AGENDA ITEM WORDING: Issuance(renewal) of a Class A Certificate of Public Convenience and
Necessity (COPCN)to Falck Southeast II Corp. d/b/a American Ambulance Service (American)for the
operation of an ALS transport ambulance service; specifically for inter-facility transports.
ITEM BACKGROUND: In September of 2012 a Class A COPCN was issued to American to operate
an ALS transport ambulance service (specifically inter-facility). In addition, at this MCBOCC meeting
American Ambulance's name change to Falck Southeast II Corp. d/b/a American Ambulance Service
was presented and approved.
American's certificate will be expiring on September 21, 2014. In view of the foregoing American is
applying to renew this COPCN which would become effective September 22,2014.
PREVIOUS RELEVANT BOCC ACTION: On September 21, 2012 the MCBOCC approved the
issuance of a Class A COPCN to American for the operation of an ALS transport ambulance service
(specifically inter-facility)for the period September 21,2012 to September 21, 2014.
CONTRACT/AGREEMENT CHANGES: The current COPCN for American will expire on
September 21, 2014. The renewal being applied for will cover the period September 22, 2014 to
September 21, 2016.

EMERGENCY MEDICAL SERVICES
CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY
WHEREAS, the Monroe County Code allows for the issuance of Certificates of Public Convenience and
Necessity for Emergency Medical Services by the Board of County Commissioners of Monroe County;
and,
WHEREAS, the Falck Southeast H Cori). d/b/a American Ambulance Service provides quality
emergency medical services to the citizens of Monroe County;and,
VMEREAS, there has been demonstrated that there is a need for the above named service to operate
in this County to provide essential emergency medical services to the citizens of this County;and,
WHEREAS,the above named service has indicated that it will comply with all the requirements of the
Monroe County Code and Chapter 401 Florida Statutes, the Board of County Commissioners of
Monroe County hereby issues a Certificate of Public Convenience and Necessity to this service for the
period beginning September 22, 2014 and ending September 21, 2016
