FIRST AMENDMENT TO TASK ORDER FOR
PROFESSIONAL ASSESSMENT OF PER-AND POLYFLUOROALKYL
SUBSTANCES (PEAS)SERVICES BETWEEN MONROE COUNTY AND
CARDNO,INC.FOR JOE LONDON FIRE TRAINING ACADEMY
In accordance with the Agreement for Professional Architectural and Engineering Services,made
and entered into on December 19,2018 between MONROE COUNTY,hereinafter referred to as
the "County," and CARDNO, INC. hereinafter referred to as "Consultant," where professional
services are allowed if construction costs do not exceed $2,000,000.00 or for study activity if the
fee for professional services for each individual study under the contract does not exceed
$200,000.00 (`Agreement"), the parties enter into this First Amendment ("Amendment") to the
task order issued on January 22, 2020,revised on February 19, 2020("Task Order").
All terns and conditions of the Agreement apply to this Task Order, unless this Task Order
amends, adds, or modifies a provision or an Article of the Agreement in which case the Article
will be specifically referenced in this Task Order and the amendment, addition, or modification
shall be precisely described. These additions apply only to the project referenced in this Task
Order.
WHEREAS, Florida Department of Environmental Protection Site Investigation Section
collected samples of ground water, surface water, sediment and soil to test for perfluorooctanoic
acid(PFOA) and perfluorooctane sulfonate(PFOS) at the Monroe County-Joe London Fire
Training Academy(Facility)on September 26, 2019; and
WHEREAS, the Florida Department of Environmental Protection ("FDEP") notified Monroe
County Fire Rescue of high concentrations of said contaminants in a letter dated November 8,
2019; and
WHEREAS, the County is required under Rule 62-780.600, Florida Administrative Code, to
conduct a site assessment at the Facility; and
WHEREAS, the Consultant conducted said site assessment and submitted the findings to the
Florida Department of Environmental Protection; and,
WHEREAS,the Florida Department of Environmental Protection has requested that further study
and assessment be conducted.
NOW THEREFORE, IN CONSIDERATION of the mutual promises and covenants set forth
below,the parties hereby agree as follows:
1. In accordance with Article II, SCOPE OF BASIC SERVICES of the Agreement,the
Consultant will conduct soil and groundwater assessments and prepare a Site Assessment
Report(SAR)in accordance with Tasks 1-4 shown on Cardno's Technical and Cost

Proposal, Attachment"A" to this Task Order, incorporated herein by reference. The
scope of work includes:
a. Surface Water Assessment including:
1) Collection of two(2)surface water samples from the marine surface water
body, collected at or near low tide, to the northeast and northwest of the
Joe London facility, for analysis of PFAS constituents.
b. Soil Assessment including:
1) Collect step-out delineation samples to the north, south, east, and west of
the R-SCTL exceedance location(SS-8);
2) Analyze these samples for both total PFAS concentrations and synthetic
precipitation leaching procedure(SPLP) analysis;
3) Collect 2 soil samples from a single soil boring located immediately
adjacent to well MW-I and to run SPLP analysis on these two depth
interval samples;
4) Use data to draw conclusions regarding the potential leaching of PFAS
from shallow soil(and limestone), throughout the site.
c. Ground Water Assessment including:
1) Install five (5) (originally four (4)) monitor wells to a depth of
approximately 12 feet below land surface(ft bls);
2) Sample the wells for PFAS constituents of concern.
3) Groundwater elevation data will be collected from all monitor wells prior
to sampling of the five new wells.
d. Data Evaluation and Report Preparation(Supplemental SAR Report):
1) Evaluate and discuss information obtained during implementation of
Tasks 1 through 3 within a Supplemental SAR document;
2) Sample results will be summarized in tables and on figures,and associated
lithologic logs, groundwater sampling logs, and laboratory analytical
reports will be included within the document;
3) Conclusions and recommendations will be provided in the report,
including a preliminary recommendation regarding the need for additional
sampling or potential remedial measures.
2. In accordance with Article VII,paragraph 7.1.1 of the Agreement,the County shall pay the
Consultant a lump sum fee of Twenty-five Thousand, Eight Hundred and 00/100 Dollars
($25,800.00)paid for the following phases:
a. Drilling(five shallow soil borings and five deeper borings with well installation) -
$16,000.00 upon completion and approval by the Assistant County Administrator.
b. Laboratory(10 soil samples[SPLP&total PFAS],5 GW samples+4 QA samples)
- $5,800.00 upon document review and approval by the Assistant County
Administrator.

c. Cardno fees and expenses (includes one meeting with FDEP) - $4,000.00 upon
document review and approval by the Assistant County Administrator.
3. This Amendment is effective on the 21'day of October 2020.
4. All other terms and conditions of the Task Order remain unchanged and in full force and
effect.

