Yes ❑ No i If Yes, explanation: • Change Order was included in the original specifications. Yes ® No ❑ If Yes, explanation of increase in price: The walks and ramps in this Change Order were effected by grade elevation changes due to the Village of Islamorada requiring extensive revisions to the site after the bid. This was all related to the hammock, adjacent these doors and sidewalk. Additionally, it would be more costly and less desirable to have steps. The MCSO will use entrances on this side to bring in DUI arrests, and navigating steps vs. a ramp when intoxicated or if a person is wheel chair bound presents the county with a liability alleviated by correcting this situation with ramps that meet ADA requirements. • Change Order exceeds $25,000 or 5% of contract price (whichever is greater). Yes ❑ No If Yes, explanation as to why it is not subject for a calling for bids: • Project architect approves the change order. Yes ® No ❑ If no, explanation of why: • Change Order is correcting an error or omission in design document. Yes ❑No Should a claim under the applicable professional liability policy be made? Key West, FL 33040 RE: Mariner's, Proposal Request No. 19 Recommendation i Dear Don_ I have reviewed the contractor's response to Proposal Request No. 19. The following are my findings: I 1. This proposal request developed during a field review of existing site conditions. Specifically, the existing ramped sidewalk to Door No. 32 is not fully code compliant. We have an opportunity to correct minor code violations and create an additional ADA accessible entrance at the Detective Division. A similar situation exists at the entrance to Patrol and Proposal Request No. 19 addresses that area as well, by adding curbs, ramps, handrails and sidewalks. 2. Patrol Officers bring D.U.I. suspects in at this entrance and detectives bring suspects and prisoners in for interrogation at their entrance. In loth cases, the proposed ramps and sidewalks will make transporting suspects by the police safer for the officers and the public. This change will improve working conditions and reduce the potential for injury and/or lawsuits. 3. I have determined that the pricing is fair. 4. This work was not included on the original documents or is a modification of the work that was included. Handrails are added for safety and full ADA compliance. 5. The County receives benefit through this change because they will have a safer and more efficient facility, which will fully serve the needs of disabled citizens. Specifically, more wheelchair accessible entrances are being provided. Based on the above, I recommend accepting this change order request in the amount of$13,334.20.
