06/23/2020 Agreement TASK ORDER FOR PROFESSIONAL ARCHITECTURAL AND ENGINEERING SERVICES BETWEEN MONROE COUNTY AND WOOD ENVIRONMENT & INFRASTRUCTURE SOLUTIONS,INC. FOR VETERANS MEMORIAL PARK ADA PAVILION AND TIKI HUT REPLACEMENT In accordance with the Agreement for Professional Architectural and Engineering Services,made and entered into on December 19, 2018 between MONROE COUNTY, ("County") and WOOD ENVIRONMENT & INFRASTRUCTURE SOULUTIONS, INC., ("Consultant"), where professional services are allowed if construction costs do not exceed $2,000,000.00, or for study activity if the fee for professional services for each individual study under the contract does not exceed $200,000.00 ("Agreement"), the parties enter into this Task Order. All terms and conditions of the Agreement apply to this Task Order, unless this Task Order amends, adds,or modifies a provision or an Article of the Agreement of which will be specifically referenced in this Task Order and the amendment, addition, or modification shall be precisely described. These additions apply only to the project referenced in this Task Order. This Task Order is effective on the �3rd of ;1$ n.e, , 2020. WHEREAS,Veterans Memorial Park on Little Duck Key is a County owned and maintained park, and WHEREAS, the park sustained heavy damage during Hurricane Irma which require repairs to be made including replacement of the ADA pavilion and five tiki structures. NOW,THEREFORE, in consideration of the mutual promises and covenants set forth below, the parties agree as follows: 1. In accordance with Article II, SCOPE OF BASIC SERVICES of the Agreement,the Consultant will provide structural engineering and design services including complete construction drawings and specifications for the pavilion structure repairs and will review submittals from qualified contractors for erection of the new tiki structures. The Scope will include: a. Task I —Conceptual Design; Consultant will provide conceptual design consisting of up to two recommendations for construction materials and at least one site layout. b. Task 2— Final Design; Consultant will finalize design upon discussion with County and provide Final Design package including drawings and specifications. c. Task 3 — Bidding& Permitting Support; Consultant will prepare and submit a Monroe County building permit package for the pavilion and tiki structures. Consultant will hold a pre-application meeting with the Department of 1 Environmental Protection to discuss the project. Consultant will respond to any bidding or permitting comments and update the design package if needed in order to receive applicable permits. d. Task 4—Construction Support Services; Consultant will respond to RFIs and Submittals from the Contractor during construction and confirm compliance with the Consultant's design. 2. In accordance with Article V I I paragraph 7.1.1 of the Agreement,the County shall pay the Consultant a lump sum total of Twenty-One Thousand Five Hundred Fourteen and 00/100 dollars($21,514.00)paid on a percent complete basis for the following phases: a. Task 1, Conceptual Design - $3,274.00 upon receipt of conceptual design with recommendations and approval by the Director of Project Management b. Task 2, Final Design - $11,004.00 upon document review and approval by the Director of Project Management c. Task 3, Bidding & Permitting Support- $ 5,856.00 upon completion of bidding and receipt of all required permits d. Task 4, Construction Support Services - $1,380.00 upon completion of project All other Terms and Conditions of the Agreement remain unchanged and in full forces and effect.
