PROJECT MANAGEMENT CO. ("Consultant"), dated February 15, 2017 ("Agreement'). The
Agreement is incorporated herein and forms an integral part of this Task Order.

Taxiway A Extension—Supplemental Survey
SCOPE OF WORK
Jacobs was contracted through Task Order No. 12/15-94 to provide permitting,design,and
bidding services for the extension of Taxiway as shown in Figure No. 1. The extension of Taxiway A
will result in environmental impacts,which will require the creation of environmental mitigation
areas utilizing two identified offsite areas.
The original scope of services included the survey to two offsite parcels. These parcels were
identified as Tarpon Belly Key and a property that was for sale at Spain Boulevard in Key West,FL.
The Tarpon Belly Key property was approved by the Board of County Commissioners to be
acquired by the Key West International Airport. The other property at Spain Boulevard was not
able to be acquired by the Airport as it was already under agreement to be purchased by others.
A third property was identified to replace the previously identified Spain Boulevard. The property
known as the Hawk Missile Site,which is owned by the City of Key West,was identified to provide
the necessary mitigation to offset the scheduled improvements at Key West International Airport.
Airport management and directed Jacobs to proceed with the inclusion of this new parcel to be
part of the mitigation approach.
The change in properties has resulted in an increase of the scope of work for the topographical
and wetland survey for the offsite mitigation area. The below is a discussion of the additional
survey services.
SCOPE OF SERVICES
HORIZONTAL&VERTICAL CONTROL
The Jacobs team will establish new control required for this survey.All control will be
referenced to North American Datum 1983(NAD83)(2011 Adjustment)and North
American Vertical Datum 1988(NAVD88).
TOPOGRAPHIC SURVEY
The Jacobs team will provide a Topographic Survey of the project site as shown on the
attached Exhibit"A"(obtained from the client).
The attached Exhibit"A"depicts two separate areas outlines in red(6.1 total acres).
Tasks for this proposal include:
• Pavement and Ground Elevations measured on a 50-foot grid
•Survey grade break lines within limits of survey
•Vegetation and Tree Lines within survey limits
•Driveways and roadways
•Specimen Trees Only 30">DBH
•Wetlands within the project limits as delineated by others.
Providing location of visible improvements including visible foundations,buildings,
drainage,fencing,etc.(See NOTE below).
NOTE:These site(s)are within an abandoned/deLipidated industrial site with extensive
perimeter berms,numerous concrete and steel support structures,buildings,wood
pilings,drainage,fencing and temporary stockpiles of shell,time rock and clay materials.
Buildings will be located as an envelope(typically a 4 corner rectangle)with type
description and height.
Temporary material stockpiles will be described and located as enveloped areas with
minimal points located atop piles to depict average top height of piles within each
enveloped areas.Any drainage within limits will include full data on accessible pipes or
culverts including size,apparent condition,invert and bottom of structure elevations and
any immediate connecting drainage outside limits.
Total Survey Areas: 6.1 acres
BOUNDARY SURVEY(OPTIONAL)
The OSM team will provide a Boundary Survey of the two project sites as shown on the
attached Exhibit"A". Boundary survey includes monuments set at all corners of the two
mitigation sites as required by Florida Statutes.(See NOTE under SCHEDULE below)
The Jacobs Team will locate wetlands on both the subject sites(as marked by others).
This will be dependent upon delineation performed and completed as scheduled.
ASSUMPTIONS/DESIGN PARAMETERS
1. All work will be performed pursuant to the Master Agreement for Professional Services
between Project Management Co.and Monroe County, FL agreement signed between the
parties on February 15,2017.

O'Neal Surveying and Mapping, Inc. (OSM) is pleased to provide you with a scope of services for the Key
West Airport Mitigation Site Survey at the former Hawk Missile Base, Key West, Florida. The project and
services include the following:

*NOTE: This survey is to be performed in conjunction with two current OSM survey projects at the
proposed EYW TW A Extension and at a proposed mitigation site at Tarpon Belly Keys. The field
work for this project is scheduled to be performed by March 12, 2021 (weather permitting) with
Notice to Proceed to OSM on or before Feb. 26, 2021.
NOTE: OSM will locate wetlands on both the subject sites (as marked by others). This will be dependent
upon delineation performed and completed as scheduled (March 2-3, 2021).
*NOTE: OSM MUST receive Notice To Proceed with Optional Boundary Survey during current crew
deployment and schedule (on or before March 5, 2021).
WORK ZONE SAFETY:
All work zone safety, rules and procedures as required will be observed and strictly adhered to by OSM.
Survey activity will be coordinated with Jacobs staff prior to and during commencement.
QUALITY CONTROL/QUALITY ASSURANCE AND FIELD REVIEW:
Verification of the field conditions as related to the collected survey data prior to any survey data submittal.
All activities required to supervise and coordinate the survey project will be performed by the OSM project
supervisor, a Florida P.S.M. or their delegate.
OSM will implement their QA/QC plan to include review, response to comments and any resolution
meetings if required, preparation of submittals for reviews, etc.
Deliverables will include the ACAD Civil 3D digital file as well as signed and sealed copies (or certified
digitally signed/sealed copy) of the survey.
Any change in the above stated scope or schedule may result in additional fees to be determined.
