalf of the Board of County Commissioners.

AMENDMENT 2 TO THE AGREEMENT FOR ENGINEERING DESIGN AND
PERMITTING SERVICES FOR THE NO NAME KEY BRIDGE REPAIRS
PROJECT.
THIS CONTRACT AMENDMENT 2 IS ENTERED INTO BETWEEN Monroe
County, whose address is 1100 Simonton Street, Room 2-216 Key West,Florida 33040,
hereafter the"County",and Kisinger Campo & Associates, Corp. a Corporation of the
State of Florida, whose address is 201 North Franklin Street,Suite 400, Tampa,
Florida 33602,its successors and assigns, hereinafter referred to as"CONSULTANT",
Paragraph 3.1.1 is added to ARTICLE III SCOPE OF ADDITIONAL SERVICES,and
is to state the following:
3.1.1 The CONSULTANT will perform for the COUNTY Engineering Design
Services for emergency repairs as described in Attachment D, Proposal for
Engineering Design for Emergency Repairs.
The service's as described are for a Lump Sum fee not to exceed$36,060.00

Re: No Name Key Bridge - Emergency Repair Proposal (Amendment 2 to the Engineering Design
Contract)
Dear Mr.Briggs:
We are pleased to submit this proposal for engineering services for the above referenced project. The scope of
services for this task will consist of the development of repair plans for Bent 26 of the No Name Key Bridge.
The services will be performed in conformance with the Scope of Services in the Professional Services
Agreement,and in accordance with the following:
1. PROJECT OBJECTIVES
This project will consist of designing emergency repairs for Bent 26 of the No Name Key Bridge.
Repairs will be based on deficiencies noted in the most recent Florida Department of Transportation
(FDOT) bridge inspection reports and a follow-up field review to confirm these conditions. As part of
these services, the Consultant shall develop repair plans, construction documents and coordinate with
the County and selected contractor as noted.
2. SPECIFIC SERVICES
The Consultant will review the most recent bridge inspection reports and visit the site to observe the
current condition of the cap, beams and bearings at Bent 26. Visible deficiencies (cracks, spalls and
desegregation of aggregate) in the caps, beams, and bearings shall be documented and included as part
of the design calculations.
The consultant will coordinate with the County's selected contractor in an effort to determine a cost
effective repair that can be quickly constructed with the contractor's available equipment and
capabilities. The Consultant will develop contract plans and documents for the restoration of the
appropriate bearing area for Beam 26-4 at Bent 26 and concrete restoration of associated bridge
elements. Concrete restoration will likely consist of crack injection and spall repair. The consultant
will also provide post design services as noted in the attached fee sheet.
The need for traffic control plans is not anticipated. Lane closures will be directed in accordance with
the FDOT 600 series Design Standards and/or county approved procedures.

3. COUNTY RESPONSIBILITIES
The County shall provide copies of the bridge inspection reports, allowed lane closure/bridge closure
times and other pertinent information at the Consultant's request. In addition,the County shall provide
all required repair permitting and Construction Engineering Inspection(CEI).
4. BEGINNING AND LENGTH OF SERVICES
The above outlined services shall begin upon the receipt of the Notice to Proceed from the County.
Final plans will be delivered to the County prior to the agreed upon production date. Submittals will
include:
• preliminary plans(in electronic pdf format and 11x17 hard copy of quantity requested)
• Final plans(final signed and sealed hard copies and CD upon approval)
5. COMPENSATION
The fee for the services detailed above is based on the hourly rates listed in the master contract. These
rates result in a total lump sum compensation of$36,060.00 in accordance with the attached Summary
Fee Sheet.
