Scope:
Limits
The survey shall consist of four(4)parcels comprised of"Commercial Ramp", "General
Aviation', "Additional General Aviation Parking" and "Additional General Aviation
Parking West".
1. Sketch &Description:
We shall prepare sketch and descriptions for the four (4) Signature Lease Boundaries
based on the approximate sketches provided to us. The Sketch&Description shall be in
accord with the Standards of Practice as set forth in Chapter 5J-17 FAC pursuant to
Section 472 Florida Statutes.
Prior to execution of the final documents, we shall submit the Sketch & Description for
review and final approval.
Changes and/or revisions to the configuration of the Sketch & Description after final
approval, shall incur additional fees.

2. Boundary Survey of Signature Lease Boundaries:
A boundary survey of each individual Signature Lease Boundary parcel shall be
performed and certified in accord with the Standards of Practice, Chapter 5J-17, Florida
Administrative Code, pursuant to Chapter 472.027, Florida Statutes. Monuments shall
be established at all changes in direction, unless if falls on the paved surfaces, where we
are prohibited from setting markers that will perforate the surface.
Limited location of existing improvements critical to the description of the parcels shall
be performed so that calls within the legal description can be made. Some examples are
the location of the edge of the paved aprons, paved taxiways, face of buildings and face
of hangars.
Requirements to the survey requested by governmental agencies that are more stringent
than Standards of Practice as set forth in Chapter 5J-17 FAC pursuant to Section 472
Florida Statutes will be an additional expense to client based on our contracted hourly
rates.
Clarification:
• The legal descriptions of the four(4) Signature Lease Boundaries shall be described
as a portion of the overall boundary of the Key West Airport property,with Point of
Commencement originating from a well monumented boundary line of the airport
property.
• This proposal is based on performing the boundary survey and the locations of
existing improvements, simultaneously with one mobilization.
• Client shall arrange for all access to the property for our crew to perform the survey.
Delays caused by the inability to gain access to the property at no fault of Avirom&
Associates, inc. may incur additional charges.
• This proposal is based on the site being free and clear of storm debris. We will not
move obstacles that impede obtaining the survey data, or if field conditions are
considered hazardous to the safety of our crews.
• The fee for the fieldwork has a factor for delays due to airport operations; however,
excessive delays caused by severe air traffic may incur additional fees.
Timeframe:
We can mobilize to the site within two(2)weeks after receipt of an executed contract and
have the work completed four(4)weeks after mobilization.
Deliverable:
Upon completion of the survey, we will provide three(3) signed and sealed hard copies
of the sketch and descriptions and digitally signed PDF copies. Federal Express,
deliveries and out-of-pocket expenses will be charged to the client at cost.

Access Road Design — Phase 1
Scope of Work
Proiect Descri tion
The proposed project is a two-lane landside access road that provides connectivity from S. Roosevelt
Boulevard to EYW's fuel farm, fixed base operator (FBO), overflow parking, commercial transportation
and airport support areas. The proposed access road is located on the west side of EYW adjacent the
current Department of Motor Vehicles (DMV) office and one-hour parking lot and will require
reconfiguration of the impacted parking areas and commercial transportation staging area.
The proposed access road will separate airport support services from airport passenger traffic on Faraldo
Circle, reducing traffic congestion in the terminal area. The proposed access road would also eliminate
fuel truck and other large vehicle traffic from the ingress/egress passenger terminal area, providing an
added level of safety, as well as reconfiguring impacted parking and staging areas to provide enhanced
customer parking and hotel shuttle and Transportation Network Company (TNC) staging away from the
congested terminal curb.
The Phase 1 scope of services is for a schematic design (15%) of the access road and impacted parking
areas. The design will include a preliminary road and parking geometry and initial drainage layout. The

project will also include identifying the processes and procedures to construct the access road with Florida
Department of Transportation (FDOT), South Florida Water Management District (SFWMD), Florida
Department of Environmental Protection (FDEP), US Army Corps of Engineers (USACE), DMV, Monroe
County,and Keys Energy. The approximate location of the Access Road is shown in Figure 1. Removal of
existing entrance roads is also as shown in Figure 1.
General Scope of Services
One schematic design package will be submitted for this project. The schematic design package includes
the following:
1. Management of the project from data collection to schematic design of the project.
2. Coordination with the geotechnical subconsultant(Tierra South Florida)and survey subconsultant
(Avirom).
3. Identify process and procedures for permits with FDOT, Monroe County, SFWMD, Keys Energy,
USACE,and FDEP.

