PERMITTING SERVICES BETWEEN MONROE COUNTY AND KISINGER CAMPO
AND ASSOCIATES, CORPORATION
FOR BIMINI DRIVE BRIDGE (#904603) REPLACEMENT PROJECT
In accordance with the Continuing Contract for On Call Professional Architectural and
Engineering Services made and entered on the 19th day of December 2018, between
Monroe County hereinafter referred to as the "County" and Kisinger Campo and
Associates Corporation hereinafter referred to as "Consultant" where professional
services are allowed if construction costs do not exceed $4,000,000 in accordance with
F.S. 287.055 (as amended per Chapter 2020-127).
All terms and conditions of the referenced Contract For On Call Professional
Engineering Services apply to the Task Order, unless the Task Order modifies an Article
of the Agreement of which will be specifically referenced in this Task Order and the
modification shall be precisely described.
This Amendment to the Task Order dated November 20, 2018 is effective November 17,
2020 and is being amended as follows:
Article II Scope of Basic Services, Paragraph 2.1, the Scope of this Task Order is
amended to include:
This task will consist of Post Design Services for the Bimini Drive Bridge (#904603)
Replacement Project. Tasks will include attending the preconstruction meetings,
attending progress meetings, shop drawing review, submittal review, responding to
contractor questions/change requests, dynamic testing of foundation piling, geotechnical
supervision and review, environmental/permitting, support/inspection and FOR
construction coordination and additional Scope of Services as included in
Attachment A.
The Consultant shall furnish to the County, upon project completion, the following:
• 1 set of 11" X 17" signed and sealed Record Drawings
• 1 PDF version of signed and sealed Record Drawings
• 1 set of final documentation
• 1 set of final CADD files on CD
The Consultant's Engineer of Record in responsible charge of the project's design shall
professionally endorse (signed and sealed and certified) the record prints, the special
provisions and all reference and support documents.
Article VII, Paragraph 7.1 is amended to include the following:

Project Description: Monroe County on Call Professional Engineering Services -2017 Contract
Re: Task Work Order No. 1 —Amendment No. 2—Bimini Drive Bridge—Post Design Services
Dear Ms. Clarke:
Kisinger Campo &Associates (KCA)is pleased to submit this Scope of Services and Fee Estimate for the
above referenced project. This task will consist of Post Design Services for the Bimini Drive Bridge
Replacement Project. Tasks will include attending the preconstruction meetings, attending progress
meetings, shop drawing review, submittal review, responding to contractor questions/change requests,
dynamic testing of foundation piling, geotechnical supervision and review, environmental/permitting
support/inspection and FOR construction coordination. For additional scope details,see the attached Scope
of Services and Staff Hour Estimation Forms.
We have estimated the level of effort required for all engineering services described above. Our total
estimated not to exceed fee amounts to $122,974.00. This fee includes all KCA post design efforts, sub-
consultant efforts, coordination, QA/QC and supervision. The fee estimate is based on hourly rates which
include our overhead.
Our expenses were estimated separately and are not included in the fee estimate. Our total estimated
expenses amount to $8,600. Expenses will be itemized and submitted separately with each invoice. For a
detailed description of the fee and estimated expenses, see the fee proposals in the attached Appendix.

SUPPLEMENTAL SCOPE OF SERVICES
Project Description: Bimini Drive Bridge Replacement
Monroe County, Florida
PURPOSE
The purpose of this Exhibit is to describe additional services (Post Design Services) outside the
original scope of work agreed to between the COUNTY and the CONSULTANT and amend
the original task work order with this supplement.
PROJECT COMMON AND PROJECT GENERAL TASKS
Post Design Services
General — The general tasks include services to continue contract maintenance and
project coordination as required, including construction coordination meetings and
special purpose meetings and coordination activities during construction. These services
include working with the County's designated Construction Engineering and Inspection
consultant (CEI) to reconcile issues during construction. These general tasks will also
include support as necessary of activities regarding local stakeholder concerns.
Construction Assistance - The CONSULTANT shall provide to the COUNTY qualified
representation during the construction phase to deal with issues concerning the intent
and interpretation of the construction contract plans and documents prepared in the
work. Should changed conditions be encountered in the field, and when requested by the
COUNTY, the CONSULTANT shall respond in a timely manner with suitable
engineering solutions, which take into account the changed conditions. On site
appearance of the CONSULTANT shall be made during construction at the request of
the COUNTY or its designated representative. During construction,the CONSULTANT
may be requested by the COUNTY or its designated representative to review contractor
proposed field changes or to respond with a recommended solution to remedy particular
field situations not covered by the plans and specifications.
Contract Maintenance — Maintenance of the project including status updates for the
duration of the construction project ( days contract duration), including project
certification and closeout.
PROJECT MANAGEMENT AND STRUCTURES SUPPORT
Post Design General: The CONSULTANT shall provide additional services for the
review and approval structures related Requests for Information (RFI's), Requests for
Modifications (RFMs), shop drawings and other miscellaneous contractor submittals in
accordance with the construction documentation and accepted design criteria established
during project development. Utility coordination support will also be provided as
necessary.

