Detailed description of change order and justification:
The County engaged the services of a Surveyor for a Boundary, Mean High Water, and Topographic
Survey of the property on which the new library will be built The survey results had discrepancies and
reported incorrect datum information on elevations thus requiring revisions to construction documents after
the construction contract was executed. As a result of this error, a condensing unit stand is now required to
raise the unit from the 8.0 mechanical slab elevation designed from incorrect datum to the required 10.04
flood elevation. This additional work results in a contract increase of$7,915 03.
No additional time has been requested for this work.
Chance order is 0.11% of original contract once.

added construction costs, including but not limited to, the mechanical slab being design at the
incorrect elevation and thus requiring the installation of a condensing unit stand to achieve proper
elevation.

product Control Section to be used in Miami badeCounty and other areas where allowed by the Authority
Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami—Dade County Product

revision stamp with the Notice of Acceptance number & expiration date by Miami-Dade County Product
Control Section.
