Property owners, Gary and Jennifer Moline were issued permit #13301974 for FEMA INSPECTION
COMPLIANCE PROGRAM on May 17, 2013 as evidenced by Monroe County Receipt# 03 000015212
in the amount of $300 for property located at 335 Laguna Ave.; Key Largo, FL. The owners have
requested a refund as the flood inspection was no longer required to renew their flood insurance after
June 29, 2013. A refund in the amount of$300 is requested to be issued to the owners as noted above
and on the attached Requisition for Refund from County Treasury.

• Planning and Environmental Resources Permit:
This Permit serves as a Planning and Environmental Resources permit, under the Monroe County Land
Development Code.
• APPEAL PERIOD:
o Local Appeal Expiration Date:
30 Days from 05/17/2013
Building permit shall not be rendered until the time within which to file any local administrative
appeals pursuant to local ordinances has expired. Work completed prior to the expiration of this
local appeal period is at owner's risk.
o State Appeal Expiration Date (If applicable):
30 Days from 05/17/2013 + 45 Days from receipt of permit from Monroe County to the state.
A development order shall not take effect or be acted upon by the developer until 45 days after
rendition, unless a later date is specified in the order, and the effectiveness of a development
order shall be stayed by the filing of a notice of appeal pursuant to Section 380.07, Florida
Statutes. Whenever possible, local governments shall issue concurrently all local permits or
development orders that may be applicable to the proposed development. Monroe County has
up to 5 days to process and submit to state.
• MONROE COUNTY, STATE OF FLORIDA, and other FEDERAL AGENCIES CODES:
o In consideration of the granting of this permit, it is agreed that in all respects the work will be
performed and completed in accordance with the permitted plans and the applicable Building,
Zoning and Environmental codes Monroe County, State of Florida and Federal agencies.
o This permit may be revoked at any time upon the violation of any of the provisions of said laws,
ordinances or rules and regulations or upon any change in the plans and specifications
unauthorized by this department.
o In addition to the requirements of this permit, there may be additional restrictions applicable to
this property that may be found in the public records of this county, and there may be additional
permits required from other governmental entities such as Water Management Districts, State
Agencies, and/or Federal Agencies.
• EXPIRATION:
Permits shall expire and become null and void if work, as defined in this permit
o is not commenced within 180 days from the date of issuance date and an approved inspection
has not been obtained within 180 days, unless extended by the Building Official
0 or 180 days from the State Appeal date and does not meet 180 day inspections thereafter unless

SUBJECT: Conclusion of the Pilot Inspection Procedure in Florida
y The Pilot Inspection Procedure for Monroe County,the Village of Islamarada,and the City of
Marathon,FL,is not being extended and will end on June 28,2013.The following guidance
addresses the requirement for current and former policyholders in these communities.
a
�' • Current policyholders who have been notified of the inspection requirement and have
policy renewal dates prior to June 29,2013,cannot renew these policies without an
inspection. Inspections are also required for new applications for affected properties
whose policies will become effective prior to June 29,2013.Alternatively,these
policyholders can obtain new policies after the inspection procedure ends.They must
submit a new application and appropriate effective date rules must be applied.The
underwriting file must contain an Elevation Certificate(EC)and photographs. We
anticipate that very few policies will fall into this category.
• Current policyholders who were notified of the requirement for inspections and have
policy renewal dates after June 28,2013,are not required to obtain inspections as a
requirement for policy renewal.
• Former policyholders who were previously notified of the inspection requirement but
never obtained inspections of their buildings may reapply for flood insurance.The
effective date of such policies can be no earlier than June 29,2013,and appropriate
effective date rules must be applied in all cases.Full underwriting information must be
submitted for this new business, including an application,EC,and photographs.
With the expiration of the inspection procedure,NFIP Legacy System Services will no longer be
sending lists of properties requiring an inspection. Insurers should not send letters to
policyholders requiring an inspection.