ROADWAY/MOT/DRAINAGE SUPPORT
Post Design General: The CONSULTANT shall provide additional services for the time
and information necessary to review and approve all roadway related Requests for
Information (RFIs) and Requests for Modifications (RFMs) including those related to
drainage,traffic control plans, signing/pavement marking and utilities.
ENVIRONMENTAL PERMITTING SUPPORT
Post Design General: The CONSULTANT shall provide post design environmental and
permitting support services. These services include coordination with environmental
regulatory agencies, which have issued permits and authorizations for the project;
coordination with the project design team, CEI, and Monroe County for review of
Requests for Information (RFI's), questions, or construction methods to ensure
compliance with permits and authorizations issued for the project. The Florida Keys
National Marine Sanctuary (FKNMS) has required pre-and post-construction surveys of
submerged resources (stony corals) within the project right of way (ROW). KCA will
perform and prepare documentation for submerged resource surveys as required by the
FKNMS.
GEOTECHNICAL SUPPORT
Post Design General: The CONSULTANT through our sub-consultant Tierra SF, shall
provide additional services for the time necessary to review and approve all geotechnical
related shop drawings and Requests for Information (RFI's) in accordance with the
construction documentation and accepted design criteria established during project
development. The review of pile load test results and site meetings will also be provided
upon request.
DYNAMIC TESTING
Post Design General: The CONSULTANT through our sub-consultant GFL, shall
provide the dynamic testing and reporting as required by the specifications for driven
piles. See the attached Dynamic Testing Proposal for additional information.
EXISTING SCOPE AND AMENDMENTS
All other tasks and scope are and will remain in effect for the project duration under the
current Agreement as well as contract language not modified or mentioned in this
amendment.

Hi Jason:
Thank you for your interest in our dynamic pile testing services. As you may be well aware, GRIL
Engineers, Inc., along with our sister company Pile Dynamics, Inc., are the inventors, developers, and
manufacturers of modern dynamic pile testing methods and equipment (e.g., Pile Driving AnalyzerO
(PDA), Pile Integrity Tester (PIT), GRLWEAP wave equation analysis, CAP P computer program,
Saximeter, etc.). GRIL is the world's oldest and largest specialty consulting testing firm specialized in
PDA dynamic pile testing. We routinely perform PDA pile testing consulting services throughout the
United States and around the world, with extensive experience in Florida. Naturally, we are
interested in performing the dynamic PDA pile testing and related data analyses for your project as
specialty sub-consultants. GRIL will assign qualified and experienced engineers for the field-testing,
data analysis and reporting of results. All work will be done according to project specifications and
requirements, and professional and technical standards and care.
As Dynamic Testing Engineer (DTE), we understand that our scope of service covers the field PDA
testing and reporting of two (2) 18-inch square prestressed concrete test piles monitored during initial
driving. GRIL will issue a summary report of the testing results and include recommendations for
production pile length and driving criteria (Project Geotech Design Engineer/ EOR to review and
issue final requirements). Our unit rate charges for this scope of work will be the following..
1. Review of contractor's Pile Installation Plan (PIP) and pre-piledriving meeting (GRL attends
meeting via teleconference)- $ 1,000.00
2. Field testing (Engineer, PDA Equipment, mobilization, and travel), first day on site per trip- $
3,500.00
3. Field testing (Engineer and PDA Equipment), additional subsequent days during the same trip
to site: $ 1,500.00/day
4. Report of testing results (2 test piles), with recommendations for production piles- $ 2,500.00
*Add $500/day for weekend or holiday field testing and/or required travel to the jobsite
Our estimated total charges are $13,500 and assumes the following quantities, naturally, we will
charge only for work done:
• One (1) Pile Installation Plan review and teleconference pre-piledriving meeting
(included wave equation analysis)
• Two (2) mobilizations, and four (4) days of GRIL engineer and PDA equipment onsite
• One (1) summary report (2 test piles) and recommendations for production pile driving.
Please note that we must charge for each day onsite and any additional work performed outside of
this listed scope.
We require at least one week notice for mobilization. Any testing equipment (gages, cables, etc.) that
is damaged due to circumstances outside of our immediate control during the testing will be
repaired/replaced and charged to you at cost.
GRL is not liable (including claims for damages of any kind related thereto) for any delays (including
schedule-related) or failures to perform services due to government-imposed or health-related travel
or shipping restrictions, including health-related quarantines affecting GRL offices, personnel or
equipment suppliers.
