ITEM BACKGROUND: The General Services Administration requested approval of the SLA No. 1 as
part of its data merge with the CCR.gov database. This is a house -keeping item updating the mailing address
listed on the lease.

THIS AGREEMENT, made and entered into this date by and between County of Monroe, whose address is
1100 Simonton Street

Key West, Florida 33040-3110
hereinafter called the Owner or Lessor, and the UNITED STATES OF AMERICA, hereinafter called the Government:
WHEREAS, the parties hereto wish to amend the Lease to update Lessor's correct address and to insert a 9-digit zip code; and
WHEREAS, the Government wishes Lessor to provide correct and current information regarding CCR.gov;
NOW THEREFORE, the parties for the consideration hereinafter stated covenant and agree that the said Lease is hereby
amended, effective as of the date hereof, as follows:
1. Part II, Paragraph 10a of the Form 3626 of the Lease is hereby deleted and the following is inserted in lieu thereof:
"10a. Name and Address of Owner,
County of Monroe
1100 Simonton Street
Room 2-205
Key West, Florida 33040-3110
2. Owner's address in any prior SLA is hereby deleted and the address set forth at Paragraph 1. above is inserted in lieu thereof.
3. Lessor represents that the correct U.S. Mail address currently on file with CCR.gov is:

4. Lessor represents that the correct DUNS number currently on file with CCR.gov is: 073876757
5. Lessor understands that knowingly providing false or misleading information may violate Title 18, Section 1001 of the US Criminal Code.
6. All other terms and conditions of the Lease remain in full force and effect.

Good Afternoon Mr. Horton, Ms. Pearson,
The General Services Administration is preparing for a data merge between its databases
and the CCR database. Leases that contain conflicting data have been flagged and I am
contacting the Owners/Lessors in order to get that information corrected prior to the merge
event.
I spoke with Lisa Tennyson earlier, who verified the information Monroe County has registered
in CCR is accurate. However, the data within GSA records reflect the airport address rather
than the Lessor/Owner address. In order to correct the information in our records, I prepared
a Supplemental Lease Agreement that reflects the correct data as it is listed in the CCR
database.
Please review the information for accuracy, and let me know if there are any discrepancies.
If the information is correct, please have the person authorized to execute documents under
this lease sign the SLA, then scan or fax the SLA to me at your earliest opportunity. I will have
the SLA signed by the Contracting Officer and return a copy to you for your
records. Because of the short timeframe we have to make all of these corrections, GSA is
not requiring original signatures on SLAs for this project.
Please make every effort to expedite this request because the deadline is approaching
quickly. As I have stated, GSA is preparing for a data merge. If GSA data does not match
CCR data, the lease will be flagged and all rental payments will stop. It is imperative that
you process this SLA ASAP.
