Detailed description of change order and justification:
Change Order No. 4 is required by the Fire Marshall and Monroe County Sheriffs Office. It is described
as follows:
1) Replacement of rusted fire sprinkler system in the existing building. Fire Marshall $3,004.23
requirement.
2) Two additional fire extinguishers. Fire Marshall requirement. 102.24
3) CADD drawings of the existing facility fire alarm system as required by the Fire Marshall. 1,595.21
4) Install a different type of electronic security hinge on security doors as requested by the 2,934.64
Sheriffs Office.
Change Order No. 4 thereby increases the design/build contract with Gomez Construction Company by
$7,636.32, from $346,427.32 to$354,063.64. Contract time remains unchanged.

Monday, June 14, 1999, and will return at 8:00 a.m Monday, June 21, 1999.
During my absence, Dave Koppel, County Engineer, will have full authority to act
in my behalf, including attendance at meetings and execution of agenda, fiscal
and payroll documents.