4. Identify appropriate NEPA Category and requirements.
5. Identify utility,environmental,and overhead lighting impacts of proposed access road and parking
improvements.
6. Coordination with Keys Energy for overhead utility relocation as required.
7. Coordination with FDOT, Monroe County,and city of Key West for the proposed access road,curb
cut,and sidewalk.
8. Coordination with SFWMD for the preliminary drainage design alternatives.
9. Coordination with FDEP for wetland mitigation asrequired.

Assumptions/Design Parameters
1. This project will be funded through PFC Application No. 17.
2. McFarland Johnson will investigate the potential impacts the project will have on local and state
permit agencies, including but not limited to FDOT, FDEP, USACE,SFWMD, Monroe County, Keys
Energy,and the city of Key West.
3. The road geometry, pavement and drainage layout will be in accordance with the most current
FDOT and Monroe County Design Standards.
4. A new curb cut and side walk will be constructed at the intersection with S. Roosevelt Boulevard.
5. The existing wetland mitigation area will be delineated by McFarland Johnson and surveyed by
Avirom. Potential wetland mitigation areas will be investigated.
6. It is assumed that the existing storage sheds will be relocated by the Airport prior to construction.
7. It is assumed that the fuel farm will not have to be relocated.
8. Reconfiguration of the existing parking lots will seek to consolidate and maintain, If not increase,
available parking. Reconfiguration of the hotel shuttle and TNC staging area will seek to increase
available curb-frontage.
Specific Scope
Specific tasks (Basic and Special Services) related to the above items are identified in each phase of the
project as described below.
Task 1—Data Collection
1. Coordinate with survey and geotechnical subconsultants.
2. Perform a detailed topographic survey of Project Area(access road and surrounding parking areas)
as shown in Figure 1. The survey will consist of the following:
A. A 25-foot by 25-foot grid for the pavement areas shall be surveyed.
B. A 50-foot by 50-foot grid for turf areas shall be surveyed.
C. Survey shots taken on pavement will have an accuracy of 0.01'and those on turf shall be 0.1'.
D. Grade breaks within the grid will be surveyed.
E. Locate all above ground structures, utilities and pavement markings.
F. Locate all drainage inlets and manholes,provide top of grate and invert elevations for existing
drainage inlets and manholes

G. Locate top of bank and toe of slope of the existing swale.
H. Provide vertical project control points(benchmarks)on NAVD '88 datum.
I. Provide horizontal project control points on NAD'83 datum.
J. Provide a Baseline of Survey based on pavement location.
3. Perform a geotechnical investigation to assist in determining the best method to construct the
Access Road and developing the drainage design. The geotechnical investigation includes:
A. SPT-Borings(5 @ 10')
B. Four(4)pavement cores.
C. Perform three(3) Constant Head Tests to a depth of 20 feet per SFWMD method.
D. Up to four(4)CBR/LBR tests depending on uniformity of soils across the site.
E. A written summary report of this investigation including the calculated values for hydraulic
conductivity.
4. Delineate existing wetlands within the 4.4 acre project area and proposed 0.38 acre mitigation
site. Prepare a memo and figures) summarizing the findings of the wetland delineation. The
memo will also describe other notable environmental conditions observed during the delineation.
5. Coordinate with USFWS and FL DEP to preliminarily identify potential federal and state listed
endangered species that may be locatedwithin the project area
6. Coordinate with the Florida Division of Historic Resources to Identify known historic,architectural,
and archaeological sites within the project area.
7. Research SFWMD permits for existing drainage design.
8. Research record drawings for underground utilities. (i.e.sewer,water,electric).
9. Perform two(2)site walk through surveys to locate and note any special site conditions that would
affect construction techniques or materials. Also, to field verify that all existing features in the
field were picked up during the actual survey.
Task 2-Basic Services&Schematic Design
Based on the information collected in Task 1, a schematic design will be developed for all elements
identified in this scope of work.
1. Prepare for and attend meetings with EYW to discuss project progress and review the design
options.
•2. Prepare for and attend meetings with FDOT,Monroe County,Keys Energy,and SFWMD to discuss
the details of the Access Road.

3. Develop two (2) alignment alternatives for the Access Road entering the airport and/or tenant
leasehold and S. Roosevelt Boulevard to comply with FDOT and Monroe County design standards.
4. Develop up to three(3) parking configuration alternatives for one hour and tenant parking areas
in conjunction with the access road alternatives.
5. Develop up to three(3) hotel shuttle and TNC staging road and curb configuration alternatives in
conjunction with the access road alternatives.
6. Develop a preliminary pavement design that includes recommended methods of construction and
recommended pavement design.
7. Develop preliminary construction quantities and planning level cost estimates at the schematic
document design levels for each alternative.
8. Develop drainage layout alternatives for the proposed project area to discuss with SFWMD during
the Pre-Application meeting.
9. Evaluate the potential wetland and environmental impacts and research avoidance,minimization,
and mitigation options.
10. Summarize environmental permitting requirements and timelines. Identify key Issues and
obstacles that will be considered by the permitting agency.
11. Identify the appropriate NEPA category and timeline based on the project description and
expected environmental consequences.
12. Develop the following anticipated plans for the final selected/preferred alternative:

