AMENDMENT 1 TO THE AGREEMENT UNDER THE
HUMAN SERVICES ADVISORY BOARD (HSAB) FUNDING
PROGRAM WITH WOMANKIND, INC.
THIS AMENDMENT is made and entered in to this 15th day of July 2020 between the BOARD
OF COUNTY COMMISSIONERS OF MONROE COUNTY,FLORIDA,hereinafter referred to as"COUNTY'
and Womankind,Inc., hereinafter referred to as'AGENCY'.
WHEREAS,the County participates in funding non-profit Human Service Organizations;and
WHEREAS, the AGENCY is a not-for-profit corporation established for the provision of
healthcare services for women living in Monroe County;and
WHEREAS,the COUNTY awarded a grant to the AGENCY to implement a program providing
healthcare services for women in Monroe County;and
WHEREAS,due to the impacts from the COVID-19 pandemic there is a need for medical
supplies and medical laboratory costs;and
WHEREAS,an amendment to the Agreement is needed to reflect a change to ATTACHMENT
C to include medical supplies and laboratory costs;and
NOW THEREFORE IN CONSIDERATION of the mutual promises and covenants contained
herein,it is agreed as follows:
1. ATTACHMENT C as revised is attached hereto and shall amended to add the following:
Due to the COVID-19 pandemic. fundina will also be used for medical
supplies and medical laboratory costs.
2. In all other respects the Agreement dated October 16,2019 remains in full force and
effect.

Specific description or list of services to be provided under this contract:
HSAB funding will be used for partial salaries of the medical team, including board certified
physicians, nurse practitioners, physician assistants, medical assistants, malpractice insurance,
medical billers, and a medical billing consultant to streamline revenue cycle management (RCM) and
increase collection rates.
In our original application, we had requested funding for partial rent of our office at 1511 Truman.
In September 2019, Womankind became a mortgage holder on the property, and we ceased seeking
reimbursement for any rental costs.
Due to the COVID-19 pandemic, funding will also be used for medical supplies and medical
laboratory costs,