Dear Kevin:
Cardno recently completed an initial investigation of PFAS impacts at the referenced
facility, submitting a Site Assessment Report(SAR)to the Florida Department of
Environmental Protection (FDEP) in May 2020. Within that document, Cardno had
proposed installation of four(4)additional monitor wells to delineate the lateral extent of
documented groundwater impacts of perfluorooctanoic acid(PFOA)and perfluorooctane
sulfonate(PFOS) above the current Provisional Cleanup Target Levels (CTLs)and U.S.
EPA Health Advisory Levels (HALs) of 70 nanograms per Liter(ng/L). FDEP has
reviewed the May 2020 SAR report and proposal, and has requested: a)an additional
groundwater monitor well to the north of the northern borrow pit (5 wells total), b)two
marine surface water samples from the water bodies north of the peninsula on which the
Joe London facility resides, and c)an evaluation of leachability of PFAS constituents
based on prior exceedances of the leachability soil cleanup target level (SCTL)that were
detected at several shallow soil sample locations.
Cardno has reviewed the FDEP comments with your staff, and provided a direct
response to FDEP on September 24, 2020. The following technical and cost proposal is
for the completion of the requested activities, as proposed within the September 24, 2020
responses.
Proposed Scope of Work
Task 1 —Surface Water Assessment
Cardno with collect two (2) surface water samples from the marine surface water body to the
northeast and northwest of the Joe London facility, for analysis of PFAS constituents. These

samples will be collected at or near low tide, with tide table documentation provided within the
associated reporting.
Task 2—Soil Assessment
An exceedance of the residential direct exposure soil cleanup target level (R-SCTL)was
encountered at a single soil sample location at the Joe London Training Academy Site; however,
the leachability to groundwater SCTL(L-SCTL)was exceeded at several locations. Per FDEP's
request to investigate the relationship between L-SCTL results and GCTL exceedances, and to
better delineate impacts associated with the single R-SCTL exceedance location, Cardno
proposes to collect step-out delineation samples to the north, south, east, and west of the R-SCTL
exceedance location (SS-8), and to analyze these samples for both total PFAS concentrations
and synthetic precipitation leaching procedure(SPLP)analysis. In addition to these four sample
locations(2 depths each), Cardno will also collect 2 soil samples from a single soil boring located
immediately adjacent to well MW-1 and to run SPLP analysis on these two depth interval
samples. These data will be used to draw conclusions regarding the potential leaching of PFAS
from shallow soil (and limestone),throughout the site.
Task 3—Groundwater Assessment
Cardno proposed four additional monitor wells to delineate PFAS impacts to groundwater within
the May 2020 SAR; however, FDEP requested an additional monitor well to the north of the
borrow pit to the north. As a result, Cardno will install five(5) monitor wells to a depth of
approximately 12 feet below land surface(ft bls), and will sample the wells for PFAS
constituents of concern.
After wells are installed and developed, the top of casing elevation of all monitor wells will be
surveyed relative to an arbitrary datum to allow determination of groundwater flow at the site.
Groundwater elevation data will be collected from all monitor wells prior to sampling of the five
new wells.
Task 4—Data Evaluation and Report Preparation (Supplemental SAR Report)
The information obtained during implementation of Tasks 1 through 3 will be evaluated and
discussed within a Supplemental SAR document. Sample results will be summarized in tables
and on figures, and associated lithologic logs, groundwater sampling logs, and laboratory
analytical reports will be included within the document. Conclusions and recommendations will
also be provided in the report, including a preliminary recommendation regarding the need for
additional sampling or potential remedial measures.
Cost Proposal
This cost proposal assumes two days total for mobilization and demobilization to and from the site
(Drilling Contractors and Consultants), four days of borings and well installations and development,
and two additional days to complete subsequent groundwater sampling. Cardno is providing the
following cost proposal as a lump sum (LS) proposal that includes all subcontractor services and a
limited amount of activity by Cardno. Substantial budget remains on the current cost-plus-multiplier
task assignment and Cardno will bill field activities for this proposal to the existing purchase
agreement, reducing the cost of this LS proposal.

