ITEM BACKGROUND: The County annually provides the required local match for transportation
services for the disadvantaged. $41,225.00 was approved by the BOCC as part of the FY2021
operating budget for required local match for the Community Transportation for Disadvantaged
program.
PREVIOUS RELEVANT BOCC ACTION: October 21, 2020, Agenda Item C. 3, Approval of
the Agreement with Guidance/Care Center for the Community Transportation for Disadvantaged
(CTD)program in the amount of$20,612.
September 16, 2020, Item H, the BOCC adopted annual operating budget for FY2021; budgeting
$41,225.00 for the CTD program.

GUIDANCE/CARE CENTER, INC. FOR
TRANSPORTATION SERVICES
THIS AMENDMENT is made and entered into this 201" day of January 2021, between the BOARD
OF COUNTY COMMISSIONERS OF MONROE COUNTY, FLORIDA, hereinafter referred to as "COUNTY,"
and Guidance/Care Center, Inc., a Florida 501(c)3 not-for-profit corporation, hereinafter referred to
as "PROVIDER".

WHEREAS, the PROVIDER is a not-for-profit corporation who provides transportation services
to the disadvantaged residents of Monroe County; and
WHEREAS, it is a legitimate public purpose to provide transportation services to the
disadvantaged residents of Monroe County; and
WHEREAS, the COUNTY and PROVIDER entered into an Agreement ("Agreement") on October c
21, 2020 for the PROVIDER to implement said services under the program; and
WHEREAS, the PROVIDER entered into an Agreement on July 1, 2020 with the State of
Florida Commission for the Transportation Disadvantaged (Contract No. G1018) for the provision of
transportation services and other responsibilities identified in Chapter 427, Florida Statutes, or rules 0)
thereof for Fiscal Year 2020-21; and
WHEREAS, on December 17, 2020 the PROVIDER and the State of Florida Commission for the `CD

Transportation Disadvantaged agreed that the July 1, 2020 Grant Agreement (Contract No. G1018) >`V-
is to be amended and supplemented as described in Amendment 1; and --
WHEREAS, an amendment is needed to reflect the changes to Contract No. G1018 as defined
in Amendment 1 between the PROVIDER and the State of Florida Commission for the Transportation v,
Disadvantaged;
NOW THEREFORE IN CONSIDERATION of the mutual promises and covenants contained herein, a
it is agreed as follows: y
1. Attachment D to the Agreement will be revised to incorporate Amendment #1 to the
Florida Commission for the Transportation Disadvantaged Trip & Equipment Grant
Agreement (Contract No. G1018) between the State of Florida Commission for the
Transportation Disadvantaged and Guidance/Care Center, Inc., which increases the total
project cost to $412,292.00 and local cash funds to $41,225.00, extends the completion
of the project and expiration of agreement from December 31, 2020 to June 30, 2021 and
replaces Exhibit "B", Project Budget and Disbursement Schedule (page 1 only) and Exhibit
"C" State Financial Assistance.
2. Section 1 FUNDING, Item 1., AMOUNT OF AGREEMENT, sub-section b. shall be
amended to reflect additional required local match funding:
b) The sum of FORTY-ONE THOUSAND TWO HUNDRED TWENTY-FIVE
DOLLARS AND NO/100 ($41,225.00), for Community Transportation
Coordinator-related transportation services to residents of Monroe CD

3. In all other respects the Agreement dated October 21, 2020 remains in full force and

I. PROJECT COST;
Estimated Project Cost shall conform to those eligible costs as indicated by Chapter 427, Florida Statutes,
Rule 41-2, Florida Administrative Code, the most current Commission policies and Fiscal Year 2020-21
Program Manual and Application Instructions for the Trip & Equipment Grant. Trips shall be purchased at
the fares indicated in Exhibit 8, Page 2 of 2.
Grantee shall invoice on a monthly basis actual costs that may be above or below (+/-) the amount of the
monthly allocation disbursement schedule reflected below. At times, this may result in "underbilling" or
"overbilling". Any excess(underbilled)funds may be recaptured on a monthly invoice that does not exceed the W
cumulative total of funds disbursed to date with supporting documentation. No excess funds remaining on the
grant at the end of the grant period will be reimbursed to the Grantee. Any overpayment of TD funds must be
repaid to the Commission upon project completion. Reimbursement payment for transportation services shall
not be provided to the Grantee until documentation supporting such services has been approved. In addition,
payment shall not be provided to the Grantee for capital until the capital has been received and proof of
payment and other back up documentation, as requested, is provided to the Commission.The project must be
completed (capital received and approved by the Grantee) no later than June 30, 2021. Grantee will make y
every effort to submit invoices within thirty (30) days after the month of service provision. >