• Demolition Plans(4)

Topographic Survey of Proposed Access Road
Dear John,
The topographic survey will be performed to support the design of a proposed access road at Key
West International Airport(E'YW).
Limits:
The survey limit is based on the exhibit you provided in your e-mail dated February 5, 2019 and
shall extend to the centerline of Overseas Highway all within the area defined in green on Exhibit
A(below), incorporated herein for reference.
Scope:
Topographic Survey:
Obtain sufficient elevations to define the existing topography in detail based on a 25-foot by 25-
foot grid on impervious surfaces and 50-foot by 50-foot grid on natural ground surfaces, including
significant grade changes in elevations. The elevations shall be referenced to the North American
Vertical Datum of 1988 (NAVD 88) and horizontally referenced to the North American Datum of
1983 (NAD 83-2011)Epoch 2010.00 and tied to the National Geodetic Survey(NGS) benchmark
network. Additionally,we shall establish a minimum of four(4)benchmarks at the site. We shall
calculate and place the vertical conversion factor from NAVD 88 to the National Geodetic Vertical
Datum of 1929 (NGVD 29)on the face of the map.
We shall locate all above ground improvements including, but not limited to pavement, ramps,
driveways, steps, gates,top of curb,bottom of curb,flow line, edge of pavement,pavement

markings, centerline of pavement,bottom of gutter, back of walk, face of walk,top of bank,toe of
slope, and above ground evidence of utility. We will obtain rim elevations,pipe size,material and
inverts of sewer and drainage structures where possible. Due to the nature of obtaining the data
where the structures are in use,the information obtained may be the best approximation of the true
measurements. It is the responsibility of the end user to verify the diameter and material utilizing
as-built drawings or other resources prior to construction. Utility locations will include fire
hydrants,water valves,meter boxes,vaults and electrical outlets,power poles and overhead wires.
We will locate wells, main irrigation valves, but will not locate individual sprinkler heads.
There is no provision for the excavation, probing or location of underground utilities structures or
improvements. Utilities shall be located to the extent that they are above ground and visible. This
proposal does not include the contracting of an underground utility locating service to locate the
subsurface utilities.
The survey data will be reported to the nearest 0.03 feet for horizontal locations and for the vertical
component, 0.1 feet spot elevations on ground shots and 0.01' spot elevations on paving or hard
surfaces, with all work performed in accordance with the Standards of Practice as defined in
Chapter SJ-17,Florida Administrative Code.
We shall establish a monumented Baseline of Survey, based on the existing pavement location.
We shall obtain all recorded plats and right-of-way maps to graphically delineate the platted
boundary lines within the digital files. Surveyor shall recover sufficient monumentation in the
field to spatially place the boundary within the digital file. There is no provision to perform
boundary surveys or surveys to resolve conflicts, ownerships or occupation issues relative to the
recorded instruments. This information will be spatially placed within the digital file of the
topographic survey, based on recovered monumentation to depict the lines relative to the
topographic locations. This is not a boundary survey and the information should not be relied
upon as such. It is strongly recommended that if the design is contingent on accurate boundary
placement and especially prior to construction,a boundary survey should be performed to establish
the lines. We will not be responsible for the misuse of the topographic survey for purposes it was
not intended.
Tree Survey:
We will locate all indigenous and protected specimen trees having a 3" and larger caliper diameter
at breast height relative to the boundary, with the tree species listed to the best of knowledge and
ability of the surveyor (without the benefit of a botanist or landscape architect). It is the
responsibility of the end user to verify the identity of the species.
We will tag each tree with a number and obtain the diameter and horizontal position of the trees.
It is the responsibility of the client's consultant (botanist or landscape architect) to correctly
identify the species relative to the tag number, which we will provide as an Excel tabulated listing.