BLANKET NOTIFICATION TO OTHERS OF CANCELLATION OR NONRENEWAL ENDORSEMENT
This endorsement adds the following to Pad Six of the policy.
PART SIX
CONDITIONS
Blanket Notification to Others of Cancellation or Nonrenewal
1. If we cancel or non-renew this policy by written notice to you, we will mail or deliver notification that such
policy has been cancelled or non-renewed to each person or organization shown in a list provided to us by
you if you are required by written contract or written agreement to provide such notification. However, such
notification will not be mailed or delivered if a conditional notice of renewal has been sent to you. Such list:
a. Must be provided to us prior to cancellation or non-renewal;
b. Must contain the names and addresses of only the persons or organizations requiring notification that
such policy has been cancelled or non-renewed;and
a Must be in an electronic format that is acceptable to us.
2. Our notification as described in Paragraph 1. above will be based on the most recent list in our records as of
the date the notice of cancellation or non-renewal is mailed or delivered to you. We will mail or deliver such
notification to each person or organization shown in the list:
a. Wthin seven days of the effective date of the notice of cancellation, if we cancel for non-payment of
premium;or
b. At least 30 days prior to the effective date of:
(1) Cancellation, if cancelled for any reason other than nonpayment of premium;or
(2) Non-renewal, but not including conditional notice of renewal.
3. Our mailing or delivery of notification described in Paragraphs 1. and 2. above is intended as a courtesy only.
Our failure to provide such mailing or delivery will not:
a. Extend the policy cancellation or non-renewal date;
b. Negate the cancellation or non-renewal;or
c. Provide any additional insurance that would not have been provided in the absence of this endorsement.
4. We are not responsible for the accuracy, integrity, timeliness and validity of information contained in the list
provided to us as described in Paragraphs 1.and 2. above.
All other terns and conditions of this policy remain unchanged.
This endorsement changes the policy to which it is attached and is effective on the date issued unless otherwise stated.

THIS ENDORSEMENT CHANGES THE POLICY. PLEASE READ IT CAREFULLY.
This endorsement modifies insurance provided under the:
Commercial Automobile Coverage Part
A. If we cancel this Coverage Part by written notice to the first Named Insured for any reason other than nonpayment of
premium, we will mail or deliver a copy of such written notice of cancellation:
1. To the name and address corresponding to each person or organization shown in the Schedule below;and
2. At least 10 days prior to the effective date of the cancellation, as advised in our notice to the first Named Insured,
or the longer number of days notice if indicated in the Schedule below.
B. If we cancel this Coverage Part by written notice to the first Named Insured for nonpayment of premium, we will mail
or deliver a copy of such written notice of cancellation to the name and address corresponding to each person or
organization shown in the Schedule below at least 10 days prior to the effective date of such cancellation.
C. if notice as described in Paragraphs A. or B. of this endorsement is mailed, proof of mailing will be sufficient proof of
such notice.

Advice of Cancellation to Entitles Other than the Named Insured Limited to EJdall
Notification
In the event that the Insured cancels this policy for any reason other than non-payment of
premium, and:
(a) the effective date of cancellation is prior to the expiration of the Period of Cover, and
(b) the Named Insured is under an existing contractual obligation to notify a certificate
holder when this policy is cancelled (hereinafter the'Certificate Holders.), and has
provided to the Underwriters either directly or through its broker of record, the email
address of the contact at such entity,and the Underwriters have received information
after the Named Insured received notice of cancellation of this policy and prior to this
policy' cancellation effective date, tia electronic spreadsheet that is acceptable to the
Underwriters.
The Underwriters shall protide adtice of cancellation via e-mail to such Certificate Holders.
Proof of the Underwriters emailing such addce, using the information prodded by Cardno
USA Inc, will sense as proof that the Underwriters have fully satisfied their obligations under
this endorsement.This endorsement does not affect, in any way, caerage prodded under
this Policy or the cancellation of this Policy or the effective date thereof, nor shall this
endorsement invest any rights in any entity who is not an Insured under this Policy.
