The Contract is changed as follows:
As a result of permit delays and a required flood elevation permit change to raise the slabs at Cudjoe Key
Transfer Station 24 inches and the slab at Long Key Transfer Station 12 inches, contract increased by
$10,700.00 for additional concrete material and a time extension of 84 days was required for contractor to
complete the project and provide proper closeout documentation.
The original Contract Sum $202,611.00
Net change by previously authorized Change Orders $ 0.00
The Contract Sum prior to this Change Order was $202,611.00
The Contract Sum will be increased by this Change Order $ 10,700.00
The new Contract Sum including this Change Order is $213,311.00
The Contract Time will be increased by eighty-four(84) days.
The date of final completion date as of this Change Order is December 12,2014.

Enclosed please find hilly-executed original of Change Order #1 for two (2) Hazardous
Material Storage Sheds increasing contract time by eighty-four (84) days for permit delays
and changing contract sum by $10,700 for required flood elevation permit change to raise
concrete slabs at the Cudjoe Key and Long Key Transfer Stations. The change order was
executed by Debbie Frederick, Deputy County Administrator, today (3/9/2015), in
accordance with Ordinance 026-1998.
