Comments: OMB Form Revised 9/11/95 MCP #2 AMENDMENT 1 to AGREEMENT This Amendment 1 to the Agreement dated October 17, 2014, is made and entered into this 10th day of December, 2014, between the BOARD OF COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA, hereinafter referred to as "Board" or "County," and Womankind, Inc., hereinafter referred to as "PROVIDER." WHEREAS, the PROVIDER is a not-for-profit corporation established for the provision of women"s health care; and WHEREAS, the County and the PROVIDER entered into an Agreement on October 17, 2014 for the provision of the services as set forth above; and WHEREAS, the PROVIDER has requested to revise Attachment C to amend the services to be funded under the original agreement; and WHEREAS, this Amendment revises the Agreement to revise Attachment C, now, therefore, IN CONSIDERATION of the mutual promises and covenants contained herein, it is agreed as follows: 1. This Amendment revises the original Agreement to revise Attachment C (see attached). 2. In all other respects the Agreement dated October 17, 2014 remains in full force and effect. Womankind is a medical and wellness center providing high-quality family planning, primary health care, and mental health counseling to people of all income levels. 2. Annual gynecological exams and Pap tests Clinical breast exams Mammogram referrals and vouchers • Breast health diagnostics and secondary care assistance Teen services Testing and treatment for sexually transmitted infections Screening for diabetes and heart disease • Colp,oscopy Human Papillomavirus testing Menopausal counseling • Bioldentical hormones • Minor illness treatments Hormone replacement therapy A range of birth control methods • Emergency contraception • Pre-conception and interconceptional care • Pregnancy testing and options,counseling • Referrals to appropriate providers for prenatal care, abortion,and adoption services • School physicals • Human immunodeficiency Virus(HIV) pre-and post-test counseling,and HlV testing • Hispanic outreach and translation Wellness classes, educational forums Free or low cost contraceptives and antibiotics Free and low cost lab work • Referrals to specialized or secondary care providers, mental health providers (GCC) • Outreach and education • Reduced fees for those living at or below 250%of the federal poverty level • Patient navigation for health insurance coverage through the Affordable Care Act • Bridging the Cap program to provide free and sliding scale services for those patients not eligible under Title X • County-wide comprehensive sexual health education in middle and high schools throughout Monroe. 3- What specific services will be funded by this request? Womankind will use HSAB support for 50% of our rent and varying percentages of the following staff salaries: executive director, assistant director, and front desk staff.
