AGENDA ITEM WORDING: Approval of Second Amendment to the Task Order with CPH, Inc.
for the Monroe County Sheriffs Office Administration Building Envelope Evaluation and Repair
project in the amount of $33,680.00. This Second Amendment adds Construction Administration
Services.
ITEM BACKGROUND: The MCSO Administration Building has continuing water intrusion
issues during rainfall events even after repairs to the roof and replacement of the windows. The
water intrusion has been determined to be caused by the breach of the cladding of the envelope of
the building. Project Management issued a Task Order to CPH, Inc. for the evaluation and analysis
of the building on February 24, 2020 which determined the cause of the water intrusion. A First
Amendment to the Task Order with CPH was signed on June 12, 2020 which added Construction
Drawings and Specifications for the repairs of the building to the Scope of Work. Project
Management signed a construction contract with Keystar, Inc. for the repairs to the building on July
31, 2020. Project Management now desires to have CPH continue oversight of the construction
phase of the project. This Second Amendment to the Task Order will add Construction
Administration services to the Scope of Work.
PREVIOUS RELEVANT BOCC ACTION:
8/19/20 —BOCC approved/ratified a contract with Keystar, Inc. for the emergency repairs to the
exterior insulation and finish system (EIFS) at the MCSO Admin Building.

In accordance with the Consultant Agreement for Professional Architectural and Engineering
Services, made and entered into on December 19, 2018, between MONROE COUNTY,

where professional services are allowed if construction costs do not exceed Two Million and
00/100 Dollars ($2,000,000.00), or for study activity if the fee for professional services for each
individual study under the contract does not exceed Two Hundred Thousand and 00/100 Dollars
($200,000.00), hereinafter referred to as the "Agreement".
All terms and conditions of the Agreement apply to this Second Amendment to the Task Order,
unless this Amendment to the Task Order amends, adds, or modifies a provision or an Article of
the Agreement of which will be specifically referenced in this Amendment to the Task Order and

the amendment, addition, or modification shall be precisely described. These additions apply only

WHEREAS, the parties executed a Task Order on February 24, 2020 whereby the Consultant
performed an inspection and provided reports on their findings; and E

WHEREAS, the parties executed the First Amendment to the Task Order on June 12, 2020 0

NOW, THEREFORE, in consideration of the mutual promises and covenants set forth below, the
parties agree as follows:

Consultant and/or subconsultant will perform services as described below:
a. Phase IV—Construction Administration
Consultant and its Subconsultant will provide construction administration services
