I. I, Tammv Smithson. a sinale woman, the undersigned is the sole owner
of certain real property, situated, lying and being in Monroe County, State

I. The residential unit Building Permit Number is 99-1-3220
II. Under the provisions set forth in Chapter 9.5 of the Monroe County Code,
any person, including any governmental agency, prior to receiving a
building permit for any new rand development activity shal pay "Fair Share
Impact Fees".
III. Under the affordable housing provisions set for in Monroe County Code
Chapter 9.5, the owner or owners of the above-described real property
have been exempted from payment of "Fair Share Impact Fees" for a
(check one): a single-family -LL, a multi-family unit, _' a mobile home
_ to be constructed on said real property.
IV. The applicant(s) have used the affordable housing program to gain
additional points in the pennit allocation system pursuant to Monroe
County Code Section 9.5-122.3(6).
V, The covenant shall be effective for twenty (20) years, but shall not
commence running until a certificate of occupancy has been issued by the
building official for the dwelling unit to which the covenant apply,
VI. A change of ownership or tenancy requires re-qualification as required by
Monroe County Code Chapter 9.5 as amended from time to time,
VII, All of the restrictions herein shall be binding upon any transferees,
lessees, heirs or assignees. /.//
