R B Leader File, MSTR APS—Monroe County Page I of 1 Exhibit A-Scope of Work (12/15-97) Monroe County Key West International Airport Rehabilitate Taxiway A-Amendment 1 Construction Support and RPR Services BACKGROUND Under PSO 12/15-60,Jacobs was contracted by Monroe County to provide construction support and RPR services for the Rehabilitation of Taxiway A.The services also included quality assurance testing, construction administration services,site visits from our pavement subject matter experts, and full-time construction inspection.The PSO 12/15-60 services were based on 200 calendar days of construction. A Notice to Proceed was issued to General Asphalt on April 51h, 2020. Based on the contract duration of 200 calendar days, the anticipated completion date was October 22nd 2020. Due to construction delays from weather and King Tides,General Asphalt was provided a time extension with a completion date of February 1 11h, 2021 to substantially complete all of the paving, electrical and pavement marking work associated with the Rehabilitate Taxiway A project. Jacobs was able to extend the project budget beyond the original tasked completion date of October 22nd,2020 to January 1 st, 2021.This amendment is for the extension of our services from January 1st,2021 through April 1st, 2021 as detailed below, SCOPE OF WORK The Rehabilitate Taxiway A project has experienced delays due to weather and King Tides. The scope of services for Amendment 1 wil I provide construction support and resident project representative (RPR) services from January 1 st, 2021 until April I st, 2021 for the "Rehabilitate Taxiway An project at Key West International Airport (EYW). In January and February, it is anticipated that General Asphalt will complete the paving of Taxiway A and the associated stub taxiways. General Asphalt will complete the second coat of pavement marking applications and miscellaneous punch list items in March. The scope will also include services for the RPR to re-mobilize for the REIL electrical panel relocation and REIL installation.This component of the project has been put on hold due to FAA's travel restriction. Since the REILS are a FAA owned facility,the FAA must be onsite when work is being done to it.The schedule of the REIL construction is dependent on FAA lifting their travel restriction so they can be onsite during the REILS work. The original Tierra South Florida and RDM budgets were based on four (4) asphalt paving mobilizations. Due to the delays as noted above General Asphalt was required to modify their Page 1 of 4 construction schedule and phasing to now have six (6) paving mobilizations. The amendments to their scope and fee include the two additional paving mobilizations. The services by Avirom &Associates were not required for the "Rehabilitate Taxiway A, project. The fees slated to Avirom in PSO 12/15-60 can be real located to Amendment 1.An explanation of the reallocation and proposed fee is shown below: Rehabilitate Taxiway A- $ 211,636.00 Amendment 1 Fee Reallocated Avirom & ($ 26,500.00) Associates Fee Total Amendment Fee $ 185,136.00 These construction services will provide the following: AssumFrIQNS ■ Construction phase services are based on continuing services from January 1, 2021 - March, 2021 for a total duration of 90 calendar days. It also includes an additional 30 calendar days for the REIL electrical panel relocation and REIL installation once the FAA travel restrictions are lifted. In the event the construction duration exceeds the calendar days listed above,Jacobs reserves the right to revise our proposal. ■ Jacobs will invoice on a percent complete, including expenses, to compensate the level of effort required for each assignment. ARTICLE D-CONSTRUCTION ADMINISTRATION SERVICES 1. Review the Contractor's construction schedule. 2. Respond to Contractor's RFI's. 3. Attend weekly progress meetings. Observe work in progress and provide reports to Owner. Assume 16 progress meetings throughout the course of the extended construction project. 4. Prepare Field Sketches, as necessary. 5. Provide general supervision and administrative support for resident project representative (RPR). 6. Review and approve estimates submitted by the Contractor for progress/final payments. 7. Prepare and negotiate change orders,as necessary. Assume two change orders. Page 2 of 4 Services for Article D will be provided under a lump sum basis. AMENDMENT TO ARTICLE E - RESIDENT PROJECT REPRESENTATIVE (RPR) 1. Continue to coordinate and attend progress meetings with the contractor,Airport, and any other interested parties, as required prior to construction (e.g. Staging area coordination, setting up Contractor's trailer.). 2. Continue to provide full-time RPR services for the work associated with the project. The RPR's for the project shall have field experience in the type of work to be performed, be fully qualified to make interpretations,decisions,field computations,and have knowledge of testing requirements and procedures. The RPR's shall be responsible for the following: • Checking of construction activities to ensure compliance with the plans and specifications. Inform the contractor and County of any work,which is in non- compliance. • Coordinate quality assurance tests and verify they have been performed at the frequency stated in the specifications. • Review certifications for conformance with the specifications, • Document quantities of materials used on the project by actual measurements and computations in a field notebook or computer printout retained in a folder. • Maintain a set of working drawings on the job site,which can be used to prepare"As- Built"drawings. • Review the test reports and certifications for conformance with the specifications and maintain a file for all reports and certifications. • Inform the County of deficiencies so corrections can be made and retesting performed. • Jacobs does not have control over or charge of and shall not be responsible for construction means, methods,techniques,sequences or procedures of construction or for safety precautions and programs in connection with the work,since these are solely the Contractor's responsibility under the contract for construction. • Jacobs shall make every reasonable effort to ensure that the Contractor completes the work in accordance with the current approved schedule and carries out the work in accordance with the contract documents. Page 3 of 4 • Review close out documentation. • Review payment requests and certified payrolls from the contractor. • Maintain a diary,which will contain entries made and signed by the resident engineer. Each entry should include the following, plus any additional pertinent data: a. Date and weather conditions. b. Names of important visitors. c. Construction work in progress and location. d. Size of contractor's work force and equipment in use. e. Number of hours worked per day for contractor and subcontractors. • Record drawings will be prepared, in part, on the basis of information compiled and furnished by others,and may not always represent the exact location,type of various components,or exact manner in which the project was finally constructed. Consultant is not responsible for any errors or omissions in the information from others that is incorporated into the record drawings. 3. Provide full-time RPR services for the relocation of the REIL electrical panel and installation of the Runway 27 RElLs.The RPR shall also be responsible for the items listed in line item 3, 4. Quality Assurance Testing of Construction Materials will be provided by our subconsultant, Tierra South Florida.An amendment to Tierra South Florida's original fee to extend their Quality Assurance Testing is included with this scope. 5. RDM will provide review of the P-401 job mix formula, attend the pre-paving meeting,test strip,and make site visits during paving operations. RDM will assist in reviewing QC results and provide input into the quality requirements for off-peak construction.An amendment to RDMIs original fee to continue their services is included with this scope. Services for Article E will be provided under a lump sum basis.
