Substantial Completion should be no later than April 11, 1999.

the Monroe County Board of County Commissioners on November 12, 1998. Please sign the original Change
Order and mail back to us as soon as possible for execution.

Detailed description of change order and justification:
Gomez Construction Company was awarded a design/build contract at the Plantation Key Detention Facility at
the January 16. 1996 BOCC meeting: Modifications to proposed drawings dated 12/6/96. requested by the
Judges.and minor modifications by the Sheriffs office.include:
• Labor.material. &equipment to construct 2 additional meeting rooms(app. 8'x10'each). $27.741.00
• Provide accessibility from the Court Building for two storage spaces and block up from
detention side. 3.052.00
• Provide a 3'x3'borrowed light with security glazing at N.wall of Booking Room No. 103.680.00
• Provide electrically operated security lock at door No. 1 to be controlled from the
existing Control Room. 1.667.00
• Provide a security type shower at Juvenile Holding Room 105. 3.611.00
• Fill all masonry cells with concrete at walls of Janitor Room No. 107, 0.00
• Provide janitor type sink in lieu of elevated for Maint, Shop Room No. 110. 0.00
• Delete door 12 at Maintenance Shop. 0.00
• Enlarge existing control room by removing south wall and provide a new partition
approximately 10'south of present location. 6.533.00
• Remove existing exit door communicating Court Building with proposed addition and
provide new 3'0"wide door to meet accessibility requirements. 0.00
• Provide 24"x36"vision panels with security glazing at door No's 1.2.3.5.6.&9. 2,747.00
• Negotiated credit for the above work. (443.00)
• Architectural Services to incorporate all the above additions and deletions. 3,000.00
• Reimbursement for Buildina Permit from Islamorada.Village of Islands. 1.900.00
• Courthouse windows to have one hour fire resistive separation. (Fire Marshal Requirement) 1.000.00
• Exterior door to be equipped with a panic hardware device, (Fire Marshal Requirement) 477.00
The contract with Gomez Construction therefore increases in contract price by $51.965.00, from $268.000.00
to $319.965.00. and substantial completion time extends 150 calendar days, from date of BOCC approval
(11/12/98)to-April 11. 1999.