The Excel tabulated table listing which will comprise of the tag number, diameter and species of
trees(corrected by the client's consultant and returned to us) will be incorporated into our survey.
The location of shrubbery and landscaping is not included in this proposal. Large,bunched groups
of trees shall be located as clusters with a count and diameter listed. We will not locate exotic or
non-native species.
Mangroves consist of prop roots and by nature do not possess a defined tree trunk; therefore,the
edge of the mangroves will be located.
Jurisdictional Wetland Location:
We shall coordinate with the client's environmental consultant to locate the wetland flags as
established by their firm. The wetland figures as defined by the flags shall be delineated on our
survey, together with ties to the boundary and calculated areas of impacted wetlands. The flags
shall be marked with the name of the polygon and have a commencement point,usually beginning
with the number 1, and numbered sequentially with the termination point also clearly defined, so
our crew will know if a point has been skipped and also know when the last flag has been
recovered. The nomenclature of the different classes of impacted wetlands shall be provided by
client's consultant,which will be added to our survey. The survey shall be in compliance with the
survey procedures as defined in Chapter 62-343, Florida Administrative Code.
Clarifications:
1. Client shall arrange for all access to the property. Delays caused by the inability to gain access
to the property at no fault of Avirom&Associates, Inc. may incur additional charges.
2. Requirements to the survey requested by governmental agencies that are more stringent than
Standards of Practice as set forth in Chapter 5J-17 FAC pursuant to Section 472 Florida
Statutes will be an additional expense to client based on our contracted hourly rates.
3. This proposal is based on mobilization by our crew from Boca Raton to service the project.
4. This proposal is based on the site being free and clear of storm debris. We will not move
obstacles to obtain survey data, or if field conditions are considered hazardous to the safety of
our crews.
5. The Tree Survey is based on performing the field work in conjunction with the topographic
survey.
6. The Jurisdictional Wetland Location Survey is based on performing the field work in
conjunction with the topographic survey. It is recommended that the wetlands are flagged by
client's consultant prior to our crews mobilizing. Delays caused by the wetland consultant will
incur additional costs.

Deliverables:
Two signed and sealed hard copies of the survey (including a PDF file of the hard-copy) shall be
furnished to the client, together with an AutoCAD (2017 or lower) digital drawing file created in
Carlson 2017.
Timeframe:
We can mobilize to the site within two(2)weeks after a notice to proceed, contingent on available
accommodations and have the project completed within four(4) weeks after de-mobilization.
Cost:
The fee to perform the above services shall be lump sum itemized as:
1. Topographic Survey .............................................................$20,037.00 lump sum
2. Tree Survey ......................................................................$ 4,710.00 lump sum
3. Jurisdictional Wetland Location ............................................ $ 2,710.00 lump sum
TOTAL: $27,457.00 lump sum
GENERAL CONDITIONS:
All work shall be completed under the direction of a Professional Land Surveyor registered in the
state of Florida. All work shall be conducted using equipment,personnel,and procedures that will
insure compliance with the accuracy standards as defined by State of Florida Standards of Practice
in Chapter 5J-17, FAC. All documents submitted shall bear the Surveyor's seal, signature, and a
certificate that all work was done under the Surveyor's supervision and that all information
contained in the document is true and is accurately shown. The Surveyor is responsible for quality
assurance for the survey work performed_on this project. All survey information shall be recorded
in hard bound field books or in electronic data collector files.

Dear John:
As requested,Tierra South Florida,Inc. (TSF) is pleased to submit this proposal for the above-
referenced project. Based on information provided by McFarland-Johnson,proposed construction
will consist of approximately 550 lineal feet of roadway on the west side of the Key West
International Airport building,with some new roadway and some.rehabilitated pavement.
PROPOSED SCOPE OF WORK
Our scope of work will consist of:
• Five (5) Standard Penetration Test(SPT)boring to a depth of 10 feet in the roadway area;
• Four(4) asphalt cores;
• Three(3) Constant Head permeability tests to a depth of 15 feet per SFWMD method;
• Up to four(4)CBR/LBR tests;and
• A written summary report of this investigation including the calculated values of hydraulic
conductivity.
Prior to drilling at the project site, TSF will notify the local utility companies and request that
underground utilities be marked. Our experience, however, is that the utility companies will not
mark privately owned utilities. Private utilities will be the owner's/Airport's responsibility. Our
proposal assumes that private utility lines will be located in the field by others prior to
mobilization of the drill rig.TSF will recommend a utility line locating service upon request.
We believe that significant coordination will likely be required to perform the field operations.
This proposal is based on the assumption that all boring locations are open and accessible to
a truck mounted drill rig.
ESTIMATED FEES
It is proposed that the fee for the performance of the services outlined above is determined on a
unit price basis in accordance with the attached Fee Schedule and that the work be performed

pursuant to TSF's General Conditions enclosed herewith and incorporated into this proposal. On
the basis of the proposed quantities, it is estimated .that the total fee will be approximately
$13,552.50.
Our estimate covers the work needed to present the results.Not included are reviews of drawings,
preparation of construction specifications, special conferences and any other work requested after
submittal of our report.
SCHEDULE AND AUTHORIZATION
TSF will proceed with the work after receipt of a signed copy of this proposal. With our present
schedule, we can commence work within several days of project approval (weather permitting)
and fieldwork is expected take about 2 days to complete. The report can be submitted in about
three weeks after completion of the field work.
